We present two cases of acute myocardial infarction in young patients with asymptomatic COVID‐19 infection and ST‐elevation myocardial infarction (STEMI), complicated by severe acute heart failure and ventricular fibrillation, resulting cardiopulmonary resuscitation and mechanical ventilatory support. Urgent primary percutaneous coronary intervention with further complex treatment was effective in both cases with critical cardiovascular state and co‐morbid COVID‐19 infection. This report illustrates the challenges in clinical severity of STEMI with COVID‐19 infection, despite of young age and absence of clinical symptoms and chronic co‐morbidities. STEMI patients with even asymptomatic COVID‐19 infection may be presented with significantly higher rates of severe acute heart failure.
Аннотация. Трансплантация является оптимальным способом лечения пациентов с хронической болезнью почек. Пациенты с почечной недостаточностью имеют высокий кардиоваскулярный профиль риска. Цель: изучить параметры липидограммы до и в различные сроки после трансплантации почки и выявить наиболее значимые факторы, ассоциированные с дислипидемией. Материалы и методы: проведено ретроспективное исследование показателей липидного спектра: общий холестерин (ОХ) и триглицериды у 200 реципиентов почки в сроки до трансплантации почки (ТП) и через 6, 12 месяцев после операции. Результаты: отмечено снижение уровней общего холестерина и триглицеридов к концу 1 года после операции в сравнении с дострансплантационными (p<0,05). Выявлена положительная корреляционная связь между уровнем общего холестерина, триглицеридов и возрастом, индексом массы тела (p<0,05). Наибольшее влияние на параметры дислипидемии оказывает функциональное состояние почки -выявлена сильная отрицательная корреляционная связь между уровнями общего холесте-рина и триглицеридов и скоростью клубочковой фильтрации (p<0,05). Выводы: успешная трансплантация почки приводит с регрессу дислипидемии, а следовательно, и сердечно-сосудистых рис-ков. Наибольшее влияние на показатели липидного спектра оказывает фильтрационная способность почки. Пациенты с ХБП, особенно в терминальной стадии, находящиеся на за-местительной почечной терапии методом программ-ного гемодиализа (ПГД) или постоянного амбулатор-ного перитонеального диализа (ПАПД), а также реци-пиенты ренального аллотрансплантата (РАТ) имеют повышенный риск развития сердечно-сосудистых за-болеваний [1]. Для этой категории больных вероят-ность погибнуть от сердечно-сосудистого заболевания гораздо выше, чем достигнуть терминальной стадии почечной недостаточности [2]. При этом помимо тра-дицинонных факторов риска развития неблагоприят-ных сердечно-сосудистых событий, таких как возраст, пол, наличие сахарного диабета, ожирение, гиперто-ния, дислипидемия, больные с ХБП имеют такие спе-цифические, как анемия, гипергомоцистеинемия, нарушения минерального обмена и гиперпаратиреои-дизм, оксидативный стресс, гипоальбуминемия, хро-ническое воспаление. В общей популяции одной из наиболее значимых детерминант развития сердечно-сосудистых событий является нарушение метаболизма липидов, и больные с ХБП не исключение, кроме того, почечная недостаточность ассоциирована со специфи-ческими количественными и качественными аномали-ями в метаболизме липидов [3].Дислипидемия развивается уже на ранних стадиях почечной недостаточности, и связана, преимуще-ственно, с нарушением активности ключевых фермен-тов печени, влияющих на метаболизм липидов. Arterial hypertension is an established risk factor for cardiovascular complications. Since the introduction of calcineurin inhibitors (CNI), аrterial hypertension is observed in 50-80% of transplanted patients. Arterial hypertension and transplantation are closely linked, and its association may promote impaired graft and overall survival. We summarize the main mechanisms involved in the genesis of CNI-induced hypertension as follows: impaired renal function with the sodium retaining, endothelial dysfunction, renin-angiotensin-aldosterone system and sympathetic activation, production of vasoconstrictor substances, activation the renal Na+/Cl--cotransporter. Attention is focused on the dose-dependent increase in arterial pressure and the need for further study of the intensity of the pro-hypertensive effect of various calcineurin inhibitors.
