RESUMO O Telecuidado Farmacêutico consiste na prestação de serviços farmacêuticos clínicos de forma remota, através da utilização de tecnologias de informação e telecomunicações, tendo o potencial de melhorar resultados em saúde e reduzir os problemas relacionados ao uso de medicamentos. As vantagens adicionais da telefarmácia incluem o custo-benefício, possibilitando o acesso ao serviço de acompanhamento farmacêutico e a possibilidade de atingir pessoas em localidades remotas e em condições de saúde que limitam a mobilidade. Considerando esse novo paradigma de oferta de serviços farmacêuticos, bem como o atual quadro pandêmico causado pelo novo coronavírus 2019 (COVID-19), em 5 de maio de 2020 foi lançado pela Secretaria de Estado da Saúde do Rio Grande do Sul, um programa de implantação do Cuidado Farmacêutico, CUIDAR +, e o primeiro serviço: Telecuidado Farmacêutico. O objetivo deste estudo é apresentar como o serviço de Telecuidado Farmacêutico foi estruturado e ofertado no combate à COVID-19 como uma estratégia emergente. O serviço foi desenvolvido a partir da avaliação das necessidades em saúde dos usuários da Farmácia de Medicamentos Especiais de Porto Alegre e definido por meio de modelagem de serviços. Foi organizado para acompanhar os pacientes com doenças respiratórias crônicas, orientando em relação ao uso correto dos medicamentos, otimizando a adesão ao tratamento e orientando em relação às dúvidas relacionadas à COVID-19.
Budget Impact Analyses require a set of essential information on health technology innovation, including expected rates of adoption. There is an absence of studies investigating trends, magnitude of budgetary effects and determinants of diffusion rates for health technology innovations worldwide during the last decades. The present study proposes a pilot assessment on main determinants influencing diffusion rates of pharmaceutical innovations within the Brazilian Unified National Health System (SUS). Data from the Brazilian Health Informatics Department (DATASUS) was gathered to establish the main determinants of diffusion rates of health technology innovations in Brazil, specifically referring to pharmaceutical innovations incorporated in the Brazilian Program for Specialized Pharmaceutical Services (CEAF) at SUS. Information was retrieved on DATASUS relating to patients who had used one of the medicines incorporated into CEAF at least three years prior to the beginning of the study (2015) for treatment of each health condition available. Thus, data from patients adopting 10 different medicines were analyzed in the study. Results from the zero-one inflated beta model showed a higher influence on diffusion rates of pharmaceutical innovations due to: number of pharmaceutical competitors for treatment of the same disease available at CEAF (negative); medicine used in combination with other medication (positive); and innovative medicine within the SUS (positive). Further research on diffusion rates of health technology innovations is required, including wider scope of diseases and medications, potential confusion factors and other variables that may influence rates of adoption in different health systems.
Introdução: o acesso dos usuários aos medicamentos do Componente Especializado da Assistência Farmacêutica (CEAF) e do Programa de Medicamentos Especiais da Secretaria de Saúde do Rio Grande do Sul (SES/RS) ocorre nas Farmácias de Medicamentos Especiais (FME), estabelecimentos descentralizados em todo o estado para as Secretarias Municipais de Saúde nos 497 municípios. Nas FME, a solicitação dos medicamentos e fórmulas nutricionais é realizada mediante entrega presencial dos documentos de acordo com as diretrizes definidas nos PCDTs. Em face do aumento da demanda por novas tecnologias incorporadas no Sistema Único de Saúde (SUS) e objetivando ampliar e facilitar o acesso dos usuários aos medicamentos, a SES/RS desenvolveu e implantou uma plataforma digital inédita no País para consulta, solicitação e renovação de medicamentos denominada Farmácia Digital RS. Objetivos: relatar a implantação da Farmácia Digital RS na SES/ RS disponibilizada à população para consulta, solicitação e renovação de tratamentos padronizados no SUS. Métodos: relato de caso acerca da concepção e implantação de um serviço digital de solicitação, renovação e consulta de tratamentos do CEAF e do Programa de medicamentos Especiais no âmbito do SUS no Estado do RS. Resultados: a Farmácia Digital RS foi implantada em 2020 disponibilizando 75 medicamentos e fórmulas nutricionais de 12 linhas de cuidado do CEAF para usuários maiores de idade, que correspondiam a 24% dos usuários que acessaram as FME no RS, naquele período. Em março de 2022, um total de 205 medicamentos e fórmulas nutricionais de 55 linhas de cuidado estavam habilitados para a primeira solicitação de medicamentos de forma digital, podendo beneficiar cerca de 54,1% dos usuários que demandam medicamentos à SES/RS, correspondendo a 143.090 pessoas. Em 2021 também foi adicionada à plataforma a renovação de tratamentos de todos os usuários cadastrados nas FME do RS, estendendo o acesso para todas as pessoas que já retiravam medicamentos de uso contínuo nas farmácias e que necessitam de reavaliação a cada 6 meses, facilitando o acesso e redução nos prazos para continuidade do tratamento. Conclusão: A SES/RS foi pioneira entre as secretarias estaduais de saúde no lançamento de uma plataforma digital, no âmbito da Assistência Farmacêutica, no SUS, possibilitando que solicitações administrativas de medicamentos e fórmulas nutricionais sejam realizadas digitalmente, sem necessidade de que o usuário ou seu representante realize a abertura de processo administrativo presencial. Constata-se que foi viabilizado aos usuários acesso facilitado para solicitação de tratamentos pela via digital, podendo acompanhar a solicitação diretamente na plataforma, que fica disponível em tempo integral. Concomitantemente, ao profissional farmacêutico é ampliado o escopo de suas atividades voltadas ao cuidado farmacêutico e à orientação sobre o uso racional dos medicamentos, visando acompanhar os usuários que são atendidos pelas FME do estado.
Os medicamentos, quando adequadamente selecionados e usados de forma racional, se caracterizam como importantes tecnologias para a proteção e recuperação da saúde de uma população. Todavia, a incorporação de novos medicamentos é uma das principais formas de pressão nos sistemas de saúde, inclusive no Sistema Único de Saúde. Por isso, é fundamental o estabelecimento de fluxos para a avaliação da incorporação daquelas tecnologias mais custo-efetivas, como o Brasil tem feito a partir de 2006, com o estabelecimento da Política Nacional de Gestão de Tecnologias. O presente artigo descreve, de forma crítica e reflexiva, as estratégias adotadas pelo Ministério da Saúde para viabilizar, economicamente, o acesso àqueles medicamentos incorporados pelo SUS após a recomendação da Comissão Nacional de Incorporação de Tecnologias no SUS (CONITEC). A ampliação seletiva das aquisições centralizadas e o estabelecimento de acordos de preços com os fabricantes são as principais estratégias utilizadas pelo Ministério da Saúde para a redução dos preços dos medicamentos e viabilizar a incorporação de novos medicamentos. Os recursos otimizados têm sido utilizados na garantia do acesso universal a essas tecnologias incorporadas, com a minimização do impacto sobre o orçamento anual do Ministério da Saúde.
