BACKGROUND: The «gold standart» for surgical treatment of patients with proliferative diabetic retinopathy (PDR is vitreoretinal surgery. However, the question of the timing of the removal of primary cataract in this category of patients remains open.AIM: To evaluate the efficacy of phacoemulsification of primary cataract by the second stage after vitreoretinal surgery of PDR patientsMETHODS: 67 cases of surgery treatment of patients with PDR and complicated primary cataract were enrolled. This patients were divided into two groups depending on the treatment tactics. In the 1st group patients were subjected to a two–step surgical procedure: vitreoretinal surgery with silicone oil tamponade performed as the 1st step in their treatment; followed by the 2d step, phacoemulsifi tion surgery and silicone oil removal, and the IOL implantation, respectively. In the 2d group phacoemulsifi tion performed simultaneously with vitreoretinal surgery: phacoemulsification, IOL implantation, vitreoretinal surgery with silicone oil tamponade. The second step differed in the removal of silicone oil from the vitreous cavity.RESULTS: Visual functions improved in 88.6% of cases in group 1, and in 68.7% in group 2.CONCLUSIONS: Outcomes of the preliminary studies suggest that it is more viable to perform phacoemulsification surgery sometime later along with silicone oil removal on PDR patients with complicated primary cataract. This sequence of treatment procedure ensures a more gentle approach to the anatomic structures of the eye during the first stage (vitreoretinal surgery) and contributes to the reduction in the number of intraoperative and postoperative complications.
РЕЗЮМЕПо данным различных исследований, приверженность к лечению пациентов с глаукомой колеблется от 5 до 80% и существенно влияет на эффективность терапии. Ряд авторов указывают на существенное повышение приверженности у пациентов, получающих бесконсервантную терапию. Цель исследования -изучить влияние приверженности к лечению у пациентов, получающих бесконсервантные и консервантсодержащие препараты, на течение первичной открытоугольной глаукомы (ПОУГ). Материал и методы. Многоцентровое проспективное неинтервенционное исследование проходило в условиях клинической практики и охватывало 504 пациента, которые получали медикаментозную терапию ПОУГ. Все пациенты проходили подробное анкетирование. Врачами (51 человек) заполнялись дневники пациента. Результаты. Среди пациентов с высокой приверженностью к лечению доля тех, у кого не был достигнут целевой уровень внутриглазного давления (ВГД), оказалась в 3 раза меньше по сравнению с таковой в группе низкой и средней приверженности (7 против 22% соответственно). На приверженность к терапии в числе прочих причин влияет развитие побочных эффектов, количество и выраженность которых оказались существенно ниже в группе бесконсервантной терапии (13,9 против 35,4% в группе консервантной терапии). При этом доля пациентов, у которых не был достигнут целевой уровень ВГД, была в 2 раза ниже в группе бесконсервантной терапии (10 против 21% в группе терапии консервантсодержащими препаратами). Заключение. Настоящее исследование подтвердило, что бесконсервантная терапия глаукомы в условиях клинической практики способствует повышению приверженности к лечению преимущественно за счет снижения количества и выраженности нежелательных явлений, приводящих к пропускам закапываний и отказу от лечения, что способствует повышению эффективности терапии и стабилизации глаукомного процесса.Ключевые слова: глаукома, приверженность к лечению, бесконсервантная терапия, побочные эффекты.
The widespread introduction of intravitreal drug injections into clinical practice has highlighted a number of issues to be solved, including the risk of serious complications, the treatment burden on patients and the patient’s insufficient adherence to treatment, as well as a significant effect on the healthcare cost and effort. One of the ways to resolve the issues is to increase the patients’ awareness concerning the therapy they are about to receive, which contributes to the patients’ understanding of the details of treatment and enables them to make correct decisions. The best option of presenting such information to the patient is to provide them with a written document. The Expert Council on Retinal and Optic Nerve Diseases of the All-Russian Association of Ophthalmologists has developed, approved and tested an information brochure for patients who receive intravitreal injections. It offers brief information on what the procedure is, contraindications, details of preparation, and symptoms, both common ones and those requiring emergency assistance. The paper outlines the results of the study and presents a layout of an information leaflet for patients recommended for use in the Russian Federation.
