Актуальність. За тяжкістю перебігу, частотою ускладнень та летальністю виразковий коліт посідає одне з провідних місць у структурі хвороб шлунково-кишкового тракту в дітей. Мета дослідження: дати характеристику перебігу виразкового коліту в дітей на сучасному етапі. Матеріали та методи. Проведено ретроспективний аналіз 184 історій хвороби дітей, які перебували на обстеженні та лікуванні у відділенні проблем захворювань органів травлення ДУ «ІПАГ НАМН України» з 2004 по 2014 рік. Результати. Виразковий коліт зустрічається в дітей усіх вікових груп, серед яких групами ризику є діти дошкільного віку та підлітки. До факторів ризику захворювання відносяться як анте-, так і постнатальні фактори: гестози та загроза переривання вагітності, дефіцит маси тіла при народженні, штучне вигодовування, кишкові інфекції. Виразковий коліт характеризується хронічним рецидивуючим перебігом з повільним розвитком хвороби, має типові клінічні прояви — діарея, гемоколіт, больовий абдомінальний синдром, інтоксикаційний синдром, затримка фізичного розвитку. За локалізацією запального процесу переважає тотальне ураження товстої кишки (59,6 %), дещо рідше діагностується лівобічний коліт (28,5 %) і проктосигмоїдит (11,9 %). Інформативними критеріями активності виразкового коліту є педіатричний індекс активності (PUCAI), рівень фекального кальпротектину, ряд показників загального аналізу крові (гемоглобін, лейкоцити, тромбоцити, швидкість осідання еритроцитів) та біохімічних досліджень (С-реактивний білок, α-2-глобуліни). Сучасна комбінована базисна терапія, за даними катамнестичного спостереження, ефективна у 22 % хворих із тотальним колітом, у 42–58 % — з лівобічним колітом. Монотерапія препаратами 5-аміносаліцилової кислоти в 68 % випадків була ефективною в пацієнтів із сегментарним колітом мінімального ступеня активності та 100 % хворих із проктосигмоїдитом. Висновки. Проведений аналіз історій хвороби дітей з виразковим колітом показав сучасну вікову структуру захворювання, наявність гендерних розбіжностей залежно від локалізації та тяжкості запально-деструктивного процесу в кишечнику, відсутність збігу між клінічною та лабораторною активністю хвороби, ефективність лікувальних схем, рекомендованих ECCO та ESPGAN, доцільність використання нових критеріїв оцінки запального процесу (педіатричний індекс (PUCAI), фекальний кальпротектин). Отримані дані можуть бути використані для обґрунтування науково-практичної програми з проблеми виразкового коліту в дитячому віці, спрямованої на розробку таких актуальних напрямків, як епідеміологія, патогенез, вікові клінічні особливості, оптимізація протоколів діагностики та лікування.
Purpose — analyse the state of the epithelial barrier of the colon in children with different clinical forms. Materials and methods. 42 children with acute chronic colitis were examined, including 28 with ulcerative colitis (14 with active total form, 14 with moderately active segmental); and 14 with chronic non-specific opaque colitis formed a comparison group. Laboratory methods were performed on all patients — hemogram, protein-gram, blood biochemistry, fecal calprotectin concentration; endoscopic examination with biopsy of all colon regions and histological examination of biopts. Results. The clinical manifestations of ulcerative colitis (UC) during the acute period were assessed by the Paediatric Activity Index (PUCAI) and depended on the localization and activity of the inflammatory process. The average for active colitis was found to be 50.2±1.8, moderate to 35.3±1.7, minimum to 24.1±1.2, but for children with total active inflammation 19 per cent of patients had the highest rates: 65, which corresponded to clinical signs of ulcerative colitis, accompanied by unidirectional changes of surface (dystrophic changes of epithelium, crypt deformation, reduced number of flax cells) and deep (diffuse inflammatory infiltration of its own plate, presence of crypt abscesses, cryptites, vascular dilation) structures of the mucous membrane of the large intestine, which are more pronounced in the active total forms of ulcerative colitis. The period of UC exacerbation is characterized by the violation of the epithelial barrier mucous membrane colon due to reduced mucus synthesis and changes in its biochemical properties, low secretory (MUC2) and membrane-associated (MUC4) expression Mucins, mainly in the active total forms of UC, loss of the regulatory effect of the club peptide on regeneration and protection of the mucous membrane of the intestine. Conclusions. Studies based on a pathogenetic approach to determining the cause of the exacerbation of the disease have shown evidence of a significant role in the epithelial barrier of the colon membrane, This is a significant addition to the known knowledge of ulcerative colitis pathogenesis in childhood. Актуальность. Воспалительные болезни кишечника на сегодняшний день являются одной из актуальных проблем детской гастроэнтерологии, что обусловлено стремительным распространением данной патологии, смещением дебюта болезни на ранний возраст ребенка (первые три года жизни), формированием тяжелых осложнений, низкой эффективностью современных схем лечения. Проблема язвенного колита у детей требует дальнейшего всестороннего изучения, в том числе факторов риска возникновения заболевания, что позволит формирование групп риска развития болезни, обоснование профилактических мероприятий по ее предупреждению. Материалы и методы. Изучение факторов риска проводилось путем опроса 100 матерей, дети которых болели язвенным колитом. Контрольную группу составили 30 матерей здоровых детей. Результаты. На основании математического анализа медико-биологических факторов риска развития язвенного колита у детей определены наиболее значимые: отягощенная наследственность по заболеваниям органов пищеварения, кишечные дисфункции в раннем возрасте, повторные курсы антибиотикотерапии, вторичная лактазная недостаточность, частые респираторные болезни. Разработан алгоритм прогнозирования язвенного колита в детском возрасте, обоснованы подходы к профилактике данной патологии.
В настоящее время воспалительные заболевания кишечника (ВЗК), в частности язвенный колит (ЯК) и болезнь Крона (БК), являются чрезвычайно актуальной и сложной проблемой современной медицины. По данным Всемирной гастроэнтерологической организации (WGO), по тяжести течения, частоте осложнений и летальности данная патология в подавляющем большинстве стран занимает одно из ведущих мест в структуре болезней желудочно-кишечного тракта (ЖКТ). Заболеваемость ВЗК неуклонно растет и составляет на сегодняшний день, по данным разных авторов, 30-240 случаев на 100 тыс. жителей для ЯК и 10-150 случаев-для БК [5, 9, 11, 20, 21]. Проблема язвенного колита и болезни Крона в детском возрасте в последние десятилетия приобретает все большее медико-социальное значение вследствие существенного увеличения распространенности заболеваний во всех странах мира, инвалидизации больных в результате тяжелых угрожающих жизни осложнений, высокого риска трансформации в колоректальный рак.
