Objective: To reflect on the main characteristics and recommendations of Incident Reporting Systems, discuss the population’s participation in reporting, and point out challenges in the Brazilian system. Method: Reflection study, based on Ordinance No. 529/13, which instituted the National Patient Safety Program, under Collegiate Board Resolution (CBR) No. 36/13; reflections by experts were added. Results: Reporting systems are a source for learning and monitoring, allow early detection of incidents, investigations and, mainly, the generation of recommendations prior to recurrences, in addition to raising information for patients and relatives. There is little participation of the population in the reporting, regardless of the type of system and characteristics such as confidentiality, anonymity, and mandatory nature. Final Considerations: In Brazil, although reporting is mandatory, there is an urgency to advance the involvement and participation of the population, professionals, and institutions. To simplify data entry by improving the interface and importing data from the reporting system is an objective to be achieved.
ObjectiveWe aimed to develop and validate a new instrument called Questionnaire for the assessment of the knowledge, management and reporting ADR in Paediatrics by Healthcare teams (QUESA-P).DesignThis is a cross-sectional study.Settings and participantsTeams of healthcare professionals (HCP) that lead with pharmacological therapy in Paediatrician’s sector (Paediatric-HCP) in seven public hospitals in Brazil.OutcomeAn assessment of the knowledge and current management of ADR in Paediatric-HCP.MethodsWe developed and validated QUESA-P, using a standardised procedure which included item development and psychometric prevalidation using Cronbach’s Alpha, item-total correlation and test–retest validity for internal consistency and reliability. External criterion was used as criterion validation (the instrument was applied to the focus group expert vs focus group team of Paediatric-HCP in hospitals). The focus group of experts who participated in psychometrics was asked to respond to the QUESA-P twice in order to assess test-retest reliability. The content validity of the initial questionnaire was assessed by the Delphi method and pilot test. Subsequently, we made minor revisions and finalized the QUESA-PResultsSelection of domains and facets were based on literature review made in duplicate by authors. Content validity was done by trial of different examiners (panellists, n=16), conducting analysis through Delphi method (three rounds). The QUESA-P was constructed with three domains. The intraclass correlations (0.80) and the Cronbach’s alpha coefficient (0.82), indicated adequate test–retest reliability and internal consistency for each domain. The application of the QUESA to 61 Paediatric-HCP in hospital resulted in lower mean score of 42.1 ± 3.4 in all domains when compared with expert teams (n= 46) 48.2 ± 3.7 (p <0.001) indicating that the instrument is valid to discriminate QUESA experts and Paediatric-HCP.ConclusionThe selected domains can be used to check weaknesses in the identification, management and reporting of suspected ADR by Paediatric-HCP in Brazil.
A cesariana é uma intervenção obstétrica que visa salvar a vida das mulheres e seus recém-nascidos, e quando realizada sob uma indicação medica, é uma cirurgia segura e essencial, mas quando realizada sem qualquer indicação medica, não há evidencias que tragam benéficos e ainda alguns estudos relatam que trazem prejuízos para a saúde do recém-nascido e da criança. As taxas de cesárea têm crescido bruscamente em todo o mundo, sendo o Brasil um dos piores cenários, portanto o objetivo desse estudo foi identificar os prejuízos que a cesariana sem indicação pode trazer ao desenvolvimento e saúde do recém-nascido e da criança. Trata-se de uma revisão de escopo onde os artigos foram selecionados nas bases de dados LILACS e Medline. Como resultado a cesariana apresentou maior risco de atrasos no desenvolvimento e neurodesenvolvimento, já o desenvolvimento de morbidades e eventos adversos infantis foram mais prevalentes em crianças nascidas por cesárea em comparação às nascidas de parto normal. A colonização da microbiota intestinal também foi diferente quando comparado as duas vias de parto, encontra-se nas crianças nascidas por cesariana uma menor diversidade microbiana ao longo da infância, que está relacionada ao risco aumentado de desenvolvimento de doenças do sistema imunológico. Desta maneira conclui-se que precisamos mensurar o uso dessa via de parto sem indicação, pois trás consequências a saúde do recém-nascido e da criança.
O objetivo é investigar a adesão das profissionais do sexo aos serviços de saúde da assistência primária no Brasil. Trata-se de uma revisão integrativa da literatura. Para a execução da busca dos artigos utilizou-se os seguintes descritores: prostituição, profissionais do sexo, acesso aos serviços de saúde e assistência à saúde, nas bases de dados: Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), Medical Literature Analysis and Retrieval System Online (MEDLINE) e U.S. National Library of Medicine (PUBMED). Os critérios de inclusão adotados foram: ano de publicação, artigos em língua portuguesa, inglesa ou espanhola e produções científicas publicadas no período de 2010 a 2019. Foram excluídos os estudos que não abordassem a temática relacionada ao objetivo da revisão. Com base no método utilizado, foram selecionados sete artigos. Os estudos expõem que as profissionais do sexo encontram dificuldades constantemente quando procuram por atendimento nos serviços públicos de atenção primária à saúde. As causas mais citadas que dificultam o acesso estão relacionadas à forma predominante a barreiras sociais e institucionais. Compreende-se que as profissionais do sexo apresentam baixa adesão aos serviços de saúde da atenção primária no Brasil. A presença de barreiras dificulta o acesso e proporciona o distanciamento dessas mulheres dos serviços público de saúde, reduzindo a adesão ao sistema de atenção primária.
