Background & Aims: One of the basic principles of health care is to maintain patient safety. Today, patient safety is a priority for any healthcare system that seeks to ensure and improve the quality of care. Being safe from dangers and injuries when receiving healthcare services is one of the most obvious and fundamental human rights. Despite advances in healthcare, patient safety, and patient-centeredness are still worrying issues around the world. According to the World Health Organization, tens of millions of people die or become disabled annually due to clinical errors or unsafe treatment processes, which is one of the five leading causes of death in the world. Recent reports also show that approximately, 13.5% of hospitalized patients experience at least one adverse event during their hospitalization each year, 44% of which is preventable. The biggest challenge to make the healthcare system safer is changing the culture. Assessing the patient safety culture can provide information on how structures and processes within the system affect patient outcomes. However, the question remains that, given the structure of the existing healthcare system and the fact that in recent years, patient safety has been at the forefront of accreditation of medical centers, has the safety culture been able to find and establish its place in these centers and has the staff of medical centers (especially nurses) paid attention to patient safety culture due to the changes that have taken place in the past years. Therefore, this study was conducted to compare patient safety culture in nurses of Sabzevar Vasei Hospital between 2016 and 2020. Materials & Methods:The present study is a descriptive-analytical cross-sectional study that was conducted between 2016 and 2020. 123 nurses of Vasei Hospital were selected in 2016 and 124 were selected in 2020 via convenience sampling. Inclusion criteria were having at least 6 months of work experience and consent to participate in the present study, applied in both 2016 and 2020. A two-part questionnaire was used to collect data. In the first part, the demographic information of the samples including age, gender, field and degree, occupation, type of employment, work experience in the hospital, work experience in the recent ward, working hours per week were examined. The second part of the questionnaire included the Persian version of the "Patient Safety Culture Survey" (HSOPSC). The questionnaire was designed by American Agency for Healthcare Research and Quality in 2004. In the present study, the questionnaire was given to 10 experts to confirm the validity, and their opinions were applied. The reliability of the questionnaire was investigated in 2016 using Cronbach's alpha coefficient which was between 0.74-0.81 for safety culture and its various dimensions. This research is motivated by the problem of students who look lazy and not enthusiastic when asked to memorize the Qur'an. The purpose of this study was to determine the effect of applying the ACQ method on students' ability to memorize the Qur'an. This research departs from the idea that the method is a way of presenting subject matter carried out by educators so that the learning process occurs in students in an effort to achieve goals. With the ACQ method, students feel like they are playing, not pressured, enjoyable in every learning session, making them love listening to the Qur'an and far from feeling compelled. The method used in this study is a quasi-experimental form of nonequivalent control group design. The sample used in this study amounted to 31 experimental class students and 33 control class students at UPI Laboratory Junior High School, Cibiru Campus. Data collection techniques used are tests, observations, and documentation studies. Quantitative data were analyzed using a statistical test approach and qualitative data were analyzed using logic. From the research results obtained that: 1). The process of implementing the ACQ method begins with reading the QS. An-Naba, providing information related to learning steps, students sit in groups of 6 people, the teacher gives examples of memorizing the Qur'an and directs students to repeat each gesture, the teacher gives 10 minutes for students to memorize, students go forward to demonstrate their memorization . 2). Increased students' ability to memorize the Qur'an after using the ACQ method by 3%. 3). The application of the ACQ method has a significant effect on students' ability to memorize the Qur'an. Shown by the results of hypothesis testing with a significance level of 5% obtained Tcount 3.30 > Ttable 1.67022, then the proposed hypothesis is accepted.. Penelitian ini dilatarbelakangi oleh masalah pada siswa yang terlihat malas dan tidak bersemangat jika disuruh menghafalkan Al-Qur’an. Tujuan penelitian ini untuk mengetahui pengaruh penerapan metode ACQ terhadap kemampuan hafalan Al-Qur’an siswa. Penelitian ini berangkat dari pemikiran bahwa metode adalah cara menyajikan materi pelajaran yang dilakukan oleh pendidik agar terjadinya proses pembelajaran pada diri siswa dalam upaya untuk mencapai tujuan. Dengan metode ACQ siswa merasa seperti sedang bermain, tidak tertekan, menyenangkan dalam setiap sesi pembelajaran membuat mereka cinta dalam mendengarkan Al-Qur’an dan jauh dari rasa keterpaksaan. Metode yang digunakan dalam penelitian ini adalah quasi eksperiment bentuk nonequivalent control group design. Sampel yang digunakan dalam penelitian ini berjumlah 31 siswa kelas eksperimen dan 33 siswa kelas kontrol di SMP Laboratorium-Percontohan UPI Kampus Cibiru. Teknik pengumpulan data yang digunakan adalah tes, observasi, dan studi dokumentasi. Data kuantitatif dianalisis menggunakan pendekatan uji statistik dan data kualitatif dianalisis menggunakan logika. Dari hasil penelitian diperoleh bahwa: 1). Proses penerapan metode ACQ terlaksana diawali dengan membacakan QS. An-Naba, pemberian informasi terkait langkah-langkah pembelajaran, siswa duduk berkelompok beranggotakan 6 orang, guru memberi contoh menghafal Al-Qur'an dan mengarahkan siswa untuk mengulangi setiap gerakan isyarat, guru memberi waktu 10 menit untuk siswa menghafal, siswa maju mendemonstrasikan hafalannya. 2). Peningkatan kemampuan hafalan Al-Qur’an siswa setelah menggunakan metode ACQ sebanyak 3%. 3). Penerapan metode ACQ berpengaruh signifikan terhadap kemampuan hafalan Al-Qur’an siswa. Ditunjukkan dengan hasil uji hipotesis dengan taraf signifikansi 5% diperoleh Thitung 3,30 > Ttabel 1,67022, maka hipotesis yang diajukan diterima.
Reducing the risk of falling patients is one of the six patient safety goals. To reduce the risk of injury due to falls, the hospital makes the Standard Operating Procedures (SPO) prevention of patients fall as a reference to avoid Unexpected Event (KTD). One of the factors influencing the implementation of SPO is nurse compliance. The purpose of this study to determine the compliance of nurses in implementing standard operational procedures prevention of patients fall in the installation of inpatient hospital Leprosy Sumberglagah. The population in this research were all nurses in the installation of input hospitals Sumberglagah which amounted to 30 nurses, using total sampling technique and sample amounted to 30 nurses. This research uses an observation sheet instrument with 23 statements and is done in 2 weeks. The results of this research indicate that all nurses do not adhere to the Standard Operating Procedures (SPO) prevention of falling patients, of which 30 nurses (100%) in implementing prevention of patients fall out of SPO, either at low risk SPO or medium and high risk. Non-understanding of nurses on the instructions to be the cause of falling nurses on the implementation of SPO prevention of patients fell. In addition, attitude, age, employment and education factors also affect non-compliance of nurses
