РезюмеОстеопороз -системное заболевание скелета, характеризующееся снижением костной массы и нарушением ее качества (микроархитектоники), что приводит к хрупкости костей, которая проявляется переломами при незначительной травме. Количество больных остеопорозом неуклонно увеличивается из-за старения общества. Остеопороз -чрезвычайно распространенное заболевание: в мире около 200 миллионов человек, страдающих остеопорозом, и приблизительно 8,9 миллионов переломов являются низкотравматичными. В России среди лиц в возрасте 50 лет и старше остеопороз выявляется у 34% женщин и 27% мужчин. Социальная значимость остеопороза определяется его последствиями -переломами костей периферического скелета и тел позвонков, что приводит к высоким материальным затратам в области здравоохранения и обусловливает высокий уровень нетрудоспособности, инвалидности и смертности. Нормальный физиологический процесс костного ремоделирования кости включает в себя баланс между резорбцией кости и костным формированием. При остеопорозе этот процесс становится несбалансированным, что приводит к постепенным потерям костной массы и плотности из-за усиленной костной резорбции и / или недостаточного костного формирования. Было идентифицировано несколько сигнальных путей, лежащих в основе первичного остеопороза -это остеопротегерин/рецептор ядерного фактора B (RANK)/ RANK лиганд (RANKL), костные морфогенные белки, канонический wnt-сигнальный путь. Кроме того, в развитии патогенеза остеопороза участвуют генетические нарушения. Для выявления остеопороза ВОЗ рекомендует применение двухэнергетической абсорбционной рентгеновской денситометрии, которая позволяет изучить количественные характеристики костной ткани. В настоящее время существуют различные методы оценки качественных характеристик кости (микроархитектоника, способность костной ткани быть устойчивой к перелому), однако эти методы имеют ограничения, такие как высокая стоимость и ограниченная доступность для их широкого использования. Изучение маркеров костного ремоделирования в норме и при патологии помогает косвенно оценить качество костной ткани, дает перспективы в подборе таргетной терапии и улучшения ранней диагностики остеопороза. Остеопороз и остеопатии / Osteoporosis and Bone Diseases | 82 REVIEW ВВЕДЕНИЕОстеопороз и сахарный диабет имеют на сегодняш-ний день высокую медико-социальную значимость, что обусловлено высокой распространенностью данных заболеваний, снижением качества жизни пациентов, по-вышением инвалидизации и смертности.Длительное время сахарный диабет и остеопороз рассматривались как отдельные заболевания. Предыду-щие исследования показали, что пациенты с сахарным диабетом 1 типа (СД1) и 2 типа (СД2) имеют повышенный риск развития остеопоротических переломов [1][2][3]. По-этому остеопороз в последнее время считается одним из диабетических осложнений в связи со снижением прочности кости, развивающимся на фоне сахарного ди-абета. Так, по данным метаанализа 2007 года пациенты с СД1 типа имели более низкую минеральную плотность костной ткани (МПК) поясничного отдела позвоночника и бедра (Z-критерий-0.22SD и -0.37SD, соответственно), у пациентов с СД2 были более высокие показатели МПК поясничного отдела позвоночника и бедра (Z-критерий +0,41SD и +0,27SD, соответственно). Согласно значени-ям МПК, оценочные риски переломов имели 1,42-крат-ное увеличение при СД1 и 0,77-кратное уменьшение при СД2. Однако риск перелома шейки бедра по срав-нению с контролем без диабета были увеличен в 6,9 раз Накопление конечных продуктов гликирования, гипергомоцистеинемия увеличивают апоптоз остеоцитов, уменьша-ют костеобразование и замедляют костное ремоделирование, при СД. Адипонектин стимулирует экспрессию осте-окальцина и дифференцировку остеобластов посредством активации 5'АМФ-активируемой протеинкиназы (AMФK). AMФK-активация стимулирует дифференцировку и минерализацию остеобластов. Гипоадипонектинемия, которая часто наблюдается при ожирении и диабете, может участвовать в снижении прочности кости при СД.Замедленный костный метаболизм у пациентов сахарным диабетом подтверждается при изучении уровня маркеров костного ремоделирования. BACKGROUND: Deterioration of bone tissue in type 2 diabetes mellitus (T2D): lead to the increased bone brittleness and to higher risk of low-energy fractures. AIM: to study serum levels of sclerostin and cathepsin K in assessing bone metabolism in patients with type 2 diabetes mellitus. MATERIAL AND METHODS: 102 postmenopausal women aged from 46 to 67 years were examined. All patients were divided into 4 groups: the first group included 39 patients with type 2 diabetes (T2DM) and postmenopausal osteoporosis (PO), the second group — 25 patients with PO without T2DM, the third group included 21 patients with T2DM but without PO, and the fourth group (control) — 17 people. Patinets of all groups were tested for ionized calcium, phosphorus, total alkaline phosphatase (ALP), parathyroid hormone, 25 (OH) vitamin D, bone mineral density in groups I, II, and IV, levels of sclerostin and cathepsin Kin seru, were also obtained. RESULTS: No statistically significant differences have been observed between groups in sclerostin levels, a positive correlation was found between sclerostin and НbА1с (r=0.43; p=0.009) in the group of patients with T2DM and PO, a negative correlation was found between sclerostin and ionized calcium (r=–0.45; p=0.037) in the group of patients with PO. Cathepsin C in the first group was lower than in the second group (p=0.046), but taking into account Bonferroni correction this difference was not statistically significant. In the first and third groups, 25 (OH) vitamin D was lower than in the groups without T2D. The ALP negatively correlated with the duration of the postmenopause (r=–0.39 and r=–0.64; р=0.05, respectively). CONCLUSIONS: Cathepsin С was lower in patients with T2DM2 and PO, which may indirectly indicate a decreased bone resorption. Concentration of sclerostin, which plays a key role in the mechanism of inhibiting osteoblastogenesis, positively correlated with НbА1с.
Osteoporosis is a metabolic disease of the bone tissue, which is characterized by a violation of the qualitative (microarchitecture change) and quantitative characteristics (decreased bone mass) of the skeleton, and, as a result, lowenergy fractures. The medical and social significance of osteoporosis is due to its consequences: fractures of the femoral neck, vertebral bodies, radius and other large bones of the skeleton, which significantly reduces the quality of patients’ lives and entails significant financial costs for treatment and rehabilitation. Prevalence of diabetes mellitus (DM) significantly increases in all countries of the world, including the Russian Federation. Diabetes and osteoporosis were considered to be individual diseases for a long time. In many prospective large-scale studies it was proved that patients with T2DM have a greater risk of falls and fractures in contrast with the overall population. Despite the increased risk of fractures in diabetic patients, there is still no risk stratification in these patients. The link between the classic risk factors of osteoporotic fractures (such as chronic intake of glucocorticoids, use of alcohol, tobacco, etc.) and the occurrence of fractures is similar in patients with both T2DM and without it. However, it is necessary to take into account other characteristics of diabetes. The specific risk factors of fractures in T2DM patients are duration of the diabetes for more than 10 years, diabetes decompensation, the severity of diabetes, obesity, the frequency of falls, hypoglycemia, insulin, thiazolidinedione, exenatide, sulfonylureas, canagliflozin therapy, and, most likely, the reduction of 25 (OH) vitamin D, functional hypoparathyroidism, the increase of sclerostin, pentosidine, decreased bone mineral density (BMD) of the hip.
Parathyroid hormone (PTH) regulates the maintenance of serum calcium concentration in strict limits through direct effects on bones and kidneys and indirectly due to the effect on the gastrointestinal tract. PTH also regulates phosphorus metabolism. Secondary hyperparathyroidism develops in response to a decreased serum calcium and vitamin D levels, leading to an increased bone resorption. However, the increase in parathyroid hormone above the reference values is not observed in all cases of vitamin D deficiency or hypocalcemia. Supressed or inadequately normal PTH in these conditions is referred to as functional hypoparathyroidism. Various theories have been suggested to explain the functional hypoparathyroidism: magnesium deficiency, intestinal calcistat, lower reference values for plasma PTH compared to current cut off interval, biological variations of vitamin D-binding protein. However, at present none of these theories are generally accepted. The clinical significance of functional hypoparathyroidism may be that vitamin D deficiency, hypocalcemia, and hypomagnesemia are associated with a risk of fracture, regardless PTH level.
Background: Acromegaly is a severe neuroendocrine disorder caused by chronic overproduction of growth hormone and insulin-like growth factor 1 and associated with a variety of clinical manifestations, debilitating complications and progressive disability, if no biochemical control has been achieved. The Russian registry of hypothalamic-pituitary tumors allows for evaluation of the epidemiological, demographic and clinical characteristics of patients with acromegaly, as well as of the effectiveness of various treatment methods. Aim: To identify epidemiological and clinical characteristics and treatment efficacy for acromegaly in the patients in the Udmurt Republic. Materials and methods: We performed a cross-sectional study based on the registry of hypothalamic-pituitary tumors of the Udmurt Republic as per June 2022. It contains data from 77 patients with acromegaly. The study included 59 (76.62%) patients with the information sufficient for evaluation of the disease stage and its complications. Results: According to the registry of hypothalamic-pituitary tumors, the prevalence of acromegaly in the Udmurt Republic was 6.71 cases per 100,000 of the population. Patients with acromegaly in Udmurt Republic had a high prevalence of arthralgia, abnormally large feet and hands, weakness/fatigue, snoring / sleep apnea, increased sweating, dizziness, peripheral edema, obesity, menstrual cycle abnormalities, diabetes mellitus, nodular goiter, pathological fractures and hypopituitarism. Neurosurgery has been performed in 74.57% (n = 44) of the patients, with a remission of acromegaly achieved in 18 (40.9%). Medical treatment was given to 25/59 (42.37%) of the patients, with 22 of them being treated with 1st generation long-acting somatostatin analogues. Radiation therapy has been administered to 13 patients (22.03%). Biochemical control was achieved in 40/59 (67.8%) of patients with acromegaly, related to neurosurgery in most patients, as well as to the use of a growth hormone receptor antagonist and/or combined medical treatment. Conclusion: The prevalence of acromegaly in the Udmurt Republic is consistent with international data. Clinical characteristics of acromegaly generally correspond to the classic disease course, although a number of symptoms are more commonly found in the Udmurt Republic patients than in the Russian Federation registry; this might be related to the quality of the registry management. The effectiveness of treatment for acromegaly in Udmurt Republic is comparable to the international data.
