IntroductionThe success of the health technology assessment (HTA) implementation depends on the level of communication efficiency between all stakeholders. Taking this into consideration, the leading HTA experts of HTA Department of State Expert Center of the Ministry of Health of Ukraine launched the new educational initiative which includes training programs for pharmaceutical companies.MethodsСomprehensive review of the first developed HTA training programs based on the HTA Guideline (Number 593) “The state health technology assessment for medicines” approved on 29 March 2021 was conducted.ResultsLeading HTA experts of the HTA Department developed a training program, which was conducted for industry representatives according to the provisions of the HTA Guideline. In 2021 over twelve training sessions and two webinars were conducted. These learning events reflect harmonized international recommendations and approaches to HTA training programs, The Professional Society for Health Economics and Outcomes Research, ISPOR short courses in particular, European Network for Health Technology Assessment (EUnetHTA), The National Institute for Health and Care Excellence (NICE), Institute for Clinical and Economic Review (ICER), Agency for Health Technology Assessment and Tariff System (AOTMiT) guidelines. Two-hour webinars were aimed at giving theoretical and practical bases for building a Markov model with the help of Excel and TreeAge Software. Training sessions titled “HTA as a tool for assessing the value of health technology” differ by duration (7-hour, 4-hour, 3-hour) and cover core topics adapted to the needs of the audience. The main program components include HTA Roadmap in Ukraine, clinical section, economic section and practical case studies of building a Markov model. All training options included questionnaires at the beginning and i end to assess the quality of each program. Questionnaires are the tool that gives presenters the possibility to trace progress and transform the training material accordingly.ConclusionsDevelopment of extended capacity building programs in HTA for users and doers is highly prospective for further steps in HTA institutionalization in Ukraine.
IntroductionThe Health Technology Assessment (HTA) Department of the “State Expert Center of the Ministry of Health (MoH) of Ukraine” was established in 2019 for conducting assessments and development of recommendations for informing decisions for the Ministry of Health of Ukraine (MoH) regarding the financing of health technologies based on HTA. The study aimed to conduct cost analysis and compare the treatment costs of two available dosage forms of tocilizumab for subcutaneous and intravenous administration for systemic juvenile idiopathic arthritis in Ukraine. Currently there is a central procurement program financed by the state budget with pharmaceuticals provided to patients with juvenile idiopathic arthritis approved by the Cabinet of Ministers Decree of Ukraine №255 dated 13.03.2019.MethodsThe cost analysis was carried out for tocilizumab over a 1-year horizon per one patient. The analysis included drug manufacturers' prices from the registry of the MoH dated 19.09.2019. The annual number of tocilizumab vials for intravenous infusion for each weight category of patients is approved by the order MoH №334 from 14 February 2019. The direct medical costs were included in the analysis omitting cost of administration.ResultsThe direct medical costs of treatment with intravenous tocilizumab per one patient for one budgetary year ranged from between USD7,563.83 and USD30,255.30 depending on patient's weight that was in the range of 10–80 kg. The direct medical costs of treatment per one patient for one year with subcutaneous tocilizumab was USD7,782.40 for patients < 30 kg and USD15,564.80 for patients ≥ 30 kg.ConclusionsThe introduction of subcutaneous tocilizumab can potentially lead to cost savings on average USD 4,041.97 (34.2%) for patients < 30 kg and USD 5,245.82 (25.2%) for patients ≥ 30 kg per one patient for 1-year treatment compared to the intravenous route. Intravenous tocilizumab has an economic advantage over the subcutaneous route solely for the pediatric population of certain weight categories. The cost of intravenous tocilizumab was USD 218.58 lower (2.9%) for patients ≤ 13 kg and USD 2,320.25 lower (17.5%) for patients with weight 31–35 kg, compared to the cost of subcutaneous tocilizumab.
ЦЕЛЬ: оценка доклинических нарушений иннервации и кровоснабжения стоп у больных сахарным диабетом для выявления в амбулаторных условиях лиц группы риска развития синдрома диабетической стопы МАТЕРИАЛЫ И МЕТОДЫ: обследованы 60 больных сахарным диабетом 2 типа (СД 2) -1-я группа (возраст 58±5,2 лет) и 40 человек без нарушений углеводного обмена -2-я группа сравнения (возраст 54±6,7 лет). Выясняли жалобы, анамнез, проводили клиническое обследование стоп, включающее оценку чувствительности, состояния кровотока на a. dorsalis pedis, a. tibialis posterior, a. poplitea. Критериями исключения из исследования были: деформации, язвенные и другие дефекты стоп, любой этиологии. У обследованных лиц определяли индекс массы тела, плече-лодыжечный индекс; тактильную чувствительность (ТЧ) посредством монофиламента, болевую чувствительность (БЧ) -при помощи зубчатого колеса, вибрационную чувствительность (ВЧ) -камертоном С-128; степень сатурации кислородом пальцев стоп -пульсоксиметром, исследовали метрику стопы, индекс Штритера; плантографию для измерения конфигурации и площади подошвы.РЕЗУЛЬТАТЫ: анкетирование обследованных обеих групп позволило убедиться в отсутствии у них клинических нарушений иннервации стоп (парестезий, боли, снижения чувствительности и пр.). Индекс массы тела в обеих группах был сопоставимым (35,6±5,2 кг/м 2 vs 34,3±6,5 кг/м 2 ). В 1-й и 2-й группах значения плече-лодыжечного индекса достоверно не различались (0,8±0,2 vs 1,1±0,3). Сатурация кислорода в пальцах стоп в 4 раза чаще снижалась до 94 в 1-й группе по сравнению со 2-й группой (50% vs 13%) При оценке ТЧ по шкале NDS (Neuropathic disability score) в группе больных СД 2 средний показатель составил 0,85 баллов. Средние значения БЧ по шкале NDS составили: у 20% больных СД 2 он равен 0 баллов (норма) на обеих стопах, у 40 % пациентов -1 балл (отсутствие чувствительности до основания пальцев), у 15% больных -2 балла (до середины стопы), у 25% пациентов -3 балла (отсутствие чувствительности до середины лодыжки). Во 2-й группе БЧ находилась в пределах референтных значений. Средний показатель ВЧ у больных СД 2=4,9±1,1 сек (средняя степень нарушения); у 15% больных ВЧ оказалась патологически высокой, а у 75% пациентов -патологически низкой. ВЧ во 2-й группе находилась в пределах референтных значений для здоровых лиц. (9,8±1,9 сек. Индекс Штритера в 1-й группе составил 49,7, а в группе сравнения -43,5. В группе больных СД 2 уплощенная стопа выявлена в 42% наблюдений, а в группе сравнения плоскостопие обнаружено лишь в 6 % случаях. У 37% больных СД 2 с плоскостопием одновременно выявлены: нарушение вибрационной и болевой чувствительности, снижение сатурации кислородом пальцев стопы.ВЫВОДЫ: у каждого третьего обследованного больного СД 2 без клинических проявлений нарушений иннервации и кровоснабжения стоп выявлены нарушения вибрационной и болевой чувствительности с одновременным развитием плоскостопия и снижением сатурации кислорода, что позволяет предположить наличие общего объединяющего их механизма. Возможно, что эти показатели помогут в амбулаторных условия...
