Sua incidência atinge 10 a 30% dos pacientes como importante causa de morbimortalidade, cuja mortalidade pode exceder a 25%. Notam-se os fatores de risco modificáveis como de fundamental importância na tomada de decisão para o seu tratamento e sua prevenção. Desta forma, foram descritos os fatores de risco modificáveis para pneumonia associada à ventilação mecânica invasiva nos pacientes internados em Unidades de Terapia Intensiva.Métodos: Trata-se de uma revisão da literatura nas bases Lilacs, Scielo, Medline e Bdenf para reunir e sintetizar publicações e posteriormente avaliar criticamente os fatores de risco para pneumonia associada à ventilação mecânica. Resultados:Predominou o uso inadequado ou indiscriminado de antibióticos, a falta de conhecimento sobre a microbiota da UTI e a não adesão da equipe às medidas preventivas. Conclusão: Os fatores de risco apontados precisam ser difundidos para todos os profissionais a fim de serem passíveis de ações diretas com impacto a curto prazo na prevenção e no controle efetivo da pneumonia associada à ventilação. Backgound and Objectives:Ventilator-associated pneumonia is a pulmonary infection that occurs 48 to 72 hours after endotracheal intubation and institution of mechanical ventilation, being considered one of the most feared adverse effects of intensive care therapy. Its incidence affects 10-30% of patients as an important cause of morbidity and mortality, of which mortality rate can exceed 25%. Modifiable risk factors are seen as crucial in decision-making for its treatment and prevention. Thus, the modifiable risk factors for pneumonia associated with invasive mechanical ventilation in patients admitted to the intensive care unit were described. Methods: This is a literature review carried out at Lilacs, SciELO, MEDLINE and Bdenf databases, to collect and summarize publications and subsequently, critically evaluate the risk factors for ventilator-associated pneumonia. Results: The inappropriate or indiscriminate use of antibiotics, lack of knowledge about the microbiota of the ICU and non-compliance of the team regarding preventive measures predominated. Conclusion:Professionals must be made aware of the identified risk factors in order to carry out direct actions with short--term impact in the prevention and effective control of ventilator-associated pneumonia. Trata-se de uma pesquisa bibliográfica que objetivou analisar a produção de conhecimento com o tema ventilação mecânica especificamente alarmes ventilatórios indexados no banco de dados National Library of Medicine (MedLine) no período de 1996 a 2006. A metodologia adotada foi a busca virtual com o descritor ventilation alarm(s). A amostra foi composta por quatro artigos desenvolvidos na área de anestesiologia e de enfermagem. A análise dos artigos foi baseada nas seguintes informações: o delineamento do estudo, o tamanho da amostra, o periódico de publicação e a associação ou não dos alarmes ventilatórios com o cuidado de enfermagem. Na anestesiologia, os alarmes ventilatórios foram estudados como auxiliares na implementação de novas terapias. Na área de enfermagem os alarmes aparecem sob dois aspectos: a incidência de alarmes ventilatórios em unidades de cuidados intensivos e a resolutividade das ações de enfermagem diante dos alarmes. Conclui-se que são poucas as produções realizadas sobre como age a equipe de enfermagem diante de um alarme ventilatório e sua importância na assistência ao paciente sob ventilação mecânica. SUMMARYOne is about a bibliographical research that objectified specifically to analyze the production of knowledge with the ventilation mechanics and especial the ventilatórios alarms in the data base National Library of Medicine (MedLine) in the period of 1996 the 2006. The adopted methodology was the virtual search with the describer ventilation alarm (s). The sample was composed for four articles developed in the nursing and anestesiologia area. The analysis of articles was based on the following information: the delineation of the study, the size of the sample, periodic of publication and the association or not of the ventilatórios alarms with the care of nursing. In the anestesiologia, the ventilatórios alarms had been studied as assistant in the implementation of new therapies. In the nursing area the alarms appear under two aspects: the incidence of ventilatórios alarms in units of intensive cares and the influence of the actions of nursing ahead of the alarms. One concludes that the carried through productions are few on as it ahead acts the team of nursing of a ventilatório alarm and its importance in the assistance to the patient under ventilation mechanics.
Pesquisa com o objetivo de descrever as características dos pacientes críticos com hipoglicemia grave. Estudo transversal, prospectivo, realizado em 2014, em hospital público federal, com população de 42 pacientes do pós-operatório de cirurgia cardíaca que receberam insulina intravenosa, dos quais 15 apresentaram hipoglicemia. As características, com associação positiva e significância estatística para a ocorrência de hipoglicemia, foram o hematócrito baixo (p=0.03852) e a cirurgia de emergência (p=0,02884).Outras características somente apresentaram associação positiva, como a insuficiência renal com tratamento dialítico, a PaO2 ≥90mmHg e o uso de vasoaminas (OR>2). Conclui-se que o enfermeiro deve levar em consideração as características que influenciam a glicemia para que evite erros na titulação das doses de insulina administrada.
