Актуальність. За тяжкістю перебігу, частотою ускладнень та летальністю виразковий коліт посідає одне з провідних місць у структурі хвороб шлунково-кишкового тракту в дітей. Мета дослідження: дати характеристику перебігу виразкового коліту в дітей на сучасному етапі. Матеріали та методи. Проведено ретроспективний аналіз 184 історій хвороби дітей, які перебували на обстеженні та лікуванні у відділенні проблем захворювань органів травлення ДУ «ІПАГ НАМН України» з 2004 по 2014 рік. Результати. Виразковий коліт зустрічається в дітей усіх вікових груп, серед яких групами ризику є діти дошкільного віку та підлітки. До факторів ризику захворювання відносяться як анте-, так і постнатальні фактори: гестози та загроза переривання вагітності, дефіцит маси тіла при народженні, штучне вигодовування, кишкові інфекції. Виразковий коліт характеризується хронічним рецидивуючим перебігом з повільним розвитком хвороби, має типові клінічні прояви — діарея, гемоколіт, больовий абдомінальний синдром, інтоксикаційний синдром, затримка фізичного розвитку. За локалізацією запального процесу переважає тотальне ураження товстої кишки (59,6 %), дещо рідше діагностується лівобічний коліт (28,5 %) і проктосигмоїдит (11,9 %). Інформативними критеріями активності виразкового коліту є педіатричний індекс активності (PUCAI), рівень фекального кальпротектину, ряд показників загального аналізу крові (гемоглобін, лейкоцити, тромбоцити, швидкість осідання еритроцитів) та біохімічних досліджень (С-реактивний білок, α-2-глобуліни). Сучасна комбінована базисна терапія, за даними катамнестичного спостереження, ефективна у 22 % хворих із тотальним колітом, у 42–58 % — з лівобічним колітом. Монотерапія препаратами 5-аміносаліцилової кислоти в 68 % випадків була ефективною в пацієнтів із сегментарним колітом мінімального ступеня активності та 100 % хворих із проктосигмоїдитом. Висновки. Проведений аналіз історій хвороби дітей з виразковим колітом показав сучасну вікову структуру захворювання, наявність гендерних розбіжностей залежно від локалізації та тяжкості запально-деструктивного процесу в кишечнику, відсутність збігу між клінічною та лабораторною активністю хвороби, ефективність лікувальних схем, рекомендованих ECCO та ESPGAN, доцільність використання нових критеріїв оцінки запального процесу (педіатричний індекс (PUCAI), фекальний кальпротектин). Отримані дані можуть бути використані для обґрунтування науково-практичної програми з проблеми виразкового коліту в дитячому віці, спрямованої на розробку таких актуальних напрямків, як епідеміологія, патогенез, вікові клінічні особливості, оптимізація протоколів діагностики та лікування.

According to modern views based on evidence-based medicine, the concept of "poly-endometrium" is defined as a benign, exophytic nodophilic formation of the uterine mucus that consists of glands and stroma, predominantly fibrous, containing a "tangle" of thick-walled blood vessels. The aim of the study – to learn the morphological features of tissues of polyps of the body of the uterus and endometrium with the establishment of the role of inflammatory factor in the pathogenesis of hyperproliferative changes in women of reproductive age. Materials and Methods. The article presents the results of a survey of 62 patients of reproductive age with polyps in the body of the uterus. The presence of the glandular component in 79.03 % of cases, fibrotic polyps – 12.9 %, and micro-polyps – 8.06 % were determined, respectively. The combination of polyps with non-atypical endometrial hyperplasia was detected in 80 % of patients with micro-polyposis, in 63.6 % of women – with glandular-cystic polyps of the uterine body, in 44.4 % of the examined – with glandular-fibrous endometrial polyps and in 37.5 % of patients – with fibrous polyps. Immunohistochemically, CD138 expression was detected both in the tissues of the polyps of the uterus and adjacent endometrial tissues in 43.5 % of cases, which became a marker of the chronic endometritis in the part of the surveyed, which formed the basis of the pathogenetic approaches in the treatment of such a pathology. For processing data standard procedures using Microsoft Excel were used. The research was conducted in the Laboratory of Pathomorphology of the Department of O. Lukyanova Institute of Pediatrics, Obstetrics and Gynecology of the National Academy of Medical Sciences of Ukraine, head of the department – Corresponding Member of the National Academy of Medical Sciences of Ukraine, Professor T. Zadorozhna. Results and Discussion. In clinical analysis of complaints of polyps of the body of the uterus, clinical manifestations in the form of menstrual cord disturbance were 13 (20.97 %) patients, in 11 (17.74 %) patients there were no complaints about the desired pregnancy, at the same time as 38 (61.29 %) women were asked to carry out a preventive examination, which does not contradict the results of other researchers. During the morphological study, we established the following features of the polyps of the body of the uterus in women of reproductive age: the most frequently tested polyps of the body of the uterus, which had the glandular component – in 49 (79.03 %) cases, of which the glandular-cystic structure was found in 22 (35.48 %) of patients, the glandular-fibrous structure was in 27 (43.55 %) patients, approximately the same values ​​were found in fibrous polyps of the body of the uterus – 8 (12.90 %) of women and micropolymers of endometrium – in 5 (8.06 %) cases respectively. We found that in all groups of patients with polyps in the body of the uterus, there were signs of non-atypical hyperplasia of the endometrium. It should be noted that, according to literature, endometrial polyps larger than 15 mm were associated with hyperplasia. The above data is an important indication that the unidirectional determination of the positive expression of the CD138 inflammatory marker, both in the tissue of the polyp of the uterus and in the endometrial tissue, indicates the unity of the inflammatory factor in the pathogenesis of hyperproliferative processes and proves the significant role of the inflammatory process in the development of its pathological conditions and justifies the necessity take into account this fact in approaches to the tactics of patient management. Conclusions. The results of the conducted morphological and immunohistochemical research indicate that the basis of the formation of polyps of the body of the uterus is the inflammatory process, which is confirmed morphologically with the presence of chronic endometritis and a positive reaction to CD138. The study of pathogenetic mechanisms for the formation of endometrial pathology is a key factor in the development of methods for correction of these pathological processes, which thus allows to increase the efficiency of treatment and preserve reproductive health.

Objective: to investigate the morphological and immunohistochemical features of placental damage due to the incorporation of 137Cs depending on the scenario of pregnancy completion. Materials and methods. The study material consisted of placentas from 60 women with reproductive losses in anamnesis and signs of termination of the current pregnancy (first group) and placental samples from 30 women with an uncomplicated gestation and an unencumbered anamnesis (control group). The detailed study required the distribution of placental samples from the first group into subgroups. Subgroup 1a included 38 placentas from women who gave birth at 37–40 weeks, despite signs of termination of the current pregnancy. Subgroup 1b – placentas of 13 women who gave birth at a gestation period of 28–36 weeks + 6 days. Subgroup 1c – 9 placental samples from women who gave birth at a gestation period of 22–27 weeks + 6 days. The volumetric activity of the 137Cs in the placentas was measured using β-spectrometer. The histology of the placenta was studied using a standard technique. The following expressions were studied in placenta: CD31 / PECAM-1, CD45 / T200 / LCA, CD56 / NCAM-1, CEA / CD66e Ab-2, Vimentin, using indirect streptavidin peroxidase detection method. Results. Placentas accumulate 137Cs. The different volumetric activity of the isotope correlates with scenarios of pregnancy. Due to the action of incorporated 137Cs with a specific mass of more than 1.1 Bq/kg, placental dysfunction develops. The consequences of placental dysfunction depend on the volumetric activity of the 137Cs and the preservation of adaptive and compensatory reactions in the placenta. Morphological and immunohistochemical features of placental damage to incorporated 137Cs were established, depending on the scenario of completion of pregnancy. A marker of unfavorable completion of pregnancy is the expression of a carcinoembryonic antigen (CEA) in the placenta. Conclusions. Premature termination of pregnancy (PTP) is a multifactorial pathology associated with pathological changes in immune and neuroendocrine regulation and hereditary, infectious, and environmental factors that disrupt the adaptation mechanisms in the mother-placenta-fetus system. Intraplacental irradiation of 137Cs is one of the factors in the multifactorial nature of reproductive losses. As a result of intraplacental irradiation of 137Cs, the architecture of the placenta is disturbed, the activity of pro-inflammatory cytokines CD45 and CD56 increases, and the coagulation cascade is activated. Extreme effects depend on the volumetric activity of the isotope incorporated in the placenta and the organ’s compensatory capacity. Accumulation of up to 1.0 Bq/kg 137Cs does not affect the course of gestation. Internal irradiation with an activity of 4.5–10.4 Bq/kg 137Cs triggers late preterm labor. The nature of the damages corresponds to the category of «lesion of the maternal stroma» of the placenta. The volumetric activity of 137Cs over 10.4 Bq/kg is a probable cause of early preterm labor and antenatal fetal death. At the same time, the maternal and fetal structures of the placenta suffer damage. Expression of vimentin is a marker of placental destruction due to internal irradiation of 137Cs with a specific gravity of more than 4.5 Bq/kg. Expression of CEA in the structures of the placenta of women with PTP is a unique find and marker of premature birth and antenatal fetal death with intraplacental irradiation of 137Cs with an activity of more than 4.5 Bq/kg. Пролікували 29 пацієнтів віком від 2 місяців до 2 років із важким перебігом алергічного ентероколіту, яким виконали ендоскопічне дослідження з прицільною біопсією слизової оболонки кишечника. Для порівняння морфологічних, гістохімічних та імуногістохімічних особливостей використовували біоптати слизової оболонки кишечника дітей аналогічного віку з гастроінтестинальними розладами, що зумовлені тривалим чи хронічним перебігом неспецифічного невиразкового процесу в кишечнику.Імуногістохімічні дослідження виконали на серійних парафінових зрізах відповідно до стандартних протоколів, застосовуючи моноклональні антитіла CD68 (Macrophage Marker) Ab-3 (Clone KP1), антитіла IgA (Heavy Chain) Ab-2, IgE (epsilon-Heavy Chain) Ab-1 та систему візуалізації Ultra Vision Quanto Detectson System HRP DAB (USA).Результати. Дебют гастроінтестинальних симптомів у дітей раннього віку з алергічним ентероколітом виявляли в перші місяці життя, він характеризувався блювотою або стійкими зригуваннями, інтенсивними кольками, діареєю з домішками великої кількості слизу та/або крові, затримкою фізичного розвитку. Морфологічно встановили наявність запального процесу слизової оболонки кишечника та її еозинофільну інфільтрацію. У результаті імуногістохімічного дослідження виявили підвищену експресію CD68-макрофагів, IgA та IgE у слизовій оболонці дванадцятипалої кишки.Висновки. Діагностика алергічного ентероколіту в ранньому віці ускладнена відсутністю специфічних клінічних гастроінтестинальних симптомів і неінформативністю алергологічного обстеження (IgE-незалежний механізм розвитку запалення), що може потребувати здійснення морфологічного дослідження для верифікації діагнозу. Морфологічно в обстежених хворих встановили наявність запального процесу слизової оболонки кишечника зі значною інфільтрацією еозинофілами, за даними імуногістохімії виявили підвищену експресію CD68-макрофагів, IgA та IgE у слизовій оболонці дванадцятипалої кишки, що вказує на активацію як клітинного, так і місцевого гуморального імунітету.Клинико-морфологические особенности аллергического энтероколита у детей раннего возраста О. Імуногістохімічні особливості експресії прогестеронових рецепторів плацентарного бар'єра в жінок із багатоплідною вагітністю, що зумовлена ДРТ 1 ДУ «Інститут педіатрії, акушерства і гінекології НАМН України», м. Київ Ключові слова: прогестеронові рецептори, плацента, імуногістохімія, багатоплідна вагітність, ДРТ.Гормональні порушення є однією з основних відомих причин, що призводять до невиношування вагітності та передчасних пологів при багатоплідній вагітності, котра зумовлена допоміжними репродуктивними технологіями (ДРТ). Роль прогестерону та кількості його рецепторів відіграють важливу роль у збереженні та пролонгації вагітності. Вивчення плацент як основного місця синтезу прогестерону має високий інформативний потенціал та є найважливішим діагностичним об'єктом, а отримана при його дослідженні інформація абсолютно необхідна для повноцінного висновку про причини, механізм близьких і віддалених наслідків патології багатоплідної вагітності.Мета роботи -вивчення імуногістохімічних особливостей посліду у породіль із біхоріальною біамніотичною двійнею при спонтанному заплідненні та після застосування допоміжних репродуктивних технологій (ДРТ).Матеріали та методи. Обстежили 94 жінки, 44 з них мали багатопліддя, що зумовлене ДРТ, 42 -із самостійним багатопліддям і 38 жінок -з одноплідною вагітністю. Здійснений клініко-статистичний аналіз перебігу вагітності, пологів у групах, котрі досліджували.Встановили, що багатоплідна вагітність, котра зумовлена ДРТ, належить до гестації високого ступеня ризику, під час якого передчасні пологи трапляються значно частіше, ніж при одноплідній вагітності.Виявили, здійснивши імуногістохімічне дослідження плацент, найбільшу експресію прогестеронових рецепторів в ядрах децидуальних клітин (45%), що належать до материнської структури плаценти від жінок із багатопліддям, котре зумовлене ДРТ. Також виявили, що зі збільшенням терміну гестації спостерігається вірогідне зниження активності експресії прогестеро-нових рецепторів (від 45 до 2,5%) незалежно від способу зачаття й кількості плодів. Отже, результати дослідження вказують на безперечний зв'язок структур плацентарно-ендометріальних взаємовідносин, що є важливим компонентом у доцільності гормональної терапії. Background. Ulcerative colitis is a chronic inflammatory disease of unknown origin, characterized by a clinically recurrent course with periods of bloody diarrhea and pathomorpholo­gical-diffuse inflammatory process in the colon. The problem of ulcerative colitis requires further study of the clinical features of the disease, taking into account the localization, degree of activity of the inflammatory process, changes in the structure of the mucous membrane that will help increase the efficiency of ulcerative colitis diagnosis in childhood. Materials and methods. On the basis of clinical and statistical analysis of 116 case histories of children aged 4–18 years with ulcerative colitis, the features of its clinical forms — total, segmental and distal — were studied during the period of exacerbation of the disease. Four hundred and forty-five biopsy specimens obtained during colonoscopy were histologically examined. After biopsy sampling, specimens were fixed in 10% formalin and were processed according to the generally accepted histological method with section staining using hematoxylin-eosin and according to Van Gieson. Results. Changes in the architecto­nics of the large intestine mucosa, which reduce the resistance of the mucous barrier, as well as impaired blood supply — a factor in the development of hemic hypoxia — are significant for the mechanisms of ulcerative colitis exacerbation.
