Burnout es un síndrome que se encuentra presente con más poderío en los profesionales de la salud conduciendo al compromiso de su accionar, afectando la calidad y atención del servicio que este brinda, este síndrome es una paradoja de la atención de salud: el profesional de salud afectado con el síndrome tiende a enfermarse de manera simultánea cuando este sana a los pacientes. Por definición el síndrome de burnout se lo considera como una entidad psicológica negativa secundaria al estrés interpersonal crónico del trabajo, este hace su aparición cuando la capacidad de afrontamiento es nula, evidenciando la falta de energía de la persona y de sus recursos, culminando en el fracaso y agotamiento. El síndrome de burnout es considerado como una entidad multidimensional, en la que se pueden presentar síntomas físicos y emocionales, con serias repercusiones a nivel personal, profesional y social, cuyos componentes identificados hasta la fecha son: agotamiento emocional, despersonalización y reducida realización personal. De esto parte la necesidad de describir al síndrome de burnout como riesgo psicosocial en la atención médica.
Introducción: El desgaste profesional ocurre tras la exposición sostenida al estrés crónico, tiene mayor connotación entre los profesionales sanitarios, principalmente las enfermeras, generando consecuencias amplias e importantes sobre la salud mental, física y el ejercicio profesional. Objetivo: detectar los niveles de desgaste profesional y la relación laboral que presentan las enfermeras que laboran en un hospital de segundo nivel en Manta, Manabí, Ecuador, durante la pandemia COVID-19. Método: se realizó un estudio descriptivo, transversal, no experimental, observacional y prospectivo. Se aplicó una encuesta durante el segundo trimestre del 2020, entre las enfermeras que laboraron en el área de emergencia COVID-19, se incluyeron variables sociodemográficas y laborales, el cuestionario Maslach Burnout Inventory. Se realizó análisis descriptivo e inferencial; se emplearon los estadísticos de Spermann. Resultados: Respondieron la encuesta 32 profesionales, con una población femenina en su totalidad; 22 profesionales (28,1%) no tenían estabilidad laboral; no se presentaron los 3 criterios positivos del síndrome, prevalecieron los niveles medios con 18.8 % cansancio emocional, 14.4% de despersonalización, mientras que la mayoría (78.1%) presentaba alta realización personal, el burnout moderado prevaleció en 87.5%, además, se observó una relación positiva y significativa (p=0.025), lo que reflejó que la relación laboral de contrato fue la más expuesta a un mayor desgaste profesional. Conclusión: en el área de emergencia COVID-19, las contratadas expresaron más compromiso en las 3 dimensiones, estos hallazgos resultan importantes para trazarse un estudio más amplio, y así establecer la detección temprana de esta patología que tiene implicaciones serias en el desempeño profesional.
La enfermedad de Parkinson (EP) es un trastorno neurológico progresivo conocido por una gran cantidad de características motoras y no motoras que pueden afectar la función en un grado variable. Esta revisión describe las características clínicas de la EP con énfasis en aquellas que diferencian la enfermedad de otros trastornos parkinsonianos. Debido a que no existe una prueba definitiva para el diagnóstico de la EP, la enfermedad debe diagnosticarse con base en criterios clínicos. El temblor en reposo, la bradicinesia, la rigidez y la pérdida de los reflejos posturales se consideran generalmente los signos cardinales de la EP. La presencia y la presentación específica de estas características se utilizan para diferenciar la EP de los trastornos parkinsonianos relacionados. Otras características clínicas incluyen síntomas motores secundarios (por ejemplo: Hipomimia, disartria, disfagia, sialorrea, micrografía, marcha arrastrando los pies, festinación, congelación, distonía, reflejos glabelares), síntomas no motores (por ejemplo: Disfunción autonómica, anomalías cognitivas / neuroconductuales, trastornos del sueño y anomalías sensoriales como anosmia, parestesias y dolor). La ausencia de temblor en reposo, la aparición temprana de dificultad para caminar, la inestabilidad postural, la demencia, las alucinaciones y la presencia de disautonomía, oftalmoparesia, ataxia y otras características atípicas, junto con una respuesta deficiente o nula a la levodopa, sugieren diagnósticos distintos de la EP. Un conocimiento profundo del amplio espectro de manifestaciones clínicas de la EP es fundamental para el diagnóstico adecuado de la enfermedad. Las mutaciones o variantes genéticas, las anomalías en la neuroimagen y otras pruebas son biomarcadores potenciales que pueden mejorar el diagnóstico y permitir la identificación de personas en riesgo.
El docente universitario está constantemente inmerso en un ambiente de trabajo, que le exige un desempeño laboral con gran gasto energético físico y psicológico, pudiendo afectar su salud. El objetivo de este trabajo fue evaluar el síndrome de Burnout en docentes de la Facultad de Ciencias de la Salud de la Universidad Técnica de Manabí, Ecuador, desde junio de 2018 a junio de 2021, mediante una investigación de tipo descriptiva, transversal, de campo y prospectiva, con diseño no experimental, en 53 docentes de diferentes niveles académicos. Prevalecieron las mujeres (73,6 %), mayores de cincuenta años (52,8 %). Destacó la falta de realización personal (40,77 ± 8,48), sin diferencias entre sexos, pero con respecto a la edad sobresalieron los menores de cincuenta años con la dimensión de agotamiento emocional (p˂0,05). No hubo predominio de niveles altos para la tríada del síndrome de Burnout, no obstante, hubo casos de docentes titulares con las tres dimensiones altas. En conclusión, los docentes, perciben altamente falta de realización personal y se detectaron casos del síndrome de Burnout, específicamente en docentes titulares, recomendándose su intervención y así promover la salud del profesor universitario y por ende un buen desempeño. El crecimiento y desarrollo integral del individuo desde la infancia hasta la adolescencia resume el contexto y ámbito de aplicación de la pediatría. El cuidado y atención que les brinda esta rama de la medicina a las personas desde el nacimiento hasta la adolescencia en lo concerniente a la salud física, mental y emocional se encuentran entre los fines más elevados de la ciencia médica por cuanto se trata de una medida de protección de la especie humana ya que el crecimiento y desarrollo de un niño sano en su integralidad cuerpo, mente y espíritu sienta las bases para su posterior conversión en un adulto con pleno dominio de todas sus capacidades y cualidades humanas. El presente libro, como su nombre lo exterioriza, contiene temas esenciales de pediatría, a saber: el niño sano, crecimiento físico y desarrollo motor del niño, nutrición infantil y salud, alteraciones en el neurodesarrollo infantil, alergias y enfermedades infecciosas en la infancia y urgencias pediátricas, está pensado como un material de apoyo en la tarea que realizan profesionales de la red básica de salud, personal médico y de enfermería pediátrica y no pediátrica, padres, maestros, estudiantes por lo que su alcance y profundidad se puede definir como básico. Es decir, contiene conocimientos esenciales que todo profesional de atención primaria de salud debe saber, extensivo también a aquellas personas que acompañan de una manera u otra el crecimiento y desarrollo de los niños por la gran ayuda que estas brindan en la vigilancia y el control de un desarrollo normal.
