Contexto: el papel de la aldosterona y el de su receptor son motivos de investigación y aprendizaje continuo por su impacto en condiciones patológicas, en especial cardiacas y renales. Objetivo: realizar una revisión narrativa acerca del efecto renal y cardiaco de la aldosterona junto a su receptor y describir la evidencia disponible en estudios clínicos de los medicamentos que tienen como objetivo terapéutico. Metodología: para responder a las preguntas: ¿cuáles son los efectos cardiacos y renales de la activación del receptor de aldosterona en condiciones patológicas? y ¿qué estrategias terapéuticas implementadas se han utilizado en estudios aleatorizados? Se realizó una búsqueda en la base de datos Pubmed con las palabras clave y los términos Mesh relacionados. Resultados: esta revisión presenta una gran variedad de mecanismos de acción alternativos de la aldosterona y el receptor de mineralocorticoides y su relación con respecto a otros receptores, marcando su acción genómica y no genómica, que llevan finalmente a promover la inflamación y la fibrosis a nivel cardiaco y renal. Por último, se realiza un resumen de los estudios fase II y III de antagonistas de la aldosterona. Conclusiones: la aldosterona es una hormona de gran relevancia por su acción en el riñón y el corazón desde su desarrollo y el receptor de mineralocorticoides se revela como un objetivo terapéutico, en el que los medicamentos mejorados se destacan por ser más específicos y por tener menos efectos adversos.
Contexto: la resistencia a la hormona tiroidea representa a un grupo de síndromes caracterizados por la disminución de la sensibilidad a esta hormona. Se han descrito tres factores que promueven la resistencia: mutaciones de los genes que codifican para el receptor nuclear de T3 (TRA y TRB), alteraciones en el transporte celular de T4 y T3 y defectos en la conversión de T4 a T3 mediada por deiodinasas. Objetivo: describir las distintas formas de resistencia de la hormona tiroidea, desde puntos de vista clínicos, fisiopatológicos, genéticos y moleculares. Metodología: las bases de datos PubMed y Medline fueron consultadas con el fin de obtener referencias cruzadas que vincularan las palabras clave de manera relevante con aspectos clínicos, fisiopatológicos, genéticos y moleculares. Se usaron adicionalmente recursos del sitio web del NCBI en lo que respecta a aspectos genéticos. Resultados: las referencias encontradas mediante la búsqueda realizada permitieron hacer una exposición integrada desde el punto de vista clínico y fisiopatológico, asociados a los aspectos genéticos y moleculares de la resistencia a la hormona tiroidea. Conclusiones: se reconoce la importancia de saber las causas y las manifestaciones sindrómicas de la resistencia a la hormona tiroidea, lo cual permite hacer intervenciones personalizadas basadas en la clínica.
El hipercolesterolemia familiar (HF) es una enfermedad autosómica dominante causada por mutaciones del gen que codifica para el receptor de colesterol de baja densidad (LDLR) o en moléculas asociadas al funcionamiento de éste (1). La forma heterocigota (HFHe) se presenta con una prevalencia de 1 por cada 200 a 300 personas (2), con elevaciones de colesterol de baja densidad (cLDL) entre 160 a 580 mg/dL y aumento del riesgo de enfermedad cardiovascular temprana (5). Para el diagnóstico se precisa de la realización de escalas de puntuación como la del Simon Broome o Dutch Lipid Clinic Network y la confirmación a través de pruebas de diagnóstico genético (5). La modificación al estilo de vida y las estatinas son la terapia de primera línea (7), sin embargo, es frecuente que no se alcance la meta de reducción de cLDL o se presente intolerancia farmacológica, en estos casos se recomienda el uso de inhibidores de la proproteína convertasa subtilisina-kexina tipo 9 (PCSK9) (13,14). Presentamos el caso de una mujer de 68 años de edad con HFHe que presentó intolerancia a estatinas y no respondió a inhibidores de la PCSK9, por lo que se inició lomitapide, con reducción del 50% del cLDL y adecuada tolerancia. Este medicamento está aprobado para el tratamiento de la hipercolesterolemia familiar homocigota (HFHo) (15).
Introducción: La diabetes mellitus tipo 2 es una enfermedad con alta prevalencia a nivel mundial y su incidencia aumenta, por lo que es frecuente que los pacientes que se encuentran hospitalizados por diversas causas tengan diabetes. Las recomendaciones de la American Diabetes Association (ADA) muestran la insulina como medicamento de elección, a pesar de que ambulatoriamente reciban otros medicamentos antidiabéticos. El objetivo de este estudio es evaluar la no inferioridad de los medicamentos con efecto incretina (Inhibidores de dipeptidil peptidasa IV y agonistas de GLP-1) al compararlos con insulina de forma intrahospitalaria. Metodología: Se realizó una revisión sistemática de la literatura y metaanálisis. La búsqueda de literatura se realizó en las bases de datos Pubmed, Embase, Science direct, Scielo, Scopus, Open Grey y Google scholar. Se incluyeron ensayos clínicos aleatorizados comparando el tratamiento con medicamentos con efecto incretina vs esquema basal bolo de insulina en pacientes hospitalizados. El desenlace primario fue el control glucémico (normoglucemia) y como desenlaces secundarios hiper o hipoglucemia. Se calcularon Odds ratios (OR) como medidas de asociación con sus intervalos de confianza del 95% e índices I2 en cada análisis como medida de heterogeneidad, usando el software Review Manager. Para la interpretación, se estableció arbitrariamente 1,25 como límite de no inferioridad. Resultados: De 116 referencias identificadas como posiblemente elegibles, se incluyeron 7 estudios. Cinco comparaban inhibidores de DPP-4 vs insulina y 2 agonistas de GLP-1 vs insulina. El OR para normoglucemia fue de 1.35 (IC 95% 1.27- 1.42, I2=95%) al comparar medicamento incretina vs insulina, mientras que para el comparador medicamento incretina asociado a insulina vs insulina fue 1.5 (IC 95% 1.00-2.27, I2 = 95%). Para hiperglucemia en ambas comparaciones los OR fueron 0.81 (IC 95% 0.63-1.04, I2 = 84%) y 0.63 (IC95% 0.50-0.80, I2 = 81%), respectivamente. En el desenlace de hipoglucemia el OR fue 0.15 (IC95% 0.06 a 0.4, I2 = 23%) para medicamento incretina vs insulina y 0.44 (IC 95% 0.18-1.06, I2 = 50%) para medicamento incretina asociado a insulina vs insulina . Conclusión los medicamentos con efecto incretina fueron inferiores a la insulina para el desenlace de normoglucemia en pacientes diabéticos hospitalizados. En cambio, fueron no inferiores respecto a los desenlaces de hiperglucemia e hipoglucemia. Se requieren nuevos estudios para dar una recomendación para el uso de estos medicamentos en el ámbito intrahospitalario.
