Пароксизмальной или персистирующей формой фибрилляции предсердий (ФП) страдают 2,3 млн жителей США и 4,5 млн-стран Евросоюза. Ее частота в популяции составляет от 0,4 до 1,5-2%, увеличиваясь с возрастом и достигая в возрастной группе старше 80 лет 6-8% [1-6]. Отмечается существенный рост распространенности и встречаемости ФП. Так, за последние 20 лет частота госпитализаций по поводу ФП увеличилась на 66% [2], а к 2050 г., как ожидается, частота ФП составит более 2% [5-9]. Лечение больных с ФП является актуальной и сложной задачей клинической кардиологии и фармакологии, имеющей существенное социальное значение, поскольку ФП способствует развитию и прогрессированию сердечной недостаточности, служит причиной 15-20% инсультов, является независимым предиктором смерти [10-13]. На современном этапе лечение ФП основывается на рекомендациях Американской Коллегии Кардиологов/Американской Ассоциации Сердца/Европейского Общества Кардиологов (АСС/АНА/ESC) по ведению пациентов с ФП 2006 г. [2], Европейского Общества Кардиологов (ESC) по лечению ФП [6] и рекомендациях ВНОК по диагностике и лечению ФП [5,14]. Однако, несмотря на выход рекомендаций по терапии ФП, зачастую назначение антиаритмических препаратов (ААП) носит эмпирический характер [13,15-17]. Освещены современные данные и перспективы антитромботической терапии у больных фибрилляцией предсердий (ФП). Приведены данные последних отечественных и зарубежных рекомендаций по профилактике тромбоэмболических осложнений при ФП. Отдельно рассмотрены новые препараты для проведения оральной антикоагулянтной терапии, представлены сведения об их эффективности, безопасности и возможностях использования.
