It is important to reduce the number of inpatient cases; this can be achieved, for example, by the implementation of adequate guideline-oriented patient-care programs, as well as patient education for the improvement of therapy adherence.
Fewer patients in the intervention group were admitted and readmitted to hospital, and lower inpatient treatment costs were identified. The physician contact rate was higher than in the control group.

What is known and objective
Health economic evaluation (HEE) is increasingly used in healthcare decision‐making on the allocation of limited resources in national healthcare systems. Although the methods used for HEE vary in different countries, all economic evaluations address two questions: Are limited resources used optimally? Is value for money achieved in their use? Our objective is to explain some fundamental concepts in HEE and how these concepts are adapted in different countries, notably in Germany.
Methods
We performed a bibliographic search to identify existing methods of health economic evaluation of new drugs used by the official agencies of 11 countries (Austria, Australia, Canada, Finland, France, the Netherlands, Norway, New Zealand, Sweden, the United States and England and Wales) and compared them with that used by the German national agency IQWiG.
Results and discussion
All countries considered follow internationally established standards of HEE. The majority of countries, including Germany, utilize primary outcome parameters such as disease‐related morbidity and mortality for assessing relative efficacy and effectiveness. The most frequently recommended form of health economic evaluation is the cost–utility analysis (CUA). The German IQWIG is the only HTA body to use the cost–benefit concept of ‘efficiency frontier’ in its assessment.
What is new and conclusion
While the core principles of HEE are the same worldwide, there is a lack of harmonization in the details. This requires resource‐consuming adaptations in the analyses to meet different national requirements. We describe the core principles of HEE as a common basis for further discussions by all stakeholders.

Die Kommission 1.14 ?Qualit?tssicherung/Anhaltszahlen? der DGN hat nach 2012 mit Bezug auf 2011 erneut eine Erhebung zur Struktur neurologischer Kliniken durchgef?hrt. Die hier berichtete Erhebung fand 2014 statt mit Bezug auf das Jahr 2013. Durch vielfache Aussendung der Umfrage konnte eine verbesserte Antwortquote von 66?% erreicht werden. Mit wenigen Ausnahmen halten neurologische Kliniken Schlaganfalleinheiten vor. Erkennbar st?rker als in 2011 waren Neurologien in interdiszplin?re Notaufnahmen eingebunden. W?hrend sich die Neurologie klinisch zum Notfallfach wandelt, stagniert die infrastrukturelle Ausstattung, z.?B. die Verf?gbarkeit von diagnostischen Leistungen rund um die Uhr. Insbesondere f?r die Bereiche Liquordiagnostik und MRT-Bildgebung sind Defizite zu erkennen, die eine leitliniengerechte Versorgung aller neurologischen Notf?lle an einer relevaten Anzahl von Kliniken unm?glich macht. Deutlich r?ckl?ufig ist auch die Verf?gbarkeit einer interventionellen Radiologie. Ein Gro?teil der Kliniken halten sektor?bergreifend ambulante Versorgungsstrukturen vor. Die durchschnittliche Klinikgr??e blieb unver?ndert, lediglich bei den Gro?kliniken ist der Trend zu einer weiteren Expansion von Betten erkennbar. Die Leistungszahlen belegen im Vergleich zur Vorerhebung auch eine weitere Zunahme der Fallzahl. Die Verweildauer sinkt nun auf 6,5 Tage und ist damit 12?% k?rzer als 2009. Damit lag einerseits die Verweildauer absolut betrachtet deutlich unter dem bundesdeutschen Durchschnitt aller Fachdisziplinen von 7,5 Tagen und der Verweildauerr?ckgang war zudem deutlich akzeleriert (12?% in neurologischen Kliniken versus 6?%, Bundesdurchschnitt aller Fachdisziplinen). Gleichzeitig kam es zu einem tendenziell abnehmenden durchschnittlichen Fallerl?s. Die Angaben zur Personalst?rke waren heterogen. Die Abgrenzung des Personaleinsatzes im DRG-Bereich von dem in ambulanten Strukturen, in Lehre und Forschung gelang vielen Kliniken nicht. Ein Personalmangel war nur in wenigen H?usern so ausgepr?gt, dass Versorgungsleistungen eingeschr?nkt werden mussten. W?hrend die Mehrzahl der Kliniken sich einem oder mehreren Zertifizierungsprozessen unterzogen haben, war weiter nur ca. die H?lfte der Anwortenden davon ?berzeugt, dass dieses die Versorgungsqualit?t ihrer Patienten g?nstig beeinflusse. Neue Impulse und Ideen f?r eine patienten- und ergebnisorientierte Qualit?tssicherung lassen sich der Umfrage nicht entnehmen.
This study shows that CT-guided periradicular infiltration in lumbosciatica caused by intervertebral disc herniation is significantly superior to non-imaging, anatomical landmark-guided infiltration, regarding the parameters investigated. The high number of treatment failures in the non-imaging group underlines the inferiority of this treatment concept.
