BACKGROUND: Aneurysms of the posterior communicating segment of carotid artery (PcomA) have a high risk of rupture; when these nonruptured aneurysms are associated with oculomotor nerve palsy (ONP), the risk of rupture increases compared with asymptomatic nonruptured PcomA. OBJECTIVE: To retrospectively analyze the risk factors involved in ONP secondary to PcomA aneurysm and to study the factors involved in the recovery time of ONP once it is established. METHODS: This was a retrospective study of patients from 10 neurosurgery centers from October 2008 to December 2020. We analyzed age at diagnosis, presence of compressive neuropathy of the oculomotor nerve, presence of aneurysm rupture, largest aneurysm diameter, aneurysm projection, smoking, hypertension, diabetes, time between diagnosis and surgical treatment, as well as the outcome. RESULTS: Approximately 1 in 5 patients (119/511 23.3%) with a PcomA presented with ONP. We found that patients with aneurysms measuring greater than or equal to 7.5 mm were 1.6 times more likely to have ONP than those with aneurysms smaller than 7.5 mm. In our study, the prevalence of smoking in the PcomA + ONP group was 57.76%, and we also found that smokers were 2.51 times more likely to develop ONP. A total of 80.7% showed some degree of improvement, and 45.4% showed complete improvement with a median recovery time of 90 days. CONCLUSION: This study showed that 80.7% of patients with PcomA aneurysms undergoing surgical treatment with aneurysm clipping showed some degree of improvement of the ONP, with a median time to recovery between 90 and 120 days.
O tumor de krukenberg é um carcinoma metastático ovariano gastrointestinal. A apresentação é inespecífica com dor abdominal e alterações intestinais. Em média 30% das neoplasias ovarianas são metastáticas, sendo o estômago sua origem mais comum (76%), seguido do colorretal (11%). O objetivo do estudo foi relatar e revisar na literatura um caso de um paciente de 24 anos diagnosticada com Tumor de Krukenberg após investigação com exames de imagens devido à sintomatologia de dor abdominal e enterorragia, onde se identificou adenocarcinoma de reto e massa anexial esquerda. Foi realizado retossigmoidectomia com anastomose colo-anal, ileostomia em alça de proteção e salpingooforectomia esquerda. O tumor de Krukenberg tem o estômago como principal órgão de origem, seguido de retossigmóide, em baixa proporção. O prognóstico não é favorável; sendo um diagnóstico raro com possibilidade de recorrência após cirurgia citorredutora e muitas vezes seguida de terapia paliativa. Portanto, sendo o relato descrito apenas para um único ovário e sua origem retal, com dificuldade para traçar um prognóstico, torna-se relevante, o que demonstra um caso raro e de menor incidência na literatura.
A Perfuração Iatrogênica do Cólon (PIC), é considerada uma das complicações mais graves relacionadas à colonoscopia. As PICs podem ser manejadas de maneira conservadora, cirúrgica e endoscópica. Trazemos o relato de um paciente do sexo masculino que realizou com sucesso o tratamento conservador e endosco em regime de internação hospitalar após o dignóstico de perfuração de colon pós-colonoscopia. A mesma ocorreu durante uma polipectomia e foi diagnosticada durante o procedimento.Palavras-chave: colonoscopia, perfuração de cólon, polipectomia, tratamento conservador ABSTRACT Iatrogenic Colon Perforation (ICP) is considered one of the most serious complications related to colonoscopy. ICPs can be managed conservatively, surgically and endoscopically. We present the report of a male patient who successfully underwent conservative and endoscopic treatment in a hospital stay after the diagnosis of colonic perforation after colonoscopy. It occurred during a polypectomy and was diagnosed during the procedure.
A tuberculose peritoneal consiste na presença da Mycobacterium tuberculosis na cavidade peritoneal, mesentério e/ou omento. Sua origem relaciona-se com a rotura linfonodal ou disseminação hematogênica. É de instalação insidiosa, com perda de peso, dor abdominal e ascite. A persistência na investigação e a opção pela laparoscopia possibilitaram a definição diagnóstica e o acesso ao tratamento adequado. A laparoscopia tem sido uma ferramenta decisiva neste percurso e com potencial terapêutico já em primeira avaliação.
