A dor pós-operatória como contribuinte do prejuízo na função pulmonar em pacientes submetidos à cirurgia cardíaca
There was significant impairment of lung function which had not recovered completely on the 5th postoperative day. Pain was a complaint that persisted throughout the study period. The parameters of pulmonary function showed a significant relationship with pain. There was no correlation between pain and the characteristics of individuals, the surgical procedure or the length of hospital stay.
Objetivo: Identificar o perfil dos pacientes com câncer e em cuidados paliativos e suas demandas referentes ao atendimento multidisciplinar existente em um Serviço de Atenção Domiciliar. Métodos: Estudo transversal com coleta de dados secundários dos pacientes oncológicos em cuidados paliativos atendidos pelo Programa de Atenção Domiciliar do Grupo Hospitalar Conceição, no ano de 2013. Aprovado pelo Comitê de Ética e Pesquisa sob o número 13.203. Resultados: Foram acompanhados 63 pacientes com diagnóstico de neoplasia. Dentre esses, 71,5% (n=45) estavam em cuidados paliativos. As medianas do tempo de acompanhamento dos pacientes foram de 22 (3-146) dias e do número de visitas realizadas pela equipe foi 6 (1-34). Além das visitas semanais realizadas pelas equipes, os pacientes receberam assistência de nutricionista, fisioterapeuta e assistente social. Considerando as 45 internações, os principais cuidados foram controle da dor, curativos em feridas, controle glicêmico, e antibioticoterapia endovenosa. A principal intercorrência clínica foi a repassagem de cateter nasoenteral (n=5). A alta em condições estáveis de saúde ocorreu em 10 pacientes. Conclusão: O presente estudo permitiu a identificação das principais demandas dos pacientes em cuidados paliativos oncológicos atendidos por um serviço de Atenção Domiciliar. Observa-se na prática deste serviço que a maioria dos pacientes apresentou necessidade de cuidados complexos, mas possíveis de serem realizados no domicílio pela equipe de saúde e/ou cuidador capacitado.
Purpose: To characterize patients with stroke and to establish the prevalence of oral communication disorders (CD) related by cerebrovascular accident (CVA) and the frequency of indication or request of speech therapy. Methods: A retrospective cross-sectional study from the medical report forms of 95 patients hospitalized for stroke between June 2007 and June 2008 in a regional public hospital. Results: Out of the 95 patients, 51 (53.7%) were male (mean age was 59.8 years old), 48 (59.3%) had a history of hypertension, 82 (86.3%) had ischaemic stroke and CD were present in 53 (55.8%) of patients. The mean hospital stay was 5 days. There was no significant relationship between the type of stroke and CD, or between the site of neurological damage due to stroke and the occurrence of CD. None of the subjects received an evaluation or speech-language therapy during hospitalization, and there were no referrals to healthcare after discharge. Conclusion: More than 50% of individuals affected by stroke had oral communication disorders during the hospitalization. There was no any indication or request for speech therapy in this period, even after discharge. These findings indicate a probable failure to integrate with the speech therapy staff for the treatment of patients with neurological diseases. It is necessary to further evaluate whether this is due absence of speech therapists in the team or just healthcare professionals lacking knowledge about the possibilities of speech therapy along with disturbances in oral communication due to stroke. isquêmico e 53 (55,8%), distúrbios de comunicação oral. Não houve relação significativa entre o tipo de AVC e distúrbio de comunicação oral, tampouco entre o local da lesão neurológica decorrente do AVC e a ocorrência de distúrbio de comunicação oral. Nenhum dos sujeitos recebeu indicação de avaliação ou tratamento fonoaudiológico durante a internação ou na alta. Conclusão: Mais de 50% dos indivíduos acometidos por AVC apresentam distúrbios de comunicação oral durante o período de internação hospitalar. Não houve nenhuma indicação ou solicitação de atendimento fonoaudiológico neste período, tampouco em encaminhamento para atendimento após a alta hospitalar. Estes achados indicam provável falha da inserção fonoaudiológica em equipes que atendem pacientes acometidos por doenças neurológicas, sendo necessário avaliar mais profundamente se isto ocorre devido à ausência do fonoaudiólogo na equipe, ou ao pouco conhecimento dos profissionais sobre a atuação fonoaudiológica junto aos distúrbios de comunicação oral decorrentes de AVC. original RESUMOObjetivo. identificar os efeitos da fisioterapia aquática em indivíduos portadores de transtornos mentais. Método. estudo piloto em formato de ensaio clínico, controlado e monocego. A amostra inicial contou com 16 indivíduos com diagnóstico clínico de algum transtorno mental, os quais foram divididos em dois grupos: grupo 1 (fibromialgia n=6) e grupo 2 (transtornos de ansiedade n=10), ambos submetidos a um mesmo protocolo de intervenção fisioterapêutico de 24 sessões, de 1 hora cada. Os indivíduos responderam a vários questionários antes e ao final das intervenções. Os dados foram digitados no programa SPSS 15.0 e os testes estatísticos usados foram o teste t de student e qui-quadrado de Yates, considerando p<0,05. Resultados. obtivemos 4 perdas ao longo do estudo, portanto, a amostra final ficou sendo 12, divididos em dois grupos: grupo 1 (n=6) e grupo 2 (n=6). Ambos os grupos melhoraram após as intervenções, sendo que o grupo 1 obteve melhora significativa em mais domínios se comparados ao grupo 2, que melhorou significativamente apenas no domínio de limitações emocionais da SF-36 e na diminuição dos sintomas obsessivocompulsivos (p<0,03 para ambos). Conclusões. os resultados deste estudo sugerem que a fisioterapia aquática é benéfica no tratamento de transtornos mentais. ABSTRACTObjective. to identify the effects of aquatic physical therapy in mentally ill individuals. Method. pilot study in the format of a controlled, single blind clinical trial. The initial sample, counted with 16 individuals clinically diagnosed with some mental disorder, which were divided into two groups: group 1 (fibromyalgia n=6) and group 2 (anxiety disorders n=10), both submitted to the same 24 sessions, 1 hour each, physical therapy intervention protocol. The individuals responded to several questionnaires prior to and post interventions. The data were digitated in the SPSS 15.0 program and the statistical tests used were student's t test and Yates' Chi-square test, considering p<0,05. Results. we obtained 4 losses along the study. Thus, the final sample ended up in 12, divided into two groups: group 1 (n=6) and group 2 (n=6). Both groups improved after the interventions, being group 1 the one that obtained significant improvement in more domains if compaired to group 2, that significantly improved only in the SF-36 emotional limitations domain and in the obsessive-compulsive symptoms reduction (p<0,03 to both). Conclusions. the results of this study suggest that aquatic physical therapy is benefic in the mental disorders treatment.
