Background & study aims Corona virus disease-19 (COVID-19) pandemic has markedly impacted routine medical services including gastrointestinal (GI) endoscopy. We aim to report the real-life performance in high volume GI endoscopy units during the pandemic. Patients and methods A web-based survey covering all aspects of daily performance in GI endoscopy units was sent to endoscopy units worldwide. Responses were collected and data were analyzed to reveal the effect of COVID-19 pandemic on endoscopy practice. Results Participants from 48 countries (n = 163) responded to the survey with response rate of 67.35%. The majority (85%) decreased procedure volume by over 50%, and four endoscopy units (2.45%) completely stopped. The top three indications for procedures included upper GI bleeding (89.6%), lower GI bleeding (65.6%) and cholangitis (62.6%). The majority (93.9%) triaged patients for COVID-19 prior to procedure. N95 masks were used in (57.1%), isolation gowns in (74.2%) and head covers in (78.5%). Most centers (65%) did not extend use of N95 masks, however 50.9% of centers reused N95 masks. Almost all (91.4%) centers used standard endoscopic decontamination and most (69%) had no negative pressure rooms. Forty-two centers (25.8%) reported positive cases of SARS-CoV-2 infection among patients and 50 (30.7%) centers reported positive cases of SARS-CoV-2 infection among their healthcare workers. Conclusions Most GI endoscopy centers had a significant reduction in their volume and most procedures performed were urgent. Most centers used the recommended personal protective equipment (PPE) by GI societies however there is still a possibility of transmission of SARS-CoV-2 infection in GI endoscopy units.
Introduction: The majority of epidemiological reports focus on confi rmed cases of COVID-19. In this study, we aim to assess the health and well-being of adults not infected with Covid-19 after two months of quarantine in Morocco. Materials and methods: Two months after the declaration of quarantine in Morocco following the Covid-19 epidemic, we carried out a descriptive cross-sectional study of 279 Moroccan citizens. We used the Short Form Health Survey (SF-12) as a determinant of quality of life, which is based on eight dimensions of health. The data were collected using an electronic questionnaire distributed online. The participants also indicated their socio-demographic data, their knowledge and practices regarding the Covid-19 pandemic and whether they had chronic health problems. Results: The quality of life of all participants was moderately disrupted during the Covid-19 pandemic with a mental health score (MCS) of 34.49 (± 6.44) and a physical health score (PCS) of 36.10 (± 5.82). Participants with chronic diseases scored lower with 29.28 (± 1.23) in mental health (MCS) and 32.51 (± 7.14) in physical health (PCS). The seriousness of COVID-19 has an impact on the quality of life and health well-being of people and this impact is more marked in people with chronic health problems. Conclusion: Our results confi rm the need to pay attention to the health of people who have not been infected with the virus. Our results also point out that uninfected people with chronic illnesses may be more likely to have well-being problems due to quarantine restrictions.
Introduction : Les pseudotumeurs ou les tumeurs myofibroblastiques inflammatoires (TMI) sont des lésions rares et bénignes du tissu conjonctif. Le pancréas représente une localisation très rare de ces lésions. Nous rapportons un nouveau cas de pseudotumeur inflammatoire pancréatique avec revue de la littérature. Observation : Une patiente de 30 ans sans antécédent pathologique particulier consultait pour des épigastralgies transfixiantes évoluant depuis trois mois. L'examen clinique trouvait une légère sensibilité épigastrique isolée. Le bilan morphologique mettait en évidence une masse hypodense du corps du pancréas infiltrant la graisse péripancréatique sans signe de métastase ou de carcinose péritonéale. L'exploration chirurgicale montrait une tumeur pancréatique envahissant le mésentère et le mésocôlon transverse dont la biopsie montrait une pancréatite chronique sans signe de malignité. Deux autres biopsies transpariétales de la masse étaient sans signe de malignité. L'attitude adoptée était la surveillance. L'évolution était marquée par la régression progressive de la lésion au bout d'un an. Actuellement, nous avons un recul de quatre ans sans récidive.Discussion : À ce jour, 25 cas de TMI pancréatiques ont été rapportés dans la littérature. Ces lésions posent de grands problèmes diagnostiques avec l'adénocarcinome, et souvent le seul moyen diagnostique est l'étude anatomopathologique de la pièce opératoire. Leur pronostic paraît globalement bon avec possibilité de récidive locale. Conclusion : La TMI du pancréas est une entité rare qui peut mimer cliniquement et radiologiquement une tumeur maligne du pancréas. La particularité de notre observation est la régression spontanée sans traitement malgré l'aspect initialement invasif. Pour citer cette revue : J. Afr. Hépatol. Gastroentérol. 5 (2011).Abstract Introduction: Inflammatory myofibroblastic pseudotumors are rare benign lesions of the soft tissue. The pancreas is a very rare location of these lesions. We present a new case of pancreatic inflammatory myofibroblastic pseudotumor with literature review. Case report: A 30-year-old woman with no specific medical history consulted for epigastric pain, developed since 3 months. Slight epigastric tenderness was observed on clinical examination. The imaging results showed a pancreatic hypodense mass with infiltration of the peripancreatic fat without metastasis or peritoneal carcinosis. Surgical exploration showed a nonresectable pancreatic mass invading the mesentery and the transverse mesocolon. Pathological study showed a chronic pancreatitis without any signs of malignancy. Two other echoguided transparietal biopsies of the mass were also without malignancies. The approach adopted was to follow-up with no treatment, and the outcome was marked by the disappearance of the mass after a year. Currently, there is no recurrence after 4 years of follow-up. The duodenum is an exceptional location of the primary squamous cell carcinoma. Few cases have been reported in the literature. We report here the second case of this tumor in this exceptional location (transverse duodenum) in a sixty years-old women without medical history, who consulted for epigastric pain since 3 months, associated with an episode of melena and weight loss of 5 kg. The assessement and pathological study of endoscopic specimen confirms the squamous cell nature of the tumor. The treatment consisted of palliative chemotherapy with analgesic treatment.
