AbstrakPendahuluan: Otitis Media Supuratif Kronis (OMSK) tipe kolesteatom merupakan penyakit dengan morbiditas dan mortalitas yang tinggi akibat komplikasinya. Kolesteatom dapat menyebabkan erosi tulang dan kerusakan struktur-struktur di sekitarnya sehingga terjadi komplikasi. Kombinasi antibiotik dan tindakan bedah timpanomastoidektomi menjadi modalitas utama penatalaksanaan kasus OMSK dengan komplikasi. Laporan Kasus: Dilaporkan satu kasus seorang laki-laki 18 tahun dengan keluhan sakit kepala hebat disertai dengan penurunan kesadaran dan wajah mencong. Pada pasien terdapat riwayat telinga berair dan penurunan pendengaran. Pasien didiagnosis sebagai OMSK auris dektra dengan kolesteatom disertai komplikasi meningitis dan paresis nervus fasialis perifer. Pasien diterapi dengan antibioik dosis tinggi dan dilakukan tindakan timpanomastoidektomi dinding runtuh dengan dekompresi nervus fasialis. Kesimpulan: Penatalaksaan segera dan tepat pada OMSK dengan komplikasi dapat meningkatkan angka kesembuhan dan mencegah kematian. Abstract Introduction: Chronic suppurative otitis media (CSOM) with cholesteatoma type is a disease with high morbidity and mortality due to its complications. Cholesteatoma cause bone erosion and damage to the surrounding structures and resulting in complications. The combination of antibiotics and tympanomastoidectomy surgery becomes main modality for management of ofCSOM cases with complications. Case Report:A case of an 18-year-old man with severe headache accompanied by a decrease of conciousness and a skew face. The patient has history of discharge came out from the ear and decrease of hearing. Patient was diagnoseed as CSOM at right ear with cholesteatoma aaccompanied by meningitis and paralysis of facial nerve. Patient was treated with high-dose antibiotic and performed a canal wall down tympanomastoidectomy with decompression of facial nerve. Conclusions: Immediate and appropriate management of CSOM with complications can improve cure rates and prevent mortality.
AbstrakPendahuluan : Tuli mendadak merupakan salah satu kegawatdaruratan di bagian telinga hidung tenggorok bedah kepala dan leher (THT-KL). Tuli mendadak atau sudden sensorineural hearing loss (SSNHL) adalah tuli sensorineural lebih dari 30 dB pada 3 frekuensi berturut turut secara mendadak dalam waktu 3 hari. Etiologi tuli mendadak hingga saat ini belum diketahui dengan pasti, namun penyebab tersering tuli mendadak, yaitu idiopatik (71%). Penatalaksanaan tuli mendadak meliputi terapi konservatif, salah satunya dengan pemberian kortikosteroid secara sistemik dan lokal. Pemberian lokal dapat dilakukan dengan cara injeksi langsung intratimpani. Terapi kortikosteroid secara lokal dapat diberikan sebagai terapi primer, terapi adjuvan (kombinasi) dan salvage therapy. Laporan kasus : seorang pasien perempuan berusia 36 tahun dengan diagnosis tuli mendadak pada telinga kanan yang dilakukan salvage therapy dengan penyuntikan deksametason intratimpani sebanyak empat kali secara selang 3 hari setelah gagal dengan terapi sistemik. Kesimpulan : Injeksi kortikosteroid intratimpani digunakan sebagai salvage therapy dapat menjadi pilihan untuk pasien yang gagal diterapi dengan kortikosteroid sistemik. Kata kunci: tuli mendadak; salvage therapy; deksametason; intratimpani AbstractIntroduction: Sudden deafness is one of the emergency in otorhinolaryngology head and neck surgery (ORL-HNS). Sudden deafness or sudden sensorineural hearing loss (SSNHL) is sensorineural deafness of more than 30 dB at 3 frequencies within 3 days. The etiology of sudden deafness is recently remain unknown, but the most common cause of deafness is idiopathic (71%). Management of sudden deafness includes conservative therapy, one of which is corticosteroids systemically and locally. Local delivery is done by intratympanic injection. Local corticosteroid therapy can be administered as primary therapy, adjuvant therapy (combination) or salvage therapy. Case report : a female patient aged 36 years old with diagnosis of sudden deafness at right ear which is performed salvage therapy with dexamethasone intratympanic injection four times with interval of 3 days after systemic therapy was failed. Conclusion : corticosteroid intratympanic injection as salvage therapy may be an option for patients who failed therapy with systemic corticosteroids.
Kasus Covid-19 yang merebak di Indonesia mengharuskan dilakukannya upaya dari berbagai pihak untuk mengatasinya. Fakultas Kedokteran Universitas Andalas turut berupaya melakukan upaya pencegahan dan pengendalian Covid-19. Tujuan kegiatan ini adalah untuk meminimalisir kemungkinan penyebaran pesat Covid-19 dan usaha preventif untuk menjaga agar masyarakat sekitar Pauh, Kuranji serta Air Dingin tetap sehat dan terhindar dari Covid-19. Kegiatan ini dilaksanakan di tiga puskesmas yaitu: Puskesmas Pauh, Puskesmas Kuranji dan Puskesmas Air Dingin Kota Padang. Metode yang digunakan adalah KIE (Komunikasi Informasi Edukasi) seputar penyakit Covid-19 berupa publikasi artikel pada media masa dan pemberian masker. Sasaran kegiatan adalah masyarakat sekitar Pauh, Kuranji serta Air Dingin Kota Padang. Hasil kegiatan yang diperoleh yaitu telah memproduksi APD (Alat Pelindung Diri) sebanyak 80 faceshield, 400 masker serta 60 hazmat suit dengan melibatkan UMKM (Usaha Mikro Kecil Menengah) dan konveksi. Selanjutnya APD ini disebarluaskan kepada puskesmas yang membutuhkan, yaitu Puskesmas Pauh, Puskesmas Kuranji dan Puskesmas Air Dingin Kota Padang. Selanjutnya didistribusikan kepada pihak-pihak yang membutuhkan, yaitu masyarakat dan tenaga medis. Program pencegahan dan pengendalian Covid-19 dilakukan untuk menekan dan mengurangi angka positif Covid-19 serta melindungi tenaga medis agar dapat memberikan pelayanan maksimal kepada pasien. Selanjutnya pembuatan APD yang melibatkan UMKM dan konveksi dapat membantu perekonomian masyarakat yang merosot akibat pandemi ini.
Pendahuluan: Hipofungsi vestibuler perifer bilateral merupakan kasus yang jarang ditemui dan memiliki gejala gangguan keseimbangan yang menyebabkan gangguan penglihatan dan stabilitas postural. Etiologi penyakit ini sebagian besar idiopatik, selain itu disebabkan oleh zat ototoksik, penyakit autoimun, infeksi dan neoplasma yang melibatkan telinga dalam. Diagnosa ditegakkan berdasarkan gejala klinis, pemeriksaan keseimbangan sederhana dan tes khusus seperti dynamic visual acuity test (DVA), Rotatory Chair dan elektronistagmografi (ENG) pada tes kalori sebagai standar baku. Tatalaksana hipofungsi vestibuler perifer bilateral terutama berupa terapi rehabilitasi vestibuler berupa terapi adaptasi, subtitusi dan kompensasi/ habituasi lebih dipilih jika dibandingkan dengan terapi farmakologis atau terapi bedah. Laporan Kasus: Seorang pasien usia 65 tahun datang dengan keluhan pusing berputar dan berkurangnya pendengaran. Pemeriksaan pendengaran menunjukkan hasil tuli sensorineural dan tes DVA menunjukkan adanya penurunan tiga baris huruf pada Snellen chart dan elektronistagmografi menunjukkan paresis kanal bilateral. Pasien didiagnosis dengan hipofungsi vestibuler perifer bilateral ec. suspek prebiastasis. Tatalaksana adalah dengan terapi rehabilitasi vestibuler dengan metode adaptasi. Simpulan: Hipofungsi vestibuler perifer bilateral merupakan kasus yang jarang ditemui, dapat dibedakan dengan pemeriksaan objektif spesifik yaitu elektronistagmografi. Terapi pilihan utama untuk penyakit ini adalah berupa terapi rehabilitasi vestibuler metode adaptasi, subtitusi dan habituasi.
