ÖZET Amaç: Bu çalışmada DSM-5 tanı ölçütlerine göre yeniden düzenlenen Okul Çağı Çocukları için Duygulanım Bozuklukları ve Şizofreni Görüşme Çizelgesi (6-18 Yaş) -Şimdi ve Yaşam Boyu Şekli-DSM-5 Kasım 2016-Türkçe Uyarlaması'nın (ÇDŞG-ŞY-DSM-5-T) geçerlik ve güvenirliğinin değerlendirilmesi amaçlanmıştır. Yöntem: Yaşları 6-17 arasındaki, 150 çocuk ve ergene ÇDŞG-ŞY-DSM-5-T uygulanmıştır. ÇDŞG-ŞY-DSM-5-T ile konulan tanıların geçerliği, klinik değerlendirme ile konulan DSM-5 tanıları (uyum geçerliği) ve o tanıyı değerlendiren ölçek puanları (eş zaman geçerliği) dikkate alınarak değerlendirilmiştir. Değerlendiriciler arası güvenirlik seçkisiz yöntemle belirlenen 20 katılımcıda incelenmiştir. Ayrıca aynı yöntemle seçilen 20 farklı katılımcıyla yapılan ilk değerlendirmeden üç hafta sonra ÇDŞG-ŞY-DSM-5-T uygulanarak test-tekrar test güvenirliği araştırılmıştır.Bulgular: ÇDŞG-ŞY-DSM-5-T ile yapılan görüşme ile konulan tanıların uyumunun yeme bozuklukları, seçici konuşmazlık ve otizm spektrumu bozuklukları açısından çok iyi (κ=0,92-1,0), dışa atım bozuklukları, obsesif kompulsif bozukluk, karşıt olma/karşı gelme bozukluğu, yaygın anksiyete bozukluğu, sosyal anksiyete bozukluğu, depresif bozukluklar, yıkıcı duygudurum düzensizliği bozukluğu ve dikkat eksikliği hiperaktivite bozukluğu açısından iyi (κ=0,67-0,80) düzeyde olduğu gözlenmiştir. Değerlendiriciler arası güvenirliğin seçici konuşmazlık için çok iyi (κ=1,0), karşıt olma/karşı gelme bozukluğu, yıkıcı duygudurum düzensizliği bozukluğu, dikkat eksikliği hiperaktivite bozukluğu ve depresif bozukluklar için iyi derecede (κ=0,63-0,73) olduğu; test-tekrar test güvenirliğinin de otizm spektrumu bozuklukları için çok iyi derecede (κ=0,82), dikkat eksikliği hiperaktivite bozukluğu, karşıt olma/karşı gelme bozukluğu, depresif bozukluklar ve yaygın anksiyete bozukluğu ve için iyi derecede (κ=0,62-0,78) olduğu görülmüştür.Sonuç: Bu çalışmanın sonuçları ÇDŞG-ŞY-DSM-5-T'nin görüşme çizelgesine yeni eklenen seçici konuşmazlık, yıkıcı duygudurum düzensizliği bozukluğu ve otizm spektrumu bozuklukları dâhil birçok tanı grubu için geçerli ve güvenilir veri sağladığını düşündürmüştür. Method:A total of 150 children and adolescents between 6 and 17 years of age were assessed with K-SADS-PL-DSM-5-T. The degree of agreement between the DSM-5 criteria diagnoses and the K-SADS-PL-DSM-5-T diagnoses were considered as the measure of consensus validity. In addition, concurrent validity was examined by analyzing the correlation between the diagnoses on K-SADS-PL-DSM-5-T and relevant scales. Interrater reliabilities were assessed on randomly selected 20 participants. Likewise, randomly selected 20 other participants were interviewed with K-SADS-PL-DSM-5-T three weeks after the first interview to evaluate test-retest reliability. ÖZET:Trabzon ili ilkö¤retim ça¤› çocuklar›nda dikkat eksikli¤i hiperaktivite bozuklu¤u ve efllik eden y›k›c› davran›fl bozukluklar› yayg›nl›¤› Amaç: Bu çal›flman›n amac› Trabzon ilinde ilkö¤retim ça¤› çocuklar›nda Dikkat Eksikli¤i Hiperaktivite Bozuklu¤u'nun (DEHB) ve ona efllik eden Y›k›c› Davran›fl Bozukluklar›'n›n (YDB) yayg›nl›¤›n› incelemektir. Yöntem: Çal›flmada 6-12 yafl aral›¤›ndaki ilkokul ö¤rencilerinde (n=1126) DSM-IV tan› ölçütlerine göre DEHB ve YDB tan›lar›n›n konulmas› amac› ile Turgay'›n Çocuk ve Ergenlerde Davran›m Bozukluklar› için DSM-IV'e Dayal› Tarama Ölçe¤i kullan›lm›flt›r. Bulgular: DEHB yayg›nl›¤› %8.6 (n=97) ve alt tip da¤›l›mla-r› dikkat eksikli¤i %1.6 (n=18), hiperaktif/ dürtüsel tip %6 (n=69) ve kombine %0.9 (n=10) olarak s›ralanm›flt›r. Karfl›t Gelme-Karfl› Olma Bozuklu¤u'nun (KG-KOB) yayg›nl›¤› %14.1 (n=159) ve Davran›m Bozuklu-¤u'nunki (DB) %4.4 (n=50) olarak belirlenmifltir. DEHB olan olgularda KG-KOB'nin s›kl›¤› %57.5 (n=56)ve DB'nin s›kl›¤› %20.6 (n=20)'d›r. Sonuç: Bu çal›flman›n sonuçlar› DEHB yayg›nl›¤›n›n, erkek bask›nl›¤›n›n ve efllik eden YDB s›kl›¤›n›n kültürler aras›nda anlaml› bir farkl›l›k göstermedi¤i görüflünü desteklemektedir. Öte yandan hiperaktif/ dürtüsel alt tipin da¤›l›m› literatürde sunulan oranlardan farkl›l›k göstermektedir. Bu fark›n Türk toplumunda çocu¤a yönelik yafla ba¤l› tutumlar, yak-lafl›m örüntüleri ve beklentilerle iliflkili kültürel farklarla fle-killlenebilece¤i de¤erlendirilmifltir. ABSTRACT: Prevalence of attention deficit hyperactivity disorder and comorbid disruptive behavior disorders among school age children in TrabzonObjective: The aim of this study is to determine the prevalence of Attention Deficit Hyperactivity Disorder (ADHD) and co-morbid Disruptive Behavior Disorders (DBDs) in a sample of primary school children in Trabzon, Turkey. Method: Primary school children aged 6-12 years (n=1126) were assessed for the DSM-IV criteria of ADHD and co-morbid DBDs using Turgay's DSM-IV based ADHD and disruptive behavior disorders screening scale. Results: The prevalence of ADHD was 8.6% (n=97) and the subtypes were predominantly inattentive 1.6% (n=18), predominantly hyperactive/impulsive 6.1% (n=69) and combined 0.9% (n=10) respectively. The male to female ratio was 3.5/1 for ADHD (all subtypes). The prevalence of other DBDs in the study sample was 18.6% (n=209). The cases were distributed as Oppositional Defiant Disorder (ODD) 14.1% (n=159) and Conduct Disorder (CD) 4.4% (n=50) respectively. The frequency of ODD among ADHD cases was 57.5% (n=56) and that of CD was 20.6% (n=20). Conclusions: It was supported that ADHD did not show significant differences among cultures regarding its prevalence, male predominance and high DBD co-morbidity. ObjectiveThe aim of this study is to examine performance-based measures and behavioral ratings of executive functions (EF) as a component of preschool attention deficit hyperactivity disorder (ADHD).MethodsTwenty-one 4-to-6-year-old children with ADHD and 52 children with no psychopathology, matched on age, gender, socioeconomic status, and parental education, were enrolled. Parents were interviewed with the use of The Schedule for Affective Disorders and Schizophrenia for School-Age Children Present and Lifetime version. The Conners' Kiddie Continuous Performance Test (K-CPT) was administered to the children, and the Behavior Rating Inventory of Executive Function-Preschool version (BRIEF-P) and the Conners' Parent Rating Scale-Revised/Short Form (CPRS-R/S) were filled out by the parents.ResultsAll BRIEF-P and CPRS-R/S scores, the K-CPT measures of inattention and impulsivity were higher in the ADHD group. The CPRS-R/S ADHD index was strongly correlated with inhibition and related indexes in the BRIEF-P and was moderately correlated with inattention measures in the K-CPT.ConclusionThe current study is one of the few to investigate the features of preschool ADHD with the use of behavioral ratings of EF and a performance-based measure. Our results suggest that the BRIEF-P was able to identify behavioral difficulties in inhibition and working memory and that the K-CPT identified difficulties indicating inattention. The findings of this study support the use of a combination of methods for a complete evaluation of preschoolers with inattentive and hyperactive/impulsive behavior, the application of rating scales for screening ADHD symptoms, and the measurement of behavioral correlates of EF, along with performance-based measures.
The presented findings are unique in terms of their ability to clinically and physiologically differentiate between ADHD children with and without ED.
