Modelo do estudo: Estudo experimental. Introdução: A Doença Renal Crônica (DRC) refere-se a um diagnóstico sindrômico de perda progressiva e irreversível da função renal. O paciente submetido à hemodiálise pode apresentar limitações na capacidade funcional, função pulmonar e força muscular respiratória, com consequentes prejuízos na qualidade de vida. Objetivo: Avaliar os efeitos de um programa de exercício físico sobre a função pulmonar, capacidade funcional, qualidade de vida e dor, em pacientes que realizam hemodiálise. Metodologia: Participaram do estudo 28 pacientes de ambos os sexos, com idade entre 40 e 60 anos, em programa de hemodiálise no Instituto do Rim da Santa Casa de Misericórdia de Presidente Prudente-SP. A força muscular respiratória foi avaliada pela manovacuometria, a capacidade funcional, pelo TC6', a qualidade de vida, pelo questionário KDQOLSF, a função pulmonar, pela espirometria e a dor, pela EVA. Após as avaliações, os pacientes iniciaram o programa de exercícios, que foi desenvolvido três vezes por semana, durante 40 minutos em hemodiálise, por oito semanas. Ao final do programa, os pacientes foram reavaliados. Resultados: Não houve diferença significativa dos valores da CVF e VEF1 pré e pós-programa de exercícios, assim como do Índice de Tiffenau. O valor da PImax pós-programa foi significativamente maior que o obtido na avaliação pré-programa. Para a variável PEmax, não foi encontrada diferença significativa. As avaliações da capacidade funcional inicial e final não apresentaram diferenças significativas (p>0,05). A avaliação da qualidade de vida, quanto aos domínios das áreas específicas da DRC, mostrou que houve significância estatística, ao comparar a lista de sintomas e problemas com a sobrecarga da DRC e papel profissional. Os indicadores relativos à dor foram reduzidos, após o programa (p<0,05). Discussão: O DRC enfrenta situações complexas de dependência física, social e financeira. Apesar de não apresentar resultados estatisticamente significativos em todas as variáveis avaliadas, este estudo, corroborando outros encontrados na literatura, sugere um programa de exercício físico, com aspectos positivos para essa população. Conclusão: Embora a capacidade pulmonar e a capacidade funcional (TC6') não tenham apresentado alterações ao final do experimento, os níveis reduzidos de dor, cansaço e dispneia sugerem melhora do desempenho funcional, após programas de exercício físico para DRC.
RESUMOA incidência global de doenças renais crônicas vem aumentando a cada dia. No Brasil, aproximadamente 54,5 mil pessoas se encontram em tratamento dialítico. A doença renal crônica, diagnóstico sindrômico, progressivo e irreversível da função renal, culmina invariavelmente na terapia dialítica. Apesar dos avanços tecnológicos e dos benefícios no incremento da sobrevida, a hemodiálise acarreta complicações e alterações funcionais que prejudicam a qualidade de vida desses sujeitos. O presente estudo buscou avaliar e comparar a qualidade de vida de pacientes submetidos à fisioterapia com controles. Foram selecionados sessenta doentes renais crônicos de ambos os gêneros. Aplicou-se o questionário de qualidade de vida Kidney Disease Quality of Life Short Form para avaliar dois grupos de pacientes: um controle, sem tratamento fisioterapêutico, e um que realizava o tratamento fisioterapêutico. Os resultados demonstraram comprometimento relacionado à faixa etária, tempo de hemodiálise e gênero em grande parte das dimensões analisadas em ambos os grupos; porém, quando se fez a análise entre os grupos, pacientes que realizaram fisioterapia obtiveram melhores índices de qualidade de vida. Concluiu-se que a fisioterapia contribui para uma tendência de melhora geral da qualidade de vida de pacientes renais crônicos submetidos à hemodiálise.
INTRODUÇÃO: A doença renal crônica (DRC) é decorrente de uma lesão renal e perda progressiva e irreversível da função dos rins. A hemodiálise substitui parcialmente essa função, com o objetivo de corrigir as alterações metabólicas na DRC. Para acompanhar a adequação da diálise, é determinado o Kt/V - índice de depuração da ureia por sessão de hemodiálise. O exercício físico de moderada intensidade tem se mostrado de fundamental importância para melhorar os efeitos adversos ao tratamento dialítico. OBJETIVOS: Avaliar o Kt/V em indivíduos com DRC submetidos ao exercício físico isotônico de baixa intensidade durante a hemodiálise. MATERIAIS E MÉTODOS: Analisados dados de 15 voluntários de ambos os sexos, submetidos à hemodiálise três vezes por semana. Após duas horas do início da diálise, foi aplicado um protocolo de exercícios isotônicos de baixa intensidade de membros superiores e inferiores com duração de 30 minutos, por um período de três meses. Os valores do Kt/V foram comparados no período de três meses anteriores sem exercício e após três meses de exercício. RESULTADOS: A média dos valores do Kt/V nos três meses sem exercício foi de 1,13 ± 0,11 e após aplicação do programa de exercícios foi de 1,29 ± 0,12 (p < 0,05). CONCLUSÃO: O programa de exercício físico isotônico de baixa intensidade em pacientes com DRC, aplicados durante a sessão de diálise mostrou a melhora da eficiência dialítica.
Objetivos: Analisar e correlacionar o nível de atividade física (NAF) e a qualidade de vida (QV) de pacientes com doença renal crônica (DRC) submetidos à hemodiálise, e verificar, nos praticantes de atividades físicas (AF), se eles seguem normas e recomendações adequadas para sua prática. Método: A QV (questionário KDQOL-SFTM), o NAF (questionário IPAQ) e avaliação de normas e recomendações para prática de AF foram analisados em 101 pacientes. Resultados: Foi observado comprometimento nas diferentes dimensões da QV, com os maiores valores para função sexual e suporte social (88,6), e os menores, para papel profissional (25,2) e função física (47,7). A maioria dos pacientes apresentou baixos NAF, e a maior parte dos praticantes de AF executa exercícios incorretamente. Diferenças entre NAF e alguns domínios da QV foram observadas. Conclusões: Os pacientes com DRC submetidos à hemodiálise apresentaram baixos NAFs e comprometimento na QV, e a maioria dos que realizavam AF não seguiam normas e recomendações adequadas para sua prática.
