Le pancréas : localisation rare des tumeurs myofibroblastiques inflammatoires

Abstract

Introduction : Les pseudotumeurs ou les tumeurs myofibroblastiques inflammatoires (TMI) sont des lésions rares et bénignes du tissu conjonctif. Le pancréas représente une localisation très rare de ces lésions. Nous rapportons un nouveau cas de pseudotumeur inflammatoire pancréatique avec revue de la littérature. Observation : Une patiente de 30 ans sans antécédent pathologique particulier consultait pour des épigastralgies transfixiantes évoluant depuis trois mois. L'examen clinique trouvait une légère sensibilité épigastrique isolée. Le bilan morphologique mettait en évidence une masse hypodense du corps du pancréas infiltrant la graisse péripancréatique sans signe de métastase ou de carcinose péritonéale. L'exploration chirurgicale montrait une tumeur pancréatique envahissant le mésentère et le mésocôlon transverse dont la biopsie montrait une pancréatite chronique sans signe de malignité. Deux autres biopsies transpariétales de la masse étaient sans signe de malignité. L'attitude adoptée était la surveillance. L'évolution était marquée par la régression progressive de la lésion au bout d'un an. Actuellement, nous avons un recul de quatre ans sans récidive.Discussion : À ce jour, 25 cas de TMI pancréatiques ont été rapportés dans la littérature. Ces lésions posent de grands problèmes diagnostiques avec l'adénocarcinome, et souvent le seul moyen diagnostique est l'étude anatomopathologique de la pièce opératoire. Leur pronostic paraît globalement bon avec possibilité de récidive locale. Conclusion : La TMI du pancréas est une entité rare qui peut mimer cliniquement et radiologiquement une tumeur maligne du pancréas. La particularité de notre observation est la régression spontanée sans traitement malgré l'aspect initialement invasif. Pour citer cette revue : J. Afr. Hépatol. We present a new case of pancreatic inflammatory myofibroblastic pseudotumor with literature review. Case report: A 30-year-old woman with no specific medical history consulted for epigastric pain, developed since 3 months. Slight epigastric tenderness was observed on clinical examination. The imaging results showed a pancreatic hypodense mass with infiltration of the peripancreatic fat without metastasis or peritoneal carcinosis. Surgical exploration showed a nonresectable pancreatic mass invading the mesentery and the transverse mesocolon. Pathological study showed a chronic pancreatitis without any signs of malignancy. Two other echoguided transparietal biopsies of the mass were also without malignancies. The approach adopted was to follow-up with no treatment, and the outcome was marked by the disappearance of the mass after a year. Currently, there is no recurrence after 4 years of follow-up.
