Синдром диабетической стопы 3/2011Сахарный диабет роблема диабетической микроангиопатии как одного из осложнений сахарного диабета (СД) до настоящего времени является актуальной ввиду ее определяю-щей роли в инвалидизации и смертности больных СД. Если роль и характер изменений в микроциркуляторном русле ор-ганов-мишеней при диабетической ретинопатии, нефропатии и, в меньшей степени, при нейропатии, изучены достаточно подробно, то представления о вкладе диабетической микро-ангиопатии в формирование синдрома диабетической стопы нельзя считать полными.Предположения об особых, неатероматозных, но носящих окклюзирующий характер поражениях микрососудистого русла в стопах у больных СД высказывались еще в 50-х гг. XX века [1]. Однако только в 1984 г. было убедительно доказано отсутствие окклюзионного поражения на уровне микроциркуляторного русла [2]. В то же время, при СД имеются определенные на-рушения микрогемодинамики, которые могут быть охаракте-ризованы как изменения ауторегуляции сосудистого тонуса и кровотока, а также повышение проницаемости сосудистой стенки. Наиболее интенсивные исследования микроциркуля-ции были выполнены в 80-90-е гг. ХХ века при помощи лазер-ной допплеровской флоуметрии.Регуляция кровообращения в микроциркуляторном русле контролируется как нейрогенными, так и гуморальными меха-низмами.Нейрогенная регуляция сосудистого тонуса обеспечивается ноцицептивными С-волокнами. При повреждении кожи сти-муляция этих волокон приводит к антидромному проведению импульса и секреции субстанции Р и брадикинина, которые вызывают вазодилатацию в зонах повреждения (реактивную ги-перемию, или так называемое нейрогенное воспаление). В со-стоянии покоя у здоровых лиц при температуре комфорта только 10-15% кожного кровотока осуществляется через капилляры, а остальная кровь перфузируется через артерио-венозные шунты, поддерживая терморегуляцию [3,4]. Регуляция величины шун-тового кровотока осуществляется симпатическими волокнами: при снижении импульсации по ним происходит раскрытие арте-рио-венозных шунтов. Впервые этот эффект был описан у симпат-эктомированных собак [5]. Нейрогенные механизмы участвуют также в регуляции микроциркуляции в ортостазе. Повышение тонуса венулярного аппарата в ортостазе активируется местным симпатическим аксон-рефлексом с небольшим (примерно 20%), также симпатическим центральным компонентом [6,7].Одним из проявлений диабетической полинейропатии нижних конечностей является поражение симпатических во-локон, которое может достигать степени аутосимпатэктомии. Вклад нейрогенных механизмов в разные проявления микро-ангиопатии, по-видимому, неодинаков, и, если в отноше-нии шунтового кровотока и постуральной вазоконстрикции они имеют основное значение, то в формировании реактивной гиперемии их роль относительно невелика и, по-видимому, не превышает 30% [8]. Efficacy of sulodexin in diabetics with atherosclerosis obliterans of the lower limbs was assessed from changes in clinical manifestations of intermittent claudication, results of treadmill test, dopplerography of the lower limb vessels, lipidogram, and time course of coagulologic values. Twenty patients with involvement of the lower limb arteries with stages I I-1II according to Fontain were examined. After 10 intramuscular injections of Vessel Due F in a dose of 600 LRU, the drug was administered orally in a dose of 250 LRU twice a day for 12 weeks. Clinical improvement was attained in 88% of patients. The distance of painless walking increased from 152.7+22.2 to 283.6+52.4 m (by 86%), maximal distance from 221.2+29.5 to 376.6+56.9 m (by 70%). Study of changes in coagulologic values over the course of treatment showed increase of activated partial thromboplastin time from 30.7+2.7 to 38.4+1.5 sec (p<0.02), decrease of serum fibrinogen from 4.0+0.1 to 3.2+0.1 g/liter (p<0.003), and increase of thrombin time from 21.7+4.7 to 26.7+3.4 sec (p<0.006). Cholesterol decreased from 6.7+1.0 to 6.27+0.3 mmole/liter (p<.05) and triglycerides showed a tendency to decrease from 1.9+0.23 to 1.7+0.2 mmole/liter (NS). No allergic reactions or side effects were observed during therapy.
Type 2 diabetes mellitus (DM) is a global epidemic associated with severe vascular complications development. Diabetic neuropathy is the most common chronic complication of DM that worsens patients life quality and prognosis. Therefore, studies dealing with DM and diabetic neuropathy underlying mechanisms are extremely relevant. The review discusses current views on vitamin D role in glucose metabolism and inflammatory processes. It is reported that vitamin D deficiency can contribute to insulin resistance development, and change in vitamin D receptor activity or extra- and intracellular calcium concentration due to vitamin D deficiency can affect pancreatic -cells function and lead to decrease in insulin production. Key pathogenic mechanisms of diabetic neuropathy as well as possible relationship between vitamin D deficiency and neuropathy development are in focus of this review. Results of recent clinical trials regarding vitamin D supplementation in patients with DM are also discussed. The presented data suggest that vitamin D deficiency can be considered as a non-classical risk factor for the development of not only DM but its complications as well.
