Two cases of a large fungal ball of the paranasal sinuses and nasal cavity are presented and its removal via an endoscopic approach is detailed. The clinical symptoms of the fungal body of the paranasal sinuses are not specific. With one-sided localization and large sizes of fungal bodies, they must be differentiated from neoplasms. The most informative non-invasive diagnostic method is computed tomography. We are presenting two cases of giant fungal bodies of the paranasal sinuses and identified specific CT signs.
Objectives. Cryogenic surgical instruments have been successfully used to conduct tumor ablation. For a long time, cryoablation procedures in Russia were carried out using cryodestructors cooled with liquid nitrogen, which are able to remove heat from the ablation object with high heat flux density and quickly form a cryoablation zone, while being relatively cheap and easy to operate. However, these instruments turned out to be unsuitable for minimally invasive surgeries; therefore, they are squeezed out of practical medicine and cryosurgical instruments are cooled by throttling argon gas. This led to the purpose of the study - the choice of equipment for the organization of local supercooling of pathological tissue.Method. To solve the problem of optimizing the cryogenic pipeline, a method was chosen for finding the Pareto-optimal solution. To solve this problem, it is sufficient to increase the pressure in the fluid flow directed to the NCS using a liquid micropump. In the role of quality criteria in this task, we selected: hydraulic loss power and heat loss power.Result. The following results were obtained: the minimum pressure in the vessel, which ensures the movement of the fluid in a single-phase state, according to the magi-line of 1 m length is 0.75 MPa; With this pressure, through a line with a diameter of 1 mm, the flow rate is maintained up to 6 kg / h; heat dissipation ability of the instrument reaches 608 watts. The thermal load on the cooling system of the heatdissipating device of the device for CA is unsteady and is formed due to the heat reserve accumulated in the patient's tissues.Conclusion. The use of liquid nitrogen in the undersized cryosurgical equipment makes it possible to overcome the noted drawbacks of liquid cryodestructors.
Структурные особенности и диагностика аномалий развития осевого позвонка, дифференцированный выбор хирургической тактики Аннотация Цель. Изучение рентгенанатомии краниовертебральной зоны с учетом особенностей строения второго шейного позвонка для дифференциальной диагностики патологии и его вариативной анатомии в клиническом аспекте. Материалы и методы. В период с 2004 по 2014 г. в клинике обследованы 43 пациента, у которых выявлены особенности и аномалии развития зуба второго шейного позвонка (С II). Проведены клиникорентгенологические исследования с функциональными пробами, КТ, МРТ. Результаты. Дифференцировано осуществляли: фиксацию на уровне С I-С II позвонков; фиксацию на краниовертебральном уровне с вовлечением С I-С II позвонков; декомпрессию на уровне С I-С II позвонков; декомпрессию на краниовертебральном уровне с вовлечением С I-С II позвонков. После устранения компрессии спинного мозга обязательно выполнение фиксирующей операции. Регресс симптомов в среднем составлял 1,8 балла по шкале ASCIA. Отдаленные результаты изучены в сроки до 10 лет, в среднем 3,2 года. Выводы. Варианты развития зуба С II выявлены в 20% наблюдений; в некоторых-это индивидуальные варианты развития, в некоторых-аномалии и пороки. Наличие аномалий и пороков с нарушением краниовертебральных соотношений или возникновением вторичных патологических состояний с угрозой грубых неврологических расстройств является показанием к хирургической стабилизации краниовертебральной области, субокципитальной декомпрессии и ламинэктомии. Особенности анатомии зуба осевого позвонка нередко неправильно трактуют, что обусловливает затруднения при решении вопроса о показаниях к применению различных методов хирургического лечения и реабилитации, прогнозе и экспертной оценке. The review is devoted to new class of infectious agents-prions and their role in human illnesses mainly with CNS damage. Ethiological, patogenical and molecular-genetic features of this kind of pathology are given. The role of intracerebral way of infectious agent reception is shown. These data are important for differential diagnostics of human prion illnesses and for system development of their prophylaxis and safety.
