La hipoglucemia es una urgencia médica frecuente que en la mayoría de los casos es secundaria al uso de fármacos hipoglucemiantes, orales o inyectados, indicados en pacientes con diabetes mellitus. No obstante, puede presentarse en forma espontánea y severa relacionándose con múltiples condiciones clínicas, incluyendo las neoplasias. Ante una hipoglucemia de origen paraneoplásico se deben reconocer los mecanismos fisiopatológicos que la generan y establecer el diagnóstico oportuno y preciso para disminuir las complicaciones propias de este síndrome clínico. Presentamos dos pacientes con cuadro de hipoglucemia refractaria al manejo médico inicial, de aparición similar con patologías diferentes. El primer caso corresponde a un paciente con insulinoma y el segundo con un hemangiopericitoma.
Contexto: la acromegalia es una enfermedad rara en la población general y habitualmente tiene una evolución clínica insidiosa, lo cual genera un reto para diagnosticar al paciente a partir de la sospecha clínica y posterior confirmación con las ayudas diagnósticas. La mayoría de pacientes con acromegalia tienen como origen de la enfermedad un tumor hipofisario secretor de hormona del crecimiento y en consecuencia el tratamiento debe estar dirigido a lograr el control bioquímico y la reducción de la masa tumoral. Finalmente, el seguimiento del paciente resulta clave para lograr el control de la enfermedad y detectar las comorbilidades asociadas. Objetivo: definir criterios para el diagnóstico, tratamiento y seguimiento de la acromegalia en pacientes colombianos. Materiales y métodos: se realizó un consenso tipo Delphi modificado. Participaron 10 expertos en el manejo del paciente con acromegalia (endocrinólogos y neurocirujanos) y un grupo desarrollador. Los resultados fueron analizados y discutidos. A partir de este ejercicio, se presentaron las recomendaciones en los diferentes apartados. Resultados: se obtuvieron recomendaciones acerca del seguimiento al paciente colombiano con acromegalia y el abordaje del paciente desde el sistema de salud local, la sospecha clínica, el diagnóstico bioquímico e imagenológico, el tratamiento mediante intervención quirúrgica, el tratamiento farmacológico y la radioterapia. Conclusiones: en Colombia es importante el fortalecimiento del conocimiento médico desde la atención primaria hasta el especialista en el correcto diagnóstico, manejo y seguimiento del paciente con acromegalia para lograr la detección temprana de la enfermedad y disminuir la progresión de las comorbilidades asociadas.
Primary hyperparathyroidism is a disease with multisystemic and heterogeneous manifestations, characterized by underlying high parathormone concentrations. Despite neuropsychiatric involvement being one of the manifestations, psychosis is rare. This is the case of a 68-year-old female with a 10-day clinical course of anorexia, mutism, dysphagia, constipation, and weight loss. The patient had disorganized speech associated with paranoid delusions. Prior to this visit, the patient was recently diagnosed with a mixed anxiety-depressive disorder. For this reason, treatment with antidepressants in combination with atypical antipsychotics was administered without a satisfactory response. Neuroimaging, infectious panel, and toxicology screening showed no abnormal findings. Hypercalcemia secondary to a retropharyngeal ectopic parathyroid adenoma was the causative etiology of her primary hyperparathyroidism, and hypercalcemia treatment resolved the psychotic episode. We highlight the importance of recognizing psychosis as a possible initial presentation of hyperparathyroidism and hypercalcemia. Ruling out organic etiologies prior to diagnosing a primary cause of psychosis is crucial, as their treatment can reverse the psychotic symptoms.
Objetivo: mostrar la experiencia inicial y los resultados preliminares de la terapia con [177Lu – Dota0 – Tyr3] – Octreotate (177Lu-Dotatate) en pacientes tratados en la Fundación Cardioinfantil – Instituto de Cardiología (FCI-IC) debido a que la experiencia derivada de esta práctica en países latinoamericanos es poco conocida. Materiales y métodos: se realizó un análisis descriptivo y retrospectivo en pacientes a los que se les realizó terapia con 177Lu-Dotatate con al menos una dosis y un año de seguimiento. Se incluyeron 8 pacientes (4 mujeres y 4 hombres) de edad media: 57 años (50-63). Se administraron 24 dosis en total, 3 dosis en promedio por paciente, 187 GBq totales y 23,5 GBq dosis/paciente. Se valoró la respuesta al tratamiento y la presencia de efectos adversos. Resultados: el tiempo medio de seguimiento fue de 46 meses (26-72 meses). 5/8 pacientes tenían diagnóstico de tumores neuroendocrinos (tne) de origen pancreático, 2/8 de origen gástrico y 1/8 de yeyuno. 7/8 pacientes tenían compromiso metastásico; 1/8, a pesar deno tener afectación metastásica, fue considerado irresecable. 7/8 pacientes presentaron síntomas posterapia leves y 1/8 eventos adversos serios. 2/8 pacientes tuvieron respuesta parcial, 4/8 enfermedad estable y 2/8 respuesta completa. Sin embargo, al nal del seguimiento un paciente que tuvo respuesta completa presentó recaída y otro, respuesta parcial con posterior progresión y muerte. Conclusiones: en la institución, la terapia con 177Lu-Dotatate ha sido segura y eficaz en pacientes con TNE como alternativa terapéutica con adecuados niveles de respuesta y control de la enfermedad con eventos adversos leves, esperables y eventos serios mínimos autolimitados.
