The sacro-iliac joint of a patient who had been ill with ankylosing spondylitis for half a year was investigated histologically at necropsy. Two basic histological phenomena were apparent: inflammation and aggressive proliferating chondroid metaplasia at the chondro-osseous junction and in the subchondrium. Joint cartilage degeneration, joint cartilage fusion, capsule ossification and subchondral bone apposition were observed as epiphenomena at the sacro-iliac joint. Both basic morphological phenomena are due to the same cause which has not yet been clarified. They progress independently of each other with varying intensity. This assumption explains differing opinions and the relation of inflammation and pathological ossification of the skeletal axis as well as the unsatisfactory objective results of treatment. Only a combination therapy directed against both morphological basic phenomena will result in an objective therapeutic success and not only in a symptomatic improvement of ankylosing spondylitis.
Die Nierenpapillennekrose, herdförmige oder totale Nekrose der Papille und angrenzender Bezirke des Nierenmarkes, ist meistens eine Komplikation anderer renaler Grundkrankheiten. Häufigkeit, Ursache und klinischer Verlauf haben sich in den letzten drei Jahrzehnten deutlich verändert. Bei 15 749 Obduktionen fand sich in den Jahren 1961-1971 eine Häufigkeit von 1,51°/o (n = 238). Das Verhältnis von Männern zu Frauen betrug 1,1 : 1. 218 Patienten (91,50/o) waren über 50 Jahre, 188 (78,9°/o) über 60 Jahre alt. Der größte Anteil mit 136 Patienten (57,1°/9) entfiel auf die Medizinische Abteilung, mit weitem Abstand folgten die Urologische und Chirurgische Abteilung mit 45 (18,9°/o) bzw. 44 Patienten (18,5°/o). Als ätiologische Faktoren dominierten die Harnwegsobstruktion mit 46,6°/o (n = 111), der Diabetes mellitus mit 21,4°/o (n =51) und die chronische interstitielle Nephritis mit 16,8°/o (n = 40); der Anteil der akuten und chronischen Pyelonephritis ohne Diabetes und Harnwegsobstruktion betrug 6,70/o (n = 16) bzw. 6,3°/o (n = 15), in 13,4°/o (n = 32) handelte es sich um seltenere Ursachen. Die jeweilige Pathogenese prägt auch das klinische Bild. Drei Verlaufsformen lassen sich unterscheiden: 1. die akute schwere Nierenerkrankung mit Fieber oder Schüttelfrost, Oligurie, Urämie und hoher Letalität, überwiegend bei Harnwegsobstruktion und (oder) Diabetes mellitus, 2. die chronische rezidivierende Nierenerkrankung mit Episoden von Niere nkoliken und Hämaturie, Infektion der Harnwege und gelegentlicher Obstruktion eines Ureters durch einen abgehenden Papillensequester, 3. die progressive Urämie und Anämie ohne urologische Symptome, überwiegend bei chronischer interstitieller Nephritis nach langjährigem Analgetika-Abusus.
