Contexto: la resistencia a la hormona tiroidea representa a un grupo de síndromes caracterizados por la disminución de la sensibilidad a esta hormona. Se han descrito tres factores que promueven la resistencia: mutaciones de los genes que codifican para el receptor nuclear de T3 (TRA y TRB), alteraciones en el transporte celular de T4 y T3 y defectos en la conversión de T4 a T3 mediada por deiodinasas. Objetivo: describir las distintas formas de resistencia de la hormona tiroidea, desde puntos de vista clínicos, fisiopatológicos, genéticos y moleculares. Metodología: las bases de datos PubMed y Medline fueron consultadas con el fin de obtener referencias cruzadas que vincularan las palabras clave de manera relevante con aspectos clínicos, fisiopatológicos, genéticos y moleculares. Se usaron adicionalmente recursos del sitio web del NCBI en lo que respecta a aspectos genéticos. Resultados: las referencias encontradas mediante la búsqueda realizada permitieron hacer una exposición integrada desde el punto de vista clínico y fisiopatológico, asociados a los aspectos genéticos y moleculares de la resistencia a la hormona tiroidea. Conclusiones: se reconoce la importancia de saber las causas y las manifestaciones sindrómicas de la resistencia a la hormona tiroidea, lo cual permite hacer intervenciones personalizadas basadas en la clínica.
