Complete and continuous documentation in patient records is an important condition for adequate communication with patients, between the professions concerned and to ensure the quality of the following working steps in care provision. Part of a German research project concerning the interprofessional communication in hospital was therefore to analyse the use of the documentation system. 54 users were asked about practical aspects of their documentation system and 450 patient records were evaluated. The analysis focused on the medical and nursing documentation of admission, process and discharge. Deficits that need to be improved appeared first of all in the practical aspects of the documentation system, the flow of information between the professions, in specific gaps of medical and nursing admission, documentation of process and discharge. Quality management is asked to improve and develop the documentation in collaboration with the users and to consider specific problems when introducing computer based records.
Zusammenfassung: Auch im Bereich der Altenpflege kommt der Qualitätssicherung eine immer stärkere Bedeutung zu. Dabei ist die Zufriedenheit der Bewohner/innen mit den Bedingungen der Einrichtung ein allgemein anerkannter Aspekt der Qualität in der Versorgung. Es wird ein Untersuchungsverfahren vorgestellt, das Bewohnerzufriedenheit in Pflegeeinrichtungen erfasst. Dabei wurde ein multiperspektivischer Ansatz verfolgt, indem außer der Befragung der Bewohner/innen auch die der Angehörigen und Betreuer/innen vorgesehen ist. Das Hamburger Verfahren zur Zufriedenheit mit Pflegeeinrichtungen (HVZP) ist in zwei Durchgängen erprobt und optimiert worden und anschließend in 15 Pflegeeinrichtungen zum Einsatz gekommen, wobei N = 1009 Bewohner/innen und N = 633 Angehörige und Betreuer/innen befragt worden sind. Die testtheoretische Überprüfung des HVZP bestätigt für beide Varianten des Instruments gute Reliabilität für die gebildeten Skalen. Die Instrumente sind in der Lage, die Stärken und Schwächen einzelner Einrichtung zu identifizieren und zwischen Einrichtungen zu differenzieren. Auch Praktikabilität und Verständlichkeit der Instrumente sind zufrieden stellend.
Zusammenfassung. Innerhalb von Organisationen und Betrieben wird durch den Einsatz von Arbeits- und Projektgruppen ein “Gruppenvorteil“ in Form besserer Ergebnisse, schnellerer Entscheidungsfindung und breiterer Zustimmung erwartet. Häufig bedarf es jedoch eines derart hohen Aufwandes an Koordinations- und Integrationsleistungen, um den vielfältigen Argumenten, Einstellungen und Informationen gerecht zu werden, dass diese Erwartungen sich nicht erfüllen. Der Einsatz von Moderationstechniken soll diese Koordinations- und Integrationsleistungen erleichtern und gleichzeitig sozio-emotionale Prozesse steuern. Moderationstechniken, die theoriegeleitet die wesentlichen kognitions- und sozialpsychologischen Leistungskriterien berücksichtigen, liegen in Form der am Arbeitsbereich Sozialpsychologie der Universität Hamburg entwickelten Formalen Moderation (FORMOD) und Prozeduralen Moderation (PROMOD) vor. Experimentell konnte im Rahmen eines DFG-Projektes gezeigt werden, dass die wissensbasierte Technik PROMOD der ebenfalls untersuchten FORMOD hinsichtlich der Leistung beim komplexen Problemlösen signifikant mit mittlerer Effektstärke überlegen ist und dass sich der Einsatz beider Moderationstechniken als leistungssteigernd im Vergleich zu nicht-moderierten Gruppen erweist. Neben Leistungsparametern wurden die individuellen Problemlösestrategien und die Konformitätsprozesse als abhängige Variablen zur Vergleichsmessung herangezogen. Die experimentell erzielten Ergebnisse bezüglich der Gruppenleistung und die Darstellung der in FORMOD und PROMOD integrierten Leistungsparameter untermauern die Forderung nach einer theoriegeleiteten Entwicklung integrativer und komplexitätsangemessener Methoden zur Leistungsverbesserung in Arbeitsgruppen. Erste Praxistransfers der Methoden in die betriebliche Realität werden dargestellt und diskutiert.
