Vitiligo although a cosmetic disease without any symptoms, it carry a significant social stigma especially in Indian society. Data interpretation in this study indicates that vitiligo affects QOL in majority of vitiligo patients and such patients require more aggressive and empathic attitude from a dermatologist to cure/improve this so called chronic cosmetic disease.
Bowens' disease (BD) is a precancerous condition of skin and/or mucosa with a predilection towards sun-exposed areas. Extensive literature research failed to reveal any case of multiple pigmented BD in type V Fitzpatrick skin. Multiple BD is a therapeutic challenge with a tendency to recur. Here we present an otherwise healthy Indian male having multiple pigmented lesions of BD on sun-protected sites of the body mimicking malignant melanoma. These lesions were refractory to treatment with different modalities. This paper is an attempt to review the available literature regarding the pigmented variant of multiple BD. Rationale for a therapeutic trial of UVB therapy for multiple arsenic induced BD is also discussed.
suggestive of collodion membrane at birth. Child started developing tonic-clonic seizures by 6 months of age. He developed eye contact and started holding his head at 6 months of age. He started sitting at 12 months of age. He was operated for right-sided inguinal hernia at the age of 11 months. At presentation, he was unable to speak monosyllables.On physical examination, weight was 7 kg, length was 68 cm and head circumference was 39 cm (all <3 rd percentile for age). He had a prominent forehead with large ears [ Figure 1]. Cutaneous examination revealed nonscarring alopecia over the scalp and eyebrows. Skin over the abdomen was rough in texture due to multiple, spiny, uniformly distributed, white follicular projections [ Figure 2]. Skin over the scalp, face and shin had irregular icthyotic patches. Palms, soles, dentition, oral cavity, genitalia and nails appeared normal. Ophthalmic examination was normal except marked photophobia. Routine hematological and biochemical investigations were normal. Skeletal survey, electrocardiogram, ultrasonography of the abdomen, brainstem evoked auditory response failed to reveal any abnormality. Computed tomography scan head revealed evidence of subtle loss of the right occipital and parietal subcortical white matter. Until date, only 40 patients have been mentioned in the worldwide literature. The management of this syndrome remains a daunting task because very few case reports have described interventions for treating various clinical features of this entity. We describe IFAP syndrome in an 18-month-old male child, who showed transient improvement in his cutaneous features with oral isotretinoin therapy.
Background. Psoriasis is associated with a high impact on health-related QoL (quality of life). PUVAsol has been successfully used for treating psoriasis instead of standard PUVA therapy in developing countries. However, data for PUVAsol therapy and its effect on QoL in psoriatic patients is meagre. Objective. To investigate the effect of PUVAsol on the quality of life in patients having chronic plaque psoriasis. Materials and Methods. An observational prospective study done in patients having chronic plaque psoriasis. PASI and DLQI were calculated before initiating treatment with oral PUVAsol. These were compared with the respective scores after 12 weeks of regular treatment with PUVAsol. Statistical analysis was done using SPSS version 20.0. Results. Both PASI and DLQI showed statistically significant reduction after 12 weeks of regular treatment. 90% of patients responded favourably to PUVAsol therapy in the study and all the domains of DLQI showed significant reduction except domain of “work and school.” Conclusion. Our results show that regular PUVAsol treatment improves the physical appearance of disease as evident by decrease in PASI scores. It also improves the QoL of the patients. This study will add upon the growing evidence of efficacy of PUVAsol.
Introduction: Non adherence to antiepileptic drugs (AEDs) can vary the course of epilepsy. There is a dearth of data regarding adherence status among paediatric epilepsy patients. Objectives: To determine the level of AED adherence among paediatric patients having epilepsy and to assess the relationship between AED adherence and treatment satisfaction among paediatric epileptics. Method: A cross sectional study was conducted at a tertiary level referral hospital in northern India from July 2015 to June 2016 on children aged 2 to 13 years of age, diagnosed with epilepsy and treatment initiated with at least 1 AED for the past 6 months. Data collection was done using a questionnaire which was divided into three parts. The first part comprised demographical information about patients and their caregivers along with clinical details of epilepsy. The second part gathered information about medication adherence profile with the help of the Morisky Medication Adherence Scale-8 (MMAS-8). The third part gathered data on treatment satisfaction profile about these patients with the help of Treatment Satisfaction Questionnaire for Medication (TSQM 1.4). The estimated sample size was at least 61 patients. Data was analysed using Graph Pad InStat 6.0. Ethical approval was obtained from the institutional ethics committee. Results: A convenient sample of 112 patients with their respective immediate caregivers was enrolled in this study. The mean age of children was 9.82±2.17 years and male to female sex ratio was 1.24. The complex issue of ethical drug advertising in dermatology journals requires constant review and discussion. Dermatologists should be cautious in assessing any advertisement or claim even if it seems evidence-based. The results from our study highlight the need for a global, proactive and effective regulatory system to ensure ethical medicinal drug advertising in medical journals.
Introduction. Topical application of local anesthetics is currently considered to be the easiest, most effective, and convenient way for treatment of patients who may be undergoing superficial dermatosurgical procedures. Materials and Methods. This study compares the anesthetic potential of 2.5% lidocaine and 2.5% prilocaine topical cream with 7% lignocaine and 7% tetracaine combination cream for radio ablative dermatosurgery when applied, under occlusion, for 30 minutes. 40 subjects of achrocordons were enrolled in this split-side randomized trial. Result. The pain severity experienced by subjects in terms of visual analogue scale score was significantly lesser for lignocaine/tetracaine combination cream as compared to lidocaine/prilocaine combination. Conclusion. This small study proves the efficacy of lidocaine/tetracaine combination as a topical anesthetic cream when applied for a short time interval of 30 minutes. This will help a dermatosurgeon to perform various dermatological procedures in a better and efficient manner with a shorter waiting period for analgesia to set in.
