PURPOSE: To study the effects of peritoneal lavage with a 2% lidocaine solution, on the survival of the rats submitted to peritonitis caused by their own feces. METHODS: Forty-eight Wistar rats, weighting between 300g and 330g (mean, 311,45 ±9,67g), were submitted to laparotomy 6 hours following induction of fecal peritonitis. Animals were randomly divided into four groups of 12 each as follows: 1- Control, no therapy; 2- Drying of the abdominal cavity; 3- Peritoneal lavage with saline and drying; 4- Peritoneal lavage with a 2% lidocaine solution and drying. Animals that died were submitted to necropsy and the time of their death recorded; survivors were killed on the post-operation 11th day and necropsied. RESULTS: Death occurred within 52 h in all animals of group 1; within 126 h in 100% of those of group 2; within 50 h in 50% of those of group 3. All animals of group 4 survived. Survival on the 11 th day was higher in groups 3 and 4 than in groups 1 and 2 (p<0.001), and higher in group 4 than in group 3 (p<0.01). CONCLUSION: Peritoneal lavage with a 2% lidocaine saline solution without adrenaline, prevented the mortality of all animals with fecal peritonitis .
RESUMOOBJETIVO. Estudar aspectos funcionais e morfológicos do pólo inferior do baço de ratos tratados ou não com oxigênio hiperbárico no pós-operatório. MÉTODOS. Foram operados 79 ratos, Wistar, pesando 248,7g ± 27 , distribuídos em dois grupos: A -Simulação (n= 40), B -pólo inferior (n=39), e divididos em dois subgrupos: 11 e 70 dias. Cada subgrupo foi subdividido em não tratado com oxigênio hiperbárico (nt) (A11nt-n=10, B11 nt-n= 13, A70nt-n= 10, B70nt-n= 9) e tratado (t); A11t-n=10, B11t-n=9, A70 t-n= 10, B70t-n= 8). Foram dosados os lípides e imunoglobulinas e contadas as plaquetas e os corpúsculos de HowellJolly no pré e no pós-operatório. O baço e o pólo inferior foram retirados para histologia. RESULTADOS. Houve aumento do colesterol total (p=0,002), VLDL-colesterol e triglicérides (p= 0,003), da 013) INTRODUÇÃOA esplenectomia total pode provocar complicações no transoperatório e no pós-operatório 1 . Dessas, a mais preocupante é a infecção fulminante que foi relatada por King e Schumaker Jr 2 e foi recentemente revista 3 . As complicações infecciosas, após a retirada do baço, foram também observadas em animais de experimentação 4,5 . As alterações no metabolismo lipídico têm sido relatadas após esplenectomia total tanto em seres humanos 6 quanto em animais de experimentação 7-11 e podem causar aterosclerose 7 . Por essas razões têm sido valorizadas as operações conservadoras sobre o baço que compreendem as oclusões vasculares [12][13][14][15][16] , esplenorrafias 17 , esplenectomias parciais 18-20 e os auto-implantes 21,22 . Dessas, a mais recentemente descrita, em cães, foi a esplenectomia subtotal com preservação do pólo inferior, mesmo com a ligadura dos vasos esplênicos principais 23 . Nessa operação, observou-se aumento significante do colesterol e da fração LDL no pós-operatório mediato, e sugeriu-se a possibilidade de isquemia do pólo inferior remanescente. Por esse motivo, seria interessante utilizar um método para combater a isquemia e, possivelmente, melhorar a função do pólo inferior, no pós-operatório mediato.A oxigenoterapia hiperbárica (OHB) é um método terapêutico no qual o indivíduo é submetido à inalação intermitente de oxigênio (O 2 ) a 100%, a uma pressão maior que a atmosférica, no interior de uma câmara hiperbárica 24 . Essa modalidade terapêutica tem sido utilizada no tratamento de várias doenças. No Brasil, a Resolução CFM 1457/95 do Conselho Federal de Medicina regulamentou seu uso nas seguintes indicações: embolia gasosa, embolia traumática pelo ar, envenenamento por monóxido de carbono, gangrena gasosa clostridiana, doença de Fournier, isquemias agudas traumáticas, queimaduras térmicas ou elé-tricas, lesões por irradiação, osteomielite, retalhos ou enxertos comprometidos ou de risco. JUSTIFICATIVA E OBJETIVOS:Com base nos conhecimentos sobre a ação antiinflamatória e antibacteriana dos anestésicos locais (AL) o estudo teve como objetivo determinar o efeito da lavagem peritoneal com solução de bupivacaína na sobrevida de ratos com peritonite fecal por fezes autógenas. MÉTODO:Foram utilizados 48 ratos da linhagem Wistar, com peso entre 300 g e 330 g (311,45 ± 9,67), submetidos à laparotomia seis horas após a indução de peritonite, distribuídos aleatoriamente em quatro grupos: 1 -Controle, nenhum tratamento (n = 12); 2 -Enxugamento da cavidade abdominal (n = 12); 3 -Lavagem da cavidade abdominal com 3 mL de solução fisiológica a 0,9% e enxugamento (n = 12); 4 -Lavagem da cavidade abdominal com 8 mg.kg -1 (± 0,5 mL) de bupivacaína 0,5%, adicionada a 2,5 mL de solução fisiológica a 0,9% e enxugamento (n = 12). Os animais que faleceram foram necropsiados e o horário do óbito foi anotado. Os animais sobreviventes foram mortos no 11° dia do pós-operatório e realizou-se a necropsia.RESULTADOS: Houve 100% de mortalidade nos animais do Grupo 1, em 52 horas, 100% nos animais do Grupo 2, em 126 horas e 50% nos animais do Grupo 3 em 50 horas. Os animais do Grupo 4 sobreviveram. A sobrevida, no 11 o dia de pós-operatório, foi maior nos grupos 3 e 4 com relação aos grupos 1 e 2 (p < 0,001) e maior nos Grupo 4 com relação ao Grupo 3 (p < 0,01). CONCLUSÕES:A lavagem peritoneal com solução de bupivacaína diluída em solução fisiológica foi eficaz para evitar o óbito, por 11 dias, em 100% dos animais com peritonite fecal. BACKGROUND AND OBJECTIVES:Based on the knowledge of the anti-inflammatory and anti-bacterial actions of local anesthetics (LA), the objective of this study was to determine the effects of peritoneal lavage with bupivacaine on survival of mice with fecal peritonitis. METHODS:Forty-eight Wistar mice, weighing between 300 and 330 g (311.45 ± 9.67 g), undergoing laparotomy 6 hours after induction of peritonitis were randomly divided in 4 groups: 1 -Control, without treatment (n = 12); 2 -Drying of the abdominal cavity (n = 12); 3 -Lavage with 3 mL NS and posterior drying of the abdominal cavity (n = 12); and 4 -Lavage with 8 mg.kg -1 (± 0.5 mL) of 0.5% bupivacaine added to 2.5 mL of NS followed by drying out of the abdominal cavity (n = 12). Animals that died underwent necropsy and the time of death was recorded. Surviving animals were killed on the 11 th postoperative day and underwent necropsy. RESULTS:Group 1 presented a 100% mortality rate in 52 hours, 100% mortality rate in Group 2 in 126 hours, and Group 3 presented a 50% mortality rate in 50 hours. Animals in Group 4 survived. Purpose :To assess functional and morphological aspects of spleen auto-implants and of the splenic inferior pole of rats, post-operatively treated or not with hyperbaric oxygen, as well as the survival of these animals, were studied. Methods: Seventy-eight male Wistar rats, weighing between 192 and 283 g ( 238,3 ± 9,6g), were randomly distributed into three groups: Group1-(n=20), spleen manipulation; group 2-(n=36), spleen auto-implantation; group3-(n= 22), subtotal splenectomy preserving the inferior pole. Each group was subdivided as follows: subgroup a, not submitted to hyperbaric oxygen therapy: 1a(n=10), 2a(n=21), 3a(n= 13); subgroup b, submitted to the therapy: 1b(n=10), 2b(n=15), 3b(n=9). Blood was collected pre-operatively and 11 days after surgery, for the estimation of lipids and immunoglobulins and the counting of platelets and Howell-Jolly corpuscles. The spleen and remains were taken for histological study. Results : The number of surviving animals was significantly higher in groups 1(p<0,01) and 3(p<0,05) relative to those of subgroup 2a. Total cholesterol and the LDL fraction increased significantly in subgroup 2a (p<0,01) and 3a (p<0,05), and remained unaltered in subgroups 2b e 3b. IgM decreased more significantly in subgroup 2 than in subgroup 3 (p<0,001 vs p<0,01). The increase of platelet numbers and the appearance of Howell Jolly corpuscles was smaller in subgroup 2b compared to subgroup 2a , and in group 3 compared to group aqui-> 2. The macro and microscopic appearance in subgroup 2b were more viable than in subgroup 2a, and that of group 3 more viable than in group 2. The survival of the animals carrying their whole spleen or its inferior pole was more frequent than that of the auto-implanted animals. Conclusion : Functionality and viability of the whole spleen or of its inferior pole, were better than in the auto-implanted animals. Hyperbaric oxygentherapy contributed to increased survival frequency of auto -implanted animals, and to improve the functionality and viability of the auto-implants and the function of the inferior splenic pole, and did not interfere in animals carrying their whole spleen.
