Social change and the subsequent increase in the complexity of patient care create new challenges for all health care professionals that can only be met by close collaboration. Good cooperation between professionals from all healthcare backgrounds has proven to be essential and should therefore play a larger role in educational and study contexts.It is for this reason that over the past few years more and more countries are requiring cooperative skills and competencies in formal medical training and consequently implementing this training in their healthcare systems. This is certainly true in countries such as Sweden, Canada, England, and most recently Switzerland, where interprofessional teaching is being systematically introduced into medical training.The current situation in Germany is demonstrated by the selected projects from the Robert Bosch Foundation's support program "Operation Team." The existing need for interprofessional qualification opportunities and advanced training programs for teaching staff, the need for exam content for medical state examinations, as well as to establish a structural framework and research structure in the area of interprofessional education and competence development are outlined and discussed.
Complete and continuous documentation in patient records is an important condition for adequate communication with patients, between the professions concerned and to ensure the quality of the following working steps in care provision. Part of a German research project concerning the interprofessional communication in hospital was therefore to analyse the use of the documentation system. 54 users were asked about practical aspects of their documentation system and 450 patient records were evaluated. The analysis focused on the medical and nursing documentation of admission, process and discharge. Deficits that need to be improved appeared first of all in the practical aspects of the documentation system, the flow of information between the professions, in specific gaps of medical and nursing admission, documentation of process and discharge. Quality management is asked to improve and develop the documentation in collaboration with the users and to consider specific problems when introducing computer based records.
A telehealth system that can transmit vital signs data, and also empowers patients to maintain their autonomy and learn how to manage their disease better, can improve patients' quality of life. Such a system can form part of a high-quality and efficient health-care service. The Bosch telehealth system, which focuses on self-management, has been used by thousands of patients with long-term conditions in the USA and Europe. The efficiency of the programmes, as well as their positive effects on quality of life and compliance, have been proven for a number of chronic conditions, as well as for patients with multiple co-morbidities.
