Цель. Изучение предикторов стойкой послеоперационной когнитивной дис-функции (ПОКд) у пациентов с сахарным диабетом (Сд) 2-го типа, перенес-ших коронарное шунтирование (КШ) в условиях искусственного кровообраще-ния (ИК). Материал и методы. В исследование включено 54 пациента-мужчины с Сд 2-го типа, перенесших плановое КШ в условиях ИК, в возрасте от 45 до 69 лет. Всем пациентам помимо стандартного клинического обследования, проводи-лось нейрофизиологическое тестирование за 3-5 дней до операции, на 7-14-е сутки и через год после КШ. В качестве факторов, предположительно значи-мых в развитие стойкой ПОКд, были отобраны: интегральный показатель приверженности пациента к лечению, наличие до операции умеренных когни-тивных расстройств (УКР), развитие ранней ПОКд, а также фракция выброса левого желудочка, уровень личностной тревожности (ЛТ) и прогрессирование стенозов сонных артерий (СА), оцененные через 1 год после операции. Результаты. У пациентов с Сд 2-го типа через год после КШ, выполненного в условиях ИК были идентифицированы следующие наиболее значимые фак-торы развития стойкой ПОКд -прогрессирования стенозов СА, высокой ЛТ и низкого интегрального показателя приверженности к лечению, что позво-ляет с высокой вероятностью прогнозировать развитие стойкой ПОКд через год после КШ. заключение. Полученные результаты продемонстрировали значимость таких факторов как прогрессирование стенозов СА, высокая ЛТ и низкая привер-женность к лечению в развитии стойкой ПОКд у пациентов с Сд 2-го типа после КШ в условиях ИК. Aim. To study predictors of long-term post-operation cognitive dysfunction (POCd) in patients with diabetes mellitus 2 type (dM), underwent coronary bypass grafting (CBG) under on-pump conditions. Material and methods. Totally, 54 men included, with dM 2 type, underwent scheduled on-pump CBG, at the age 45-69 y. o. All patients, together with standard clinical assessment, were neurophysiologically tested on the 3-5th day after operation, on 7-14th day and in 1 year after CBG. As factors, suggestively significant for persistent POCd development, were selected: integral parameter of patents adherence to treatment, moderate cognition disorders (MCd) before operation, anxiety level (AL) and progression of carotid arteries (CA) stenoses, measured in 1 year before the operation. Results. In dM2 patients after on-pump CBG, the following most significant factors were identified as predictors for POCd: CA stenoses progression, high AL, low integral parameter of adherence; this allows for high probability level of prediction of POCd in 1 year after CBG. Цель. Оценить влияние трехнедельного курса аэробных физических трениро-вок на показатели когнитивного статуса пациентов, подвергшихся коронар-ному шунтированию (КШ). Материал и методы. Обследовано 92 мужчины с ишемической болезнью сердца (ИбС), перенесших КШ в условиях искусственного кровообращения (ИК). Пациенты были разделены на 2 группы: в первой группе -39 человек (средний возраст -55,5±5,3 лет) проводился трехнедельный курс велотре-нировок, во второй группе -53 человека (57,2±6,21 лет) велотренировки не проводились. Оценивали следующие показатели нейродинамики -ско-рость сложной зрительно-моторной реакции (СЗМР), уровень функциональ-ной подвижности нервных процессов (УФП) и работоспособности головного мозга (РГМ) за 5-7 дней до КШ, через 7-10 суток и через 1 месяц после опе-рации. Результаты. При оценке показателей нейродинамики в предоперационном периоде, а также на 7-10 сутки, значимых различий между группами выявлено не было. На 7-10-е сутки послеоперационного периода и через месяц после КШ в обеих группах наблюдалось ускорение сенсомоторных реакций при выполнении нейродинамических тестов по сравнению с дооперационными показателями. Через 1 месяц у пациентов без велотренировок регистрирова-лась более низкая скорость реакции при выполнении нейродинамических тестов по сравнению с пациентами, прошедшими курс велотренировок. Ана-лиз динамики количества совершенных ошибок показал их увеличение в группе пациентов с велотренировками на 7-10-е сутки после КШ с последу-ющим возвращением через 1 месяц к дооперационным значениям -в тесте УФП и РГМ, тогда как в группе пациентов без велотренировок их увеличение на 7-10-е сутки и через 1 месяц после КШ. При сравнении количества пропу-щенных сигналов выявлено, что через 1 месяц после КШ количество пропу-щенных знаков у пациентов без велотренировок было достоверно больше по сравнению с пациентами с велотренировками при выполнении теста УФП и РГМ. заключение. На фоне проведения трехнедельного курса велотренировок у пациентов, перенесших КШ, отмечено улучшение показателей нейродина-мики, что позволяет отнести тренировки к методу профилактики и реабилита-ции ранней послеоперационной когнитивной дисфункции.
Цель. Анализ структурных характеристик и электроэнцефалографической (ЭЭГ) картины ПОКД у пациентов после прямой реваскуляризации миокарда в условиях искусственного кровообращения (ИК). Материал и методы. В исследование включено 114 пациентов-мужчин, перенесших плановое коронарное шунтирование (КШ) в условиях ИК, сред-ний возраст составил 55,9±5,3 лет. Всем пациентам было проведено нейро-физиологическое тестирование за 3-5 дней до операции и на 7-10-е сутки после КШ. В те же сроки 65 пациентам из основной группы (средний воз-раст -57,2±5,1 лет) было проведено ЭЭГ-исследование. Результаты. Операция КШ, проводимая в условиях ИК, в 79% случаев ослож-няется развитием ранней ПОКД. Когнитивное снижение чаще всего наблюда-ется в таких доменах, как нейродинамика и память. Обнаружено, что ранняя ПОКД сопровождается ЭЭГ-признаками корковой дисфункции (увеличение мощности биопотенциалов низкочастотных тета-ритмов). Заключение. Согласно результатам исследования, структура ранней ПОКД в послеоперационном периоде КШ с ИК состоит из поражения функций нейро-динамики и памяти, что может являться причиной ухудшения качества жизни пациентов и снижения эффективности операции. Aim. To analyze structural chareacteristics and electroencephalographic (EEG) pattern of post-operation cognitive dysfunction (POCD) in patients after direct myocardial revascularization under artificial circulation (AI). Material and methods. Totally 114 male-patients included, underwent scheduled coronary bypass grafting (CBG) under the AI, with average age 55,9±5,3 y. In 3-5 days before operation and on 7-10 day after all patients underwent neurophysiological testing; of those 65 patients also underwent EEG. Results. The CBG operation with AI leads in 79% of cases to the early POCD. Cognitive impairment usually becomes prominent in the domains of neurodynamics and memory. It is shown that the early POCD is followed by EEG-signs of cortical dysfunction (increase of low-frequency theta-rhythm magnitude). STRUCTURE OF COGNITIVE DISORDERS AND DYNAMICS OF BIOELECTRIC ACTIVITY OF THE BRAIN IN PATIENTS AFTER DIRECT MYOCARDIAL REVASCULARIZATION Conclusion.According to the results of the study, in post-operative CBG with AI period the structure of POCD consists of injury of neurodynamics and memory, which might be a cause of decreased life quality of patients and efficacy of surgery itself. Aim. To study predictors of moderate cognitive disorders (MCD) in patients with coronary heart disease (CHD) and type 2 diabetes mellitus (DM2). Materials and methods. The study included 54 men with CPD and DM2 (mean age 56.8±4.5 years). Standard medical examination was supplemented by the assessment of cognitive status, characteristics of lipid and carbohydrate metabolism. Factors allegedly influencing MCD development included the patients’ age, education level, stenosis of carotid arteries, LV ejection fraction, arterial hypertension, insulin and HbAlc levels, HOMA and QUICKI indices, lipid metabolism, concentrations of total, HDL and LDL cholesterol, fructosamine, triglycerides, severity of coronary lesions (Syntax scale), trait and state anxiety. Results. Fructosamine level and HOMA index were the most important characteristics responsible for MCD in patients with CPD and DM2. Conclusion. The data obtained demonstrate the significance of fructosamine level and HOMA index in the development of MCD in patients with CPD and DM2.
Background. Applied standard methods of cervical cancer (CC) treatment have several disadvantages in terms of local and systemic toxicity and the risks of local recurrence. Our study proposed the use of neoadjuvant chemotherapy (NACT) in a dose-dense mode, followed by radical hysterectomy, in order to improve the results of treatment of locally advanced CC (IB2-IIB stages) (FIGO)/T1b2-2bN0M0 (TNM).Aim. Improvement of immediate and long-term results in treatment of locally advanced CC IB2-IIB stages (FIGO), determining the immediate effectiveness of dose-dense NACT, based on magnetic resonance imaging (MRI) data, and the identification of prognostic factors for tumor response to chemotherapy.Materials and methods. Between April 2016 to November 2019 120 women with IB2-IIB stages (FIGO) were included in this study. Of these, 58 patients underwent 3 cycles of intensified NACT according to the AP scheme (cisplatin 75 mg/m2, doxorubicin 35 mg/m2) and 62 patients underwent NACT according to the TR scheme (cisplatin 60 mg/m2, paclitaxel 60 mg/m2). The assessment of the effect of intensified NACT was performed according to the RECIST 1.1 criteria according to MRI data. Also 2 retrospective groups of patients were recruited: group I - surgical treatment without preoperative chemotherapy (25 patients with IB2-IB3 stages (FIGO) from November 2009 to August 2019), group II - concurrent chemoradiotherapy (44 patients with stage IIB cervical cancer (FIGO) from January 2013 to June 2018).Results. Complete clinical response was detected in 12 (10.0 %) patients, partial response - in 69 (57.5 %) patients, stable disease - in 35 (29.2 %) patients, progression was observed in 4 cases (3.3 %). After NACT in a dose dense mode, due to an objective clinical response (complete response, partial response and stable disease for stage IIA according to RECIST 1.1 criteria), 99 (82.5 %) patients underwent surgical treatment. For 21 (17.5 %) patients due to the absence achievement of objective clinical response (stable disease for stage IIB according to RECIST 1.1 criteria or progression), performed concurrent chemoradiotherapy. During the observation period, 113 (94.2 %) patients remained alive. Mortality for the first year was 1.7 % (2 cases). There was a coincidence of the results of a complete response according to MRI in 11 (91.7 %) cases out of 12 cases with the pathology data. According to multivariate analysis, it was revealed that parametrial invasion, according to the histological examination of the surgical material, is an independent prognostic factor for recurrence. According to comparative evaluation of disease-free survival curves between groups of IB2-IIB stages (FIGO) patients who received and did not receive dose-dense chemotherapy, it was found that disease- free survival in the group with NACT was higher compared to the group without NACT (p = 0.03, the log-rank criterion).Conclusions. The use of dose-dense NACT is an effective treatment mode for patients with IB2-IIB CC (FIGO), which in most cases allows achieving a tumor response with subsequent improvement in immediate and long-term results and can be considered as an alternative to standard treatment for locally advanced CC.
