Descritores Resumo ObjetivoA infecção do sítio cirúrgico constitui um grave problema dentre as infecções hospitalares por sua incidência, morbidade e mortalidade. Devido ao curto período de internação, a maioria dessas infecções se manifesta após a alta hospitalar, sendo subnotificada quando não há o seguimento do paciente cirúrgico. Programas de vigilância específicos do paciente após a alta são considerados fundamentais para controlar as infecções. O objetivo do estudo foi determinar a incidência de infecção do sítio cirúrgico e comparar sua freqüência durante a internação e após a alta. Métodos Realizou-se um estudo epidemiológico, tipo coorte, em um hospital universitário, com acompanhamento durante o período de internação e no seguimento pós-alta hospitalar, de 504 pacientes que se submeteram à cirurgia do aparelho digestivo no primeiro semestre de 2000. Resultados Das 504 cirurgias realizadas no período do estudo, 398 (79,0%) dos pacientes retornaram ao ambulatório de egressos. Do total de infecções do sítio cirúrgico diagnosticadas, a maioria (62,9%) foi notificada no ambulatório de egressos, sendo 88,0% superficiais, e 67,0% notificadas até o sétimo dia após a alta. Conclusões Comprovou-se a importância do seguimento pós-alta para a obtenção de dados fidedignos sobre as infecções do sítio cirúrgico devido à manifestação tardia na maioria dos casos, levando a subnotificação quando o seguimento do paciente é realizado somente durante a internação. AbstractObjective Surgical wound infection (SWI) is a serious hospital infection due to its incidence, morbidity and mortality. Because of the short hospital stay, the majority of SWI appears post-discharge, and its incidence is often underreported when there is no follow-up of the surgical patient post discharge. The need of a surveillance system for surgical patient after discharge is critical for infection control. The study objective was to determine the incidence of SWI in surgical patients and to compare the frequency of SWI diagnosed in-hospital and after discharge.
RESUMO Objetivo Avaliar os Programas de Controle de Infecção em hospitais do Paraná, considerando como hipótese desempenho geral mínimo de 75%. Método Estudo transversal de avaliação processual, por meio de instrumento previamente validado, composto por quatro indicadores que avaliam a estrutura técnico-operacional (PCET), as diretrizes operacionais (PCDO), o sistema de vigilância epidemiológica (PCVE) e as atividades de controle e prevenção (PCCP). O estudo foi realizado de 2013 a 2014 em 50 hospitais por amostra de acesso. Resultados A conformidade geral obtida foi 71,0% (23,88dp), sendo indicador PCET 79,4% (18,9dp); PCVE 76,0% (30,5dp); PCDO 65,5% (26,9dp); e PCCP 63,2%/ (39,5dp). Houve significância estatística para melhor desempenho dos PCIRAS a realização de auditorias internas (p=0,0099), certificação de qualidade (p=0,01949), enfermeiro exclusivo (p<0,0001), profissionais médicos contratados ou concursados (p=0,0005), maior carga horária de dedicação exclusiva dos médicos, 4 horas (p=0,001), maior tempo de experiência de médicos (p=0,0028) e enfermeiros (p=0,0094). Conclusão A conformidade geral desses programas não alcançou a hipótese inicialmente formulada, devido aos indicadores PCDO e PCCP. Desse modo, é possível considerar que os programas apresentavam adequação mínima para sua operacionalização e para realizar a vigilância epidemiológica de IRAS, mas estavam prejudicados quanto à insuficiência quantitativa e qualitativa de diretrizes operacionais (PCDO) e de ações para o controle e prevenção dessas infecções (PCCP).
RESUMENSe trata de un estudio sobre el trabajo del enfermero en el centro de material y esterilización (CME), con la finalidad de analizar su realidad social y su papel transformador en el cuidar en salud por la enfermería. Investigación estratégica, con análisis de discurso de enfermeras de CME. Se obtuvo como resultado que la gerencia es su principal actividad, en un proceso estructurado que confirma la práctica tradicional de la enfermería, teniendo como objeto la coordinación del procesamiento de materiales médico-hospitalarios utilizados en actos cuidadores. Tal práctica se caracteriza por el cuidado indirecto, que instrumentaliza no apenas el trabajo de la enfermería, sino también la de los otros profesionales. En consecuencia, no se caracteriza como cuidado especí-fico de la enfermería y del enfermero. Su potencial transformador en las relaciones y nuevos modos de producción del cuidar en salud y por el enfermero deberá ocurrir antes por la competencia del conocimiento específico en el CME que por el administrativo. It's a strategic research, with speech analysis of CSU nurses. Results show that their main activity is management, in a structured process that confirms the traditional nursing practice, having as object the coordination of medical-hospital material processing used for care. Their work is characterized by indirect care, and is instrumental not only for nurses' work, but for the work of other professionals as well. OBJECTIVE:To validate the construct and discriminant properties of a hospital infection prevention and control program. METHODS:The program consists of four indicators: technical-operational structure; operational prevention and control guidelines; epidemiological surveillance system; and prevention and control activities. These indicators, with previously validated content, were applied to 50 healthcare institutions in the city of São Paulo, Southeastern Brazil, in 2009. Descriptive statistics were used to characterize the hospitals and indicator scores, and Cronbach's α coeffi cient was used to evaluate the internal consistency. The discriminant validity was analyzed by comparing indicator scores between groups of hospitals: with versus without quality certifi cation. The construct validity analysis was based on exploratory factor analysis with a tetrachoric correlation matrix. RESULTS:The indicators for the technical-operational structure and epidemiological surveillance presented almost 100% conformity in the whole sample. The indicators for the operational prevention and control guidelines and the prevention and control activities presented internal consistency ranging from 0.67 to 0.80. The discriminant validity of these indicators indicated higher and statistically signifi cant mean conformity scores among the group of institutions with healthcare certifi cation or accreditation processes. In the construct validation, two dimensions were identifi ed for the operational prevention and control guidelines: recommendations for preventing hospital infection and recommendations for standardizing prophylaxis procedures, with good correlation between the analysis units that formed the guidelines. The same was found for the prevention and control activities: interfaces with treatment units and support units were identifi ed. CONCLUSIONS:Validation of the measurement properties of the hospital infection prevention and control program indicators made it possible to develop a tool for evaluating these programs in an ethical and scientifi c manner in order to obtain a quality diagnosis in this fi eld.

The objective of this article was to update knowledge on the microbiological aspects of the oral cavity and, verify the relation of oral hygiene with the prevention of pneumonia associated with mechanical ventilation. The studies analyzed were in favor of the use of antiseptics for decontaminating the oral cavity and pharynx, although there is still no standardization of procedures on the technique and products used in this process. This article is a theoretical-reflexive study that aims to discuss the development and changes in the quality of healthcare assistance to the patient over the years in Brazil, in light of the policies of control and prevention of Healthcare-associated Infection (HAI). Aspects of HAI and the process of change in health policy in Brazil, as well as the quality of assistance associated with its control, are approached in relation to policies of patient safety. There are various new theoretical and practical proposals created in Brazil. In spite of the difficulty of measuring patient safety, directed to the prevention and control of HAI, we emphasize that to only create policies and establish norms, guidelines, and indicators is not sufficient. If no structural support or conditions exist for interventions in the practices of healthcare professionals, aiming at results in acceptable levels, the control of HAI will not be achieved.
