Objective: To analyze the survival of exclusive breastfeeding and the factors associated with its cessation in the first month among pairs seen by a lactation consulting team. Method: This is a prospective cohort conducted with mother-infant pairs treated at the Hospital de Clínicas of Porto Alegre. Results: The sample consisted of 150 pairs. The survival curve indicates that 52.9% of the children remained on exclusive breastfeeding. The hierarchical model was constructed in four levels, and the factors associated with the cessation of exclusive breastfeeding were the milk supplementation during hospitalization, breast problems after hospital discharge and use of pacifiers. Conclusion: Awareness of these factors favors the early detection of pairs that may be predisposed to cessation of exclusive breastfeeding, who require greater support, dedication and care.
RESUMOObjetivo: identificar na literatura científica os diagnósticos de enfermagem que se relacionam diretamente com o aleitamento materno, bem como elencar os mais utilizados nessa prática. Métodos: trata-se de uma revisão integrativa, realizada em novembro de 2016, nas bases de dados LILACS, CINAHL, SCOPUS, PubMED e SciElo. Para a seleção dos estudos foram utilizados os Descritores em Ciências da Saúde "Diagnóstico de Enfermagem" e "Aleitamento Materno", bem como seus sinônimos e Medical Subject Headings correspondentes. Resultados: os diagnósticos de enfermagem que se sobressaíram na busca realizada foram amamentação eficaz e amamentação ineficaz. Conclusão: os artigos incluídos neste estudo permitiram uma visão do que os enfermeiros encontram na sua assistência frente ao desenvolvimento do aleitamento materno, bem como os diagnósticos mais elencados nessa prática. Objective: To identify the factors associated with the maintenance of exclusive breastfeeding and to verify the justification for the introduction of other liquids in the late postpartum of mothers and children attended by a breastfeeding consultant. Method: A non-comparative prospective cohort, conducted with 150 mothers and children. Data was collected at rooming-in 15 and 30 days after birth and analyzed by univariate analysis and multivariate regression. Results: The reasons for introducing water, tea, and breast milk substitutes were the following: thirst, abdominal cramps, and infant crying, respectively. Vaginal delivery, not using a pacifier or bottle and seeking professional help after discharge were associated with the exclusivity of late postpartum breastfeeding. Conclusion: The introduction of a professional breastfeeding consultant is of significant importance in the rise of exclusive breastfeeding rates.
OBJECTIVE: To identify factors associated with umbilical cord clamping in term newborns and to compare the recording of clamping time in the medical record with what was observed. METHOD: Cross-sectional study, with 300 mothers-infants, in a university hospital. Clamping time and medical records were observed, and a structured questionnaire was applied to postpartum women for sociodemographic variables. Bivariate analysis, multivariate Poisson Regression model, and Kappa concordance test were performed. RESULTS: The percentage of late/optimal clamping observed was 53.7%. The associated factors were skin-to-skin contact in the delivery room (PR = 0.76; 0.61–0.95; p = 0.014), position of the newborn below the vaginal canal (PR = 2.6; CI95%: 1.66–4.07; p < 0.001), position of the newborn at the vaginal level (PR = 2.03; CI95%: 1.5–2.75; p < 0.001), and need for newborn resuscitation in the delivery room (PR = 1.42; CI95%; 1.16–1.73; p = 0.001). Kappa concordance level of the professionals, records compared to the observation was: nurse 0.47, obstetrician 0.59, and pediatrician 0.86. CONCLUSION: the identification of associated factors and the comparison between recording and observing the clamping time can help in the planning and implementation of improvements for adherence to good practices at birth.
Objetivo:Analisar as internações por condições sensíveis à atenção primária, relacionadas às doenças do pré-natal e parto no país após a implementação da Rede Cegonha. Metodologia:Estudo descritivo, retrospectivo, realizado a partir da consulta de dados secundários disponibilizados através do Departamento de Informática do Sistema Único de Saúde, durante os anos de 2011 a 2013.Resultados:No período estudado houve um acréscimo de 5.606 casos de internações por condições sensíveis à atenção primária relacionadas às doenças do pré-natal e parto a nível nacional, sendo que a região de maior destaque foi a sudeste com 44.384 hospitalizações. Essa pesquisa apresentou um índice crescente deinternações por condições sensíveis à atenção primária por doenças relacionadas ao pré-natal e parto mesmo com a regulamentação da Rede Cegonha. Considerações finais: O aumento das internações demonstra os desafios a serem enfrentados pela atenção primária, visando a redução de adversidades consideradas evitáveis.
Objetivo: caracterizar os encaminhamentos e a resolutividade da consultoria em aleitamento materno em uma unidade de alojamento conjunto. Método: estudo transversal descritivo realizado com 231 puérperas e seus recém-nascidos internados em alojamento conjunto no sul do Brasil. A coleta de dados foi realizada no período de agosto de 2016 a maio de 2017. Os dados foram analisados mediante estatística descritiva. Resultados: os motivos prevalentes para o encaminhamento foram dificuldade na técnica de amamentação (81,7%), primiparidade (57,8%), anatomia mamária (28,7%), presença de fissuras mamilares (19,6%) e dor ao amamentar (18,3%). A satisfação com o atendimento foi relatada por 97,8% das puérperas. Quanto a resolutividade, 70,6% considerou o atendimento totalmente resolutivo, 26,4% parcialmente e 3,0% não resolutivo. Conclusão: a consultoria em aleitamento materno foi resolutiva na maioria dos encaminhamentos, motivados por dificuldades que poderiam predispor uma interrupção precoce da amamentação, demonstrando a eficácia da inserção desse profissional nos serviços de saúde.
