Objective: Identify the route of birth delivery preferred by mothers and their motivations. Method: Cross-sectional study with 361 mothers from a university hospital. Data were obtained using a structured questionnaire from February to April 2013. Descriptive analysis was performed and the Chi-square test was used to verify association among the variables. Results: 77.6% of the women preferred vaginal delivery and the reason reported by 81.8% of them was easier recovery postpartum; 20.5% believed they took part in the decision-making concerning the type of delivery; 64.5% believed the type of delivery they experienced did not involve risk for themselves, while 21.9% believed it involved risk for the newborn. Statistical association was found between number of deliveries and type of previous delivery with the preferred route of delivery. Conclusion: Women did not have adequate knowledge regarding the risks and benefits of different types of delivery and for this reason were not empowered to exert their autonomy in regard to this decision. Resultados: 77,6% das mulheres preferiram o parto vaginal, e o motivo, para 81,8% destas, foi melhor recuperação pós-parto; 20,5% acreditaram ter participado da decisão sobre o tipo de parto; 64,5% acreditavam que o parto ao qual foram submetidas não envolveu riscos para si e 21,9% acharam que envolvia riscos para o recém-nascido. Houve associação estatística entre paridade e tipo de parto anterior com a via de parto preferida. Conclusão: As mulheres não demonstraram conhecimento adequado sobre os riscos e benefícios dos tipos de parto e, por isso, acredita-se não estarem empoderadas para exercer sua autonomia nessa decisão. Hubo asociación estadística positiva entre paridad y tipo de parto anterior con la vía preferida. Conclusión: Las mujeres no tenían conocimiento adecuado sobre riesgos y beneficios de los tipos de parto y, por ello, no creen estar empoderadas para ejercer su autonomía. Palabras clave:
Objetivo comparar, após transcorridos quatro anos da implementação da Rede Cegonha, as práticas obstétricas desenvolvidas em um hospital universitário segundo classificação da Organização Mundial da Saúde. Método estudo transversal realizado no ano de adesão à Rede Cegonha (377 mulheres) e replicado quatro anos após (586 mulheres). Dados obtidos mediante prontuário e questionário estruturado. Na análise, utilizou-se o Teste Qui-quadrado. Resultados quatro anos após a Rede Cegonha, dentre as práticas da Categoria A (práticas comprovadamente úteis/boas práticas), aumentou a frequência de acompanhante, de métodos não farmacológicos, de contato pele a pele e de estímulo à amamentação e diminuiu a liberdade de posição/movimentação. Na Categoria B (práticas prejudiciais), houve redução de tricotomia e aumento de venóclise. Na Categoria C (práticas sem evidências suficientes), o Kristeller apresentou aumento. Na Categoria D (práticas utilizadas de modo inadequado), aumentou o percentual de toque vaginal acima do recomendado, de analgésicos e de analgesia e diminuiu a episiotomia. Conclusão esses resultados indicam a manutenção de uma assistência tecnocrática e intervencionista e direcionam para a necessidade de mudanças no modelo de atenção obstétrica. Um caminho consolidado mundialmente é a incorporação de enfermeiras obstetras/obstetrizes na assistência ao parto pelo potencial de utilização apropriada de tecnologias e redução de intervenções desnecessárias.
Objective To know the nurses' perception about early skin-to-skin contact. Methods A qualitative and exploratory-descriptive study carried out at the Obstetric Center of a university hospital in southern Brazil. The participants were eight nurses who answered a qualitative interview in April 2019. The interviews followed a guiding script and were analyzed according to thematic content analysis: data sorting and classification and final analysis. Results Nurses know the skin-to-skin contact practice and the importance of doing it properly. In their work routine, they perceive that this contact does not happen as it should and they understand the importance of recording it. Conclusion Nurses aim for skin-to-skin contact to be carried out in an early and uninterrupted manner; however, the limitations related to the health team's work processes are highlighted. The study reinforces the importance of reducing unnecessary interventions at birth.

Objective: To determine the prevalence of skin-to-skin contact (STSC) and breastfeeding (BF) stimulation, as well as the reasons for these practices not to be performed; to identify whether women were given information on these practices along the pre-natal monitoring. Method: A cross-sectional study carried out in a university hospital with 586 women. Data collection was from February to September 2016. Data was collected from patient records and from a questionnaire. A descriptive analysis was performed. Results: Immediately after birth, 60.1% of the newborns (NBs) had STSC, and 44.9% were stimulated to suck at the breast. After primary care, 24.1% had STSC, and 69.3% were stimulated to suck at the breast; 47.7% of the newborns did not have STSC due to their unfavorable clinical conditions; 79.2% of the women were not able to inform the reason why BF was not stimulated; 58.5% of the women had pre-natal guidance about STSC, and 90.8% about BF. Conclusion: Both STSC and BF rates could be improved, in view of the benefits provided by these practices.
The healthcare institution observes the legal provisions for the presence of a companion, but there is still the need to build awareness among healthcare professionals on the importance of companions during the moments following childbirth, and to adapt the location to accommodate the companions who will support the women.
Objetivo: identificar a prática de métodos não farmacológicos implementadas para o alívio da dor de parto em um hospital de ensino, os motivos que levaram a utilizá-los e o grau de satisfação.Materiais e métodos: estudo transversal, desenvolvido com 586 puérperas em um hospital de ensino do Brasil. A coleta de dados ocorreu entre fevereiro e setembro de 2016, tendo como fonte os registros dos prontuários, a carteira de pré-natal e um questionário estruturado aplicado 12 horas após o parto. Procedeu-se a uma análise descritiva por meio do SPSS, versão 18.Resultados: os métodos não farmacológicos mais conhecidos pelas mulheres foram banho (83,1 %) e deambulação (81,4 %). No hospital universitário, 55,5 % receberam orientação/informação sobre os métodos e o mais aceito foi o banho (66,6 %). O motivo mais relatado foi diminuição da intensidade/alívio da dor (71,8 %); 89,4 % consideram que a prática deste método lhes trouxe benefícios; para 79,9 %, o grau de satisfação foi maior ou igual a sete.Conclusão: o uso de métodos não farmacológicos é uma prática eficiente para o alívio da dor de parto. É importante empoderar e informar as parturientes quanto às estratégias disponíveis para o alívio da dor durante o trabalho de parto para que possam, nesse momento e em conjunto com os profissionais de saúde, escolher o melhor método.
Objective: to identify breastfeeding patterns, survival of exclusive breastfeeding and factors associated with its interruption, in the first six months of life of babies seen by Lactation Consulting. Method: a prospective cohort, with 231 mother-babies in a Baby-Friendly Hospital. An initial questionnaire was applied after 24 hours of birth, after consulting, as well as a follow-up questionnaire, applied by telephone at 15, 30, 60, 120 and 180 days, with sociodemographic and obstetric variables, maternal habits, father’s schooling, birth data and baby feeding. Survival Analysis was carried out. Results: at 180 days of age, exclusive breastfeeding was 12.7% and the probability was 19.6% in the analysis of the survival curve. The factors associated with its interruption were smoking during pregnancy (HR 1.66; CI 1.05 - 2.61), age ≥ 35 years old (HR 1.73; CI 1.03 - 2.90), difficulty in breastfeeding after hospital discharge (HR 2.09; CI 1.29 - 3.41), search for professional assistance (HR 2.45; CI 1.69 - 3.54) and use of a pacifier (HR 1.76; IC 1.21 - 2.58). Conclusion: lactation consultancy contributed to the improvement of the exclusive breastfeeding rates, although there are opportunities for advances.
