We studied mobilization of bone marrow stem cells the under conditions of boost-therapy with granulocytic CSF and erythropoietin. Immunophenotyping of the peripheral blood was carried out in patients after transplantation of tissue-engineered trachea during the perioperative period. We observed an increase in hemopoietic stem cell pool and appearance of multipotent mesenchymal stromal cells in systemic circulation, their maximum concentration being recorded on days 10-14 after surgery. These data indicated a positive effect of growth factors on mobilization of stem cells.

We studied the properties of a tissue-engineered trachea consisting of a polyethylene terephthalate scaffold populated with autologous bone marrow mononuclear cells. The tissue-engineered constructs were obtained before surgery, during the postoperative period, and during autopsy. Cytomorphological analysis during the postoperative period showed the presence of mesenchymal stem cells on the inner surface of the implant on day 3 after surgery and cells of the respiratory epithelium on day 10-14. In autopsy samples, single epithelial cells, endothelial cells, and basal cells were found. Biocompatibility of the tissue-engineered trachea with autologous mononuclear cells of the patient was demonstrated.

Objective: to analyze the in-hospital and long-term outcomes of classical carotid endarterectomy (CEE) in extended atherosclerotic lesions in comparison with the outcomes of this operation in local atherosclerotic plaque (AP). Materials and Methods. This study, which lasted from January 2010 to December 2020, included 148 patients with extended AP and hemodynamically significant internal carotid artery (ICA) stenosis. The term “extended” was understood as a hemodynamically significant lesion ≥ 5 cm long. These patients made up Group 1. Group 2 was formed over the same period of time from 632 patients with hemodynamically significant stenosis <5 cm long. In both cohorts, CEE with repair of the reconstruction zone with a diepoxide-treated xenopericardial patch was performed. Long-term follow-up was 71.4 ± 45.6 months. Results. The groups were comparable in terms of frequency of in-hospital complications: death (group 1: 0.67%, n = 1; group 2: 0.5%, n = 3; p = 0.74; OR = 1.42; 95% Cl 0.14-13.6), myocardial infarction (MI) (group 1: 0.67%, n = 1; group 2: 0.5%, n = 3; p = 0.74; OR = 1.42; 95% CI 0.14-13.6), ischemic stroke (group 1: 0%; group 2: 0.5%, n = 3; p = 0.91; OR = 0.6; 95% CI 0.03-11.8), combined endpoint (death + MI + stroke) (group 1: 1.35%, n = 2; group 2: 1.4%, n = 9; p = 0.74; OR = 0.94; 95% CI 0.2-4.43). The groups were also comparable in terms of frequency of long-term complications: death (group 1: 2.0%, n = 3; group 2: 2.05%, n = 13; p = 0.76; OR = 0.98; 95% CI 0.27-3.5), MI (group 1: 2.7%, n = 4; group 2: 2.4%, n = 15; p = 0.95; OR = 1.14; 95% CI 0.37-3.49), ischemic stroke (group 1: 5.4%, n = 8; group 2: 5.2%, n = 33; p = 0.9; OR = 1.03; 95% CI 0.46-2.29), ICA occlusion and restenosis (group 1: 12.8%, n = 19; group 2: 13.3%, n = 84; p = 0.99; OR = 0.96; 95% CI 0.56-1.63), combined endpoint (death + MI + stroke) (group 1: 10.1%, n = 15; group 2: 9.6%, n = 61; p = 0.98; OR = 1.05; 95% CI 0.58-1.91). Analysis of survival graphs revealed no significant intergroup differences for all types of complications (lethal outcome: p = 0.56; MI: p = 0.73; stroke/mini-stroke: p = 0.89; ICA restenosis/occlusion: p = 0.82; combined end point: p = 0.71). Their increase was uniform in both groups. However, more than half of all ICA restenoses and occlusions were visualized in the first 6 months after CEE. Conclusion. Implantation of a long patch (≥ 5 cm) is not characterized by increased incidence of restenosis and all adverse cardiovascular events during in-hospital and long-term follow-up.

Изучение госпитальных результатов экстренной каротидной эндартерэктомии (КЭЭ) и каротидной ангиопластики со стентированием (КАС) в острейшем периоде острого нарушения мозгового кровообращения (ОНМК). МАТеРИАЛ И МеТОдыC января 2008 по август 2020 г. в исследование вошли 615 пациентов с гемодинамически значимыми стенозами внутренних сонных артерий (ВСА), оперированными в острейшем периоде ишемического инсульта (в течение 3 суток от дебюта ОНМК). В зависимости от реализованного вида реваскуляризации все больные были распределены на две группы: 1-я группа -КАС (n=312); 2-я группа -КЭЭ (n=357). Критериями включения стали: 1. Неврологические нарушения легкой степени: от 3 до 8 баллов по шкале NIHSS; не более 2 баллов по модификационной шкале Рэнкина; более 61 балла по шкале Бартел; 2. Показания для КЭЭ/КАС согласно действующим национальным рекомендациям; 3. Ишемический очаг в головном мозге не более 2,5 см в диаметре.

Материалы и методы. Децеллюляризацию легких крысы выполняли перфузионным детергент-энзиматическим мето-дом при сопутствующей вентиляции трахеи атмосферным воздухом. Качество проведенной децеллюля-ризации оценивали с использованием рутинных гистологических и иммуногистохимических методов исследования, количественное содержание ДНК определяли спектрофотометрически. Для статической рецеллюляризации и перфузионной рецеллюляризации целого органа в качестве модели для изучения поведения клеток на каркасе использовали мезенхимальные мультипотентные стромальные клетки с последующей оценкой метаболической активности клеток колориметрическим методом и их жизнеспо-собности -путем окрашивания кальцеином и гомодимером этидия. Для качественной оценки матрикса легких после рецеллюляризации использовали иммуногистохимический анализ. Результаты. 92% ал-логенной ДНК удалено при проведении децеллюляризации. Гистологическое окрашивание не выявило остаточных клеток и клеточных ядер при сохранности волокон внеклеточного матрикса, что подтверж-далось иммуногистохимической реакцией матрикса с антителами к ламинину, эластину, фибронектину, коллагенам I и IV типов до и после проведения децеллюляризации. Каркас при заселении клетками не проявляет токсических свойств, поддерживает жизнеспособность и метаболическую активность клеток. Заключение. Полученный опыт децеллюляризации и рецеллюляризации легких крысы является перс-пективной основой для разработки протоколов создания тканеинженерных легких.Ключевые слова: децеллюляризация, рецеллюляризация, тканеинженерные легкие крысы. Aim. The main aim of our research is to evaluate the process of rat lung decellularization and recellularization as the initial step of tissue-engineered organs creation. Materials and methods. Rat lung decellularization was performed by perfusion with detergents and enzymes with concomitant atmospheric air ventilation through the trachea. The quality of decellularization was analyzed with routine histological and immunohistochemical staining techniques, DNA content was determined quantitatively by spectrophotometer. Culture of mesenchymal stem cells isolated from the bone marrow of primates by their characteristics met the requirements of stem cells. It was shown that transplantation of allogeneic mesenchymal stem cells (2 million cells per 1 kg body weight) immediately after ligation of the left anterior descending coronary artery between the middle and upper thirds led to neovascularization and capillarization of the ischemic myocardium.

Aim. To determine the optimal method for performing coronary artery endarterectomy (СE) based on immediate and long-term outcomes of coronary artery bypass grafting (CABG) in combination with open or closed CE in the left anterior descending artery (LAD).Material and methods. This retrospective study included 103 patients who underwent CABG in combination with closed CE and 204 after open CE in the LAD during the period from 2003 to 2016. In both groups, the patient age was comparable (65 years [56; 69] vs 67 years [58; 72] (p=0,263)). There were more men (88,3% vs 81,4% (p=0,421). The mean follow-up period was 94 months [38; 180]. Long-term outcomes were assessed in 86,4% (n=89) and 83,8% (n=171) of patients from the closed and open CE groups, respectively (p=0,141). Angiographic data were studied in 75,3% and 67,3% of patients, respectively (p=0,441).Results. In the closed and open CE groups, in-hospital mortality was 4,8% and 1,5% (p=0,0012), incidence of perioperative myocardial infarction — 11,6% vs 2,5%, (p<0,001), survival rate after 7,8 years — 81,3±5,1% and 84,4±3,2% (p=0,342), respectively. The patency of arterial shunts was significantly higher in the open CE group — 93,1% vs 80,6% (p=0,004). At the same time, the patency of venous shunts in the long-term period between the groups was comparable — 70,1% vs 73,7% (p=0,314).Conclusion. Open CE in combination with CABG provides better immediate outcomes compared to closed CE. Long-term survival and freedom from angina between the groups were comparable. In the long-term period, the patency of internal thoracic artery after open CE is better than after the closed technique. Open CE is a safe and effective method to achieve complete myocardial revascularization in patients with severe diffuse LAD atherosclerosis.
