BackgroundVisceral leishmaniasis in Brazil is caused by the protozoan Leishmania (Leishmania) chagasi and it is transmitted by sandfly of the genus Lutzomyia. Dogs are an important domestic reservoir, and control of the transmission of visceral leishmaniasis (VL) to humans includes the elimination of infected dogs. However, though dogs are considered to be an important element in the transmission cycle of Leishmania, the identification of infected dogs representing an immediate risk for transmission has not been properly evaluated. Since it is not possible to treat infected dogs, they are sacrificed when a diagnosis of VL is established, a measure that is difficult to accomplish in highly endemic areas. In such areas, parameters that allow for easy identification of reservoirs that represents an immediate risk for transmission is of great importance for the control of VL transmission. In this study we aimed to identify clinical parameters, reinforced by pathological parameters that characterize dogs with potential to transmit the parasite to the vector.ResultsThe major clinical manifestations of visceral leishmaniasis in dogs from an endemic area were onicogriphosis, skin lesions, conjunctivitis, lymphadenopathy, and weight loss. The transmission potential of these dogs was assessed by xenodiagnosis using Lutzomyia longipalpis. Six of nine symptomatic dogs were infective to Lutzomyia longipalpis while none of the five asymptomatic dogs were infective to the sandfly. Leishmania amastigotes were present in the skin of all clinically symptomatic dogs, but absent in asymptomatic dogs. Higher parasite loads were observed in the ear and ungueal region, and lower in abdomen. The inflammatory infiltrate was more intense in the ears and ungueal regions of both symptomatic and asymptomatic dogs. In clinically affected dogs in which few or none Leishmania amastigotes were observed, the inflammatory infiltrate was constituted mainly of lymphocytes and macrophages. When many parasites were present, the infiltrate was also comprised of lymphocytes and macrophages, as well as a larger quantity of polymorphonuclear neutrophils (PMNs).ConclusionDogs that represent an immediate risk for transmission of Leishmania in endemic areas present clinical manifestations that include onicogriphosis, skin lesions, conjunctivitis, lymphadenopathy, and weight loss. Lymphadenopathy in particular was a positive clinical hallmark since it was closely related to the positive xenodiagnosis.
RESUMO.-As Leptospiroses são zoonoses graves de distribuição mundial que afetam o homem e outros animais. A infecção em animais, geralmente, é inaparente, ou os sintomas quando presentes são similares a outras infecções. Neste estudo foram colhidos soros de 119 ovinos e seus respectivos rins durante abate em feiras livres no município de Teresina-Piauí. Pela técnica de soroaglutinação microscópica (SAM) obtiveram-se 34 amostras sorológicas positivas para um ou mais sorovares de Leptospira spp., com taxa de ocorrência de 28,6% de anticorpos anti-leptospiras, sendo 23 casos de infecção para um único sorovar e 11 com coaglutinações para dois ou mais sorovares. Lepitospirosis is a serious worldwide distribution disease which affects man and other animals. The infection is generally asymptomatic in animals. In cases whose symptoms are present, symptoms are similar to other infections. In the present study serum samples from 119 sheep and their kidneys were collected during their slaughter in outdoor markets in the city of Teresina, Piauí, Brazil. The Microscopic Agglutination Test (MAT) obtained 34 positive serological samples for one or more Leptospira spp. serovar with occurrence rate of 28.6% of leptospiral antibodies. There were 23 cases of infection for a single serovar, and 11 cases with coagglutination for two or more serovars. Autumnalis had the highest occurrence (29.4%) among the pathogenic serovars. The histopathological analysis of 36 kidney fragments revealed tubulo-interstitial alterations in 33 (91.7%) positive animals. Tubular lesions were observed in 20 (55.5%) positive animals. The Warthin Starry staning revealed the presence of Leptospira in 8 (22.20%) of the 36 positive samples. The immunoperoxidase staining revealed the presence of Leptospira in 12 (60%) of 20 positive samples. The inflammatory infiltrate in the positive animals was significantly more evident in the cortical-medullar and cortical regions than in the medullar region (p=0.000), however, there was no difference between positive and negative animals. The presence of hyaline casts in the proximal tubules was significantly higher in positive animals compared to the negative ones (p=0.0001). Discrete lesion was observed in glomeruli. In conclusion, the results from this study showed that sheep which are positive for Leptospira present tubulo-intersticial renal lesions with the presence of Leptospira in the tubules, conferring to these animals the condition of asymptomatic carriers.
This study was conducted to determine leptospirosis seroprevalence in sheep and their spatial distribution as well as identify risk factors associated with seropositivity in sheep from 37 herds and 11 municipalities in the Presidente Dutra microregion, Maranhão state, Brazil. We analyzed 379 blood serum samples using a Microscopic Agglutination Test (MAT). The individual seroprevalence was 32%. Of the 37 herds studied, 30 (81%, 95% CI 69-94%) had at least one seropositive animal. In seven municipalities, we observed infection in 100% of the herds. The serovars recorded were Grippotyphosa (67%), Wollfi with Hardjo (9%), Bratislava (9%), Hardjo (5%), Icterohaemorrhagiae (5%), Pomona (2%), Castellonis (2%) and Copenhageni (0.8%). We concluded that the Leptospira spp. in sheep is widespread in the area of sheep farms in Maranhão state, and a risk factor is the animals' water source.
The presence of anti leptospiral agglutinins (microscopic agglutination test -MAT) and DNA of leptospires was investigated in the kidney and urine (Polymerase Chain Reaction -PCR) in samples collected at the time of slaughter of cattle originating from the dairy basin of Parnaíba, Piauí, Brazil, as also the lesions in kidney, lung, liver, uterus, ovary and placenta (histopathology and immunohistochemistry). In the MAT, Hardjo was the predominant serovar with the highest number of reagent animals for the strain Hardjobovis/Sponselee. Anti-leptospiral antigens were scored in epithelial cells, interstitial vascular endothelium, endothelium of glomerular capillaries and Bowman's capsule of 20 positive animals. Inϐlammatory cells were more common in the kidney. PCR was positive in urine and kidney tissue. RESUMO.-[Sorologia, reação em cadeia da polimerase e histopatologia para leptospirose em amostras coletadas de vacas leiteiras, em matadouro da região de Parnaíba, Piauí.] Foi investigada a presença de aglutininas anti leptospiras (reação de soroaglutinação microscópica -SAM), de DNA de leptospiras no rim e na urina (reação de cadeia pela polimerase -PCR), bem como de lesões no rim, pulmão, ϐígado, útero, ovário e placenta (histopatologia e imunohistoquímica), em materiais colhidos, por ocasião do abate, de bovinos originários da bacia leiteira da região de Parnaíba, Piauí, Brasil. Na SAM o sorovar predominante foi o Hardjo com maior número de animais reagentes para a estirpe Hardjobovis/Sponselee. Antígenos anti leptospira foram marcados em 20 animais positivos nas células epiteliais e do endotélio vascular, endotélio dos capilares glomerulares e na cápsula de Bowman, somente nos animais infectados. O inϐiltrado inϐlamatório foi maior no rim do que nos demais órgãos. A PCR foi positiva em amostras de urina e tecido renal. TERMOS DE INDEXAÇÃO: Leptospirose, imuno-histoquímica, PCR, bovino.
Soro e rim de 75 suínos mestiços sem raça definida, criados em sistema extensivo e abatidos em Timon (MA) e Teresina (Piauí), Brasil, duas cidades separadas pelo Rio Parnaíba e 75 suínos mestiços de um sistema de criação em confinamento, filhos de Landrace, Large White e/ou Duroc foram utilizados neste estudo. Das 150 amostras analisadas pela prova de Soroaglutinação Microscópica (SAM), sete foram reagentes e o sorovar Icterohaemorrhagiae (42,86%) foi o mais frequente. Uma comparação entre os dois sistemas para examinar uma predisposição para infecção para Leptospira spp. mostrou que a suscetibilidade foi maior nos animais criados extensivamente do que naqueles criados em confinamento (teste χ2, p<0,05). A presença de infiltrado inflamatório foi significantemente maior nos animais soropositivos comparados aos soronegativos (p<0,05, Teste U de Mann-Whitney). A análise morfométrica mostrou Leptospira spp. e o antígeno de leptospira apenas nos animais soropositivos (p<0,05, teste de U de Mann-Whitney). Apoptose em células epiteliais tubulares foi significantemente mais evidente nos animais infectados comparados aos não infectados (p<0,05, Teste U de Mann-Whitney). Uma eventual associação de antígeno de Leptospira e células epiteliais em apoptose sugere um provável mecanismo de lesão renal na leptospirose suína.
