There are alternative routes and products to improve the efficacy of immunotherapy.
OBJECTIVE: This study aimed to investigate effect of salt and fluid restriction education for patients undergoing hemodialysis and nonadherent to fluid restriction treatment on blood pressure and weight gain.MATERIAL and METHODS: The study sample consisted of 100 patients, nonadherent to fluid restriction treatment, undergoing routine 3 times a week hemodialysis treatment at a hemodialysis center. Salt and fluid restriction education was given to patients once a week during two consecutive weeks. Pre and post-hemodialysis weight and means, interdialytic weight gain and means, predialysis systolic and diastolic blood pressures and means were measured.RESULTS: 67 male and 33 female patients were included. The average age was 56.2 ±13.5 years. Mean hemodialysis time was 54.7+-42.1 months. After education, there was a significant reduction in pre-dialysis weight means (p<0.001) and mean interdialytic weight gain (p:0.006). Also there was a decrease in pre-dialysis systolic blood pressure means (p:0.035) after the education. However, there was no reduction in pre-dialysis diastolic blood pressure means.CONLUSION: If patients undergoing hemodialysis are given sufficient education about salt and fluid restriction; they are eager to follow salt and fluid restriction treatment
ÖzAmaç: Bu çalışmamızda amacımız; Eskişehir Osmangazi Üniversitesi Aile Hekimliği Polikliniğine başvuran hastaların boy, kilo farkındalıklarını ve fiziksel egzersiz durumlarını değerlendirmektir. Materyal ve Metot: Eskişehir Osmangazi Üniversitesi Aile Hekimliği Polikliniğine başvuran 187 gönüllü dâhil edilmiştir. Hastaların boy, kilo farkındalıklarını ve fiziksel egzersiz durumlarını ölçmek amacıyla hazırladığımız ankette, kişilere boy ve kiloları sorularak kaydedilmiştir. Kişilerin bildirdikleri boy, kilo değerleri ile poliklinikte hekimlerin ölçtüğü boy, kilo değerleri arasında fark olup olmadığı, fark varsa bu farkın yaş, cinsiyet ve eğitim düzeyinden etkilenip etkilenmediği, en son boy, kilo ölçümlerini ne zaman yaptıkları, kilolarından memnun olup olmadıkları, spor faaliyetlerine olan ilgileri, hangi sıklıkla spor yaptıkları, spor yapanlara yaptıkları sporun türü ve sıklığı gibi sorular sorulmuştur. Bulgular: Çalışmamıza %44,40'ı kadın, %55,60'ı erkek olmak üzere 187 kişi dâhil edilmiştir. Hastalarımızın tahmin ettikleri boy ve kilolarına dikkate alınarak hesaplanan beden kitle endekslerine göre %4,30'u zayıf, %45,70'i normal kilolu, %34,90'ı yüksek kilolu, %15,10'u obez saptanmıştır. Boy ve kilo ölçümlerimizin sonunda hesaplanan beden kitle endekslerine göre ise %3,20'si zayıf, %46,50'si normal kilolu, %35,30'u yüksek kilolu, %15,00'ı obez olarak saptanmıştır. Düzenli spor yaptıklarını söyleyenlerin oranı %29,40 idi. Boy, kilo ve beden kitle indeksleri ölçüm öncesi ve sonrası anlamlı farklılık göstermedi. Sonuçlar: Hastalarımızın büyük çoğunluğunun boy ve kilolarını biliyor olmaları, boy kilo farkındalıklarının yüksek olduğunu göstermektedir. Düzenli spor yapma konusunda henüz istenen düzeye ulaşamadığımız görülmektedir ve bu konuda halkımızın bilinçlendirilmesi gerekir. Hekimler her hasta için BKİ hesaplamalıdırlar. Patients' heights and weights were recorded within the questionnaires prepared in order to measure their height and weight awareness together with their physical exercise status. Patients' declarations about their heights and lengths were compared by the physicians' measurements. If a difference was present, it was questioned whether this difference was affected by age, gender and level of education. They were also questioned about their last measurement of height and weight, about their satisfaction of their weights, about their sportive interests, the frequency and type of their sporting activities. e15502 Background: Red cell distribution width (RDW) is an elevated marker in several cancers like breast, colon, prostate and pancreatic cancer at the time of diagnosis. Gastric cancer (GC) is the fifth most common cancer and also third leading cause of cancer deaths.We aimed to determine whether RDW values differ between gastric cancer and healthy subjects, as well as to evaluate association between RDW and other factors. Also, we examined other two important inflammatory markers in GC patients: neutrophil to lymphocyte ratio (NLR) and platelet to lymphocyte ratio (PLR) Methods: This retrospective study included 68 gastric cancer patients and 42 age and sex matched healthy controls. The exclusion criteria involved cardiac disease, chronic obstructive lung disease, thromboembolism, chronic renal failure, hepatic disorders, hypertension, acute and chronic infections, stroke, hematologic disease and other accompanying cancer. Blood samples were retrospectively obtained from computerized patient database. Results: Among 110 subjects; RDW, NLR and PLR were significantly higher in GC patients when compared to healthy subjects (RDW: 44,9 vs 41,4 p < 0.0001, NLR: 3,40 vs 1,90 p < 0.0001, PLR: 245,9 vs 131,1 p = 0.007). There was no statistically significant association between these markers (RDW, NLR and PLR) and stage, histopathological subgroups and metastasis stage at the time of diagnosis. Conclusions: Elevated RDW is a simple, cheap and readily available marker and may be useful in gastric cancer at the time of diagnosis. Amaç: Orta Karadeniz Bölgesi'nde BCR/ABL negatif kronik miyeloproliferatif hastalık (KMH) tanısıyla takip edilen olguların JAK2V617F mutasyon sıklığı ve bu mutasyonun hastalığın klinik özelliklerine etkisi açısından incelenmesi amaçlanmıştır.Gereç ve Yöntem: Çalışmaya kronik miyeloproliferatif hastalık tanısı konulan 52 olgu (28 polistemia vera, 21 esansiyel trombositoz, 3 primer miyelofibrozis) dahil edildi. Hastaların tanı anındaki yaş, cinsiyet, hemoglobin, hematokrit, trombosit ve lökosit değerleri kaydedildi. Hepatomegali, splenomegali, kanama ve tromboz varlığı değerlendirildi. JAK2V617F mutasyon analizi hastaların periferik kan granülositlerinden Real Time-PCR Melting Curve analizi ile yapıldı.Bulgular: JAK2V617F mutasyon sıklığı polistemia vera, esansiyel trombositoz ve primer miyelofibrozis hastalarında sırası ile %85,7 (24/28), %47,6 (10/21) ve %33,3 (1/3) idi. Tanı yaşı ile JAK2V617F mutasyonu arasında istatistiksel olarak anlamlı ilişki tespit edilmedi (p>0,05). JAK2V617F mutasyonu varlığına göre hastalar incelendiğinde polistemia vera ve esansiyel trombositozlu hastalarda mutasyon olanlarda daha yüksek lökosit, hemoglobin ve hematokrit değerleri ile tromboz görülme oranı elde edilmiş olup, aradaki fark istatistiksel olarak anlamlı değildi (p>0,05). JAK2V617F mutasyonu olan polistemia vera hastalarında hepatomegali daha fazla görülmekte idi (p=0,030). JAK2V617F mutasyonu olan esansiyel trombositoz hastalarında tanı anında trombosit sayısı daha düşüktü, fakat aradaki fark istatistiksel olarak anlamlı değildi (p=0,397).Sonuç: JAK2V617F mutasyonun keşfi ile BCR/ABL negatif KMH'ın moleküler temeline ait bilgiler sunmaktadır. Bu mutasyonun varlığı KMH'ın klinik özelliklerini etkilemektedir. Bu çalışma bölgemizde KMH olgularında JAK2V617F mutasyonu sıklığının değerlendirildiği ilk çalışma olup, bulgularımız literatür verileri ile uyumlu bulunmuştur. JAK2V617F mutasyonunun KMH üzerine etkisinin daha iyi anlaşılabilmesi için büyük olgu sayılarını içeren çalışmalara ihtiyaç vardır.
