A ventilação não-invasiva melhora a venti-lação alveolar por criar um gradiente de pres-são transpulmonar sem a necessidade de uma via aérea artificial. Recentemente, Girou E et al. 1 publicaram uma pesquisa observacional, retrospectiva do tipo coorte avaliando 479 pacientes com doença pulmonar obstrutiva crônica (DPOC) ou edema agudo de pulmão (EAP) submetidos à ventilação mecânica con-vencional ou não-invasiva (VNI), durante um período de oito anos, em que se realizou concomitantemente o treinamento e se ela-boraram recomendações para facilitar a utili-zação da VNI. As principais variáveis estudadas foram a incidência de infecção adquirida na UTI e a taxa de mortalidade. Como resulta-dos, observou-se um aumento significativo da utilização da VNI com diminuição da freqüên-cia de pneumonia intrahospitalar de 20% em 1994 para 8% em 2001 (p = 0.04) e do risco de óbito (OR 0.37; IC 95%: 0.18-0.78). Comentário O benefício associado com a utilização da VNI resulta primariamente da redução do uso da ventilação pulmonar mecânica com intubação intratraqueal 2. Os sistemas invasivos como catéteres intravasculares e tubos intratraqueais são fatores de risco para infecção intra-hospita-lar 3. Nos pacientes submetidos à VNI a invasividade relacionada aos cuidados dos paci-entes é menor 4. Portanto, este estudo é interes-sante para nós clínicos, que tentamos passar para a prática clínica os resultados das pesquisas sobre VNI, entretanto, as evidências estão restritas a alguns tipos de pacientes adultos, principalmente com DPOC e EAP, não podendo ser extrapolados os dados existentes até o momento para pacientes na faixa etária pediátrica. Sua incidência atinge 10 a 30% dos pacientes como importante causa de morbimortalidade, cuja mortalidade pode exceder a 25%. Notam-se os fatores de risco modificáveis como de fundamental importância na tomada de decisão para o seu tratamento e sua prevenção. Desta forma, foram descritos os fatores de risco modificáveis para pneumonia associada à ventilação mecânica invasiva nos pacientes internados em Unidades de Terapia Intensiva.Métodos: Trata-se de uma revisão da literatura nas bases Lilacs, Scielo, Medline e Bdenf para reunir e sintetizar publicações e posteriormente avaliar criticamente os fatores de risco para pneumonia associada à ventilação mecânica. Resultados:Predominou o uso inadequado ou indiscriminado de antibióticos, a falta de conhecimento sobre a microbiota da UTI e a não adesão da equipe às medidas preventivas. Conclusão: Os fatores de risco apontados precisam ser difundidos para todos os profissionais a fim de serem passíveis de ações diretas com impacto a curto prazo na prevenção e no controle efetivo da pneumonia associada à ventilação. Backgound and Objectives:Ventilator-associated pneumonia is a pulmonary infection that occurs 48 to 72 hours after endotracheal intubation and institution of mechanical ventilation, being considered one of the most feared adverse effects of intensive care therapy. Its incidence affects 10-30% of patients as an important cause of morbidity and mortality, of which mortality rate can exceed 25%. Modifiable risk factors are seen as crucial in decision-making for its treatment and prevention. Thus, the modifiable risk factors for pneumonia associated with invasive mechanical ventilation in patients admitted to the intensive care unit were described. Methods: This is a literature review carried out at Lilacs, SciELO, MEDLINE and Bdenf databases, to collect and summarize publications and subsequently, critically evaluate the risk factors for ventilator-associated pneumonia. Results: The inappropriate or indiscriminate use of antibiotics, lack of knowledge about the microbiota of the ICU and non-compliance of the team regarding preventive measures predominated. Conclusion:Professionals must be made aware of the identified risk factors in order to carry out direct actions with short--term impact in the prevention and effective control of ventilator-associated pneumonia. • AbstTact: Tbis work is based on the analysis of 99 zooplankton samples collected during the Oceanographic Expedition "ESPÍRITo SAmO I", held along the east coast of Brazil, between Cabo Frio and Abrolhos Archipelago, an area characterized by the occurrence of coastal upwelling, from July to September 1984. Zooplankton was collected with vertical plankton net hauls of 250 Jlm mesh size in the upper 200 meters layer. A total of twenty one species of Siphonophora was observed, two of which were identified as being physonect and the other as ca1ycophorans. Specific diversity dose to the shore and at the neighborhood of the Vit6ria-Trindade Bank System, showed smaller values, in comparison with those in the oceanic regions. Factorial analysis was used in order to access the changes observed in the population of the eJeven most abundant species. The first two principal axes represented the influence of the nearshore -offshore gradient and the role of trophic interaction, accounted for 60% of the total variance of the data.
