В представленных клинических рекомендациях обсуждаются современные подходы к диагностике и лечению дифференцированного рака щитовидной железы у взрослых. Изменения в настоящей редакции Клинических рекомендаций касаются показаний к пункционной биопсии, скринингового определения концентрации кальцитонина, унификации заключений цитологического исследования, новых подходах к послеоперационной динамической стратификации риска рецидива, показаний к супрессивной и заместительной терапии, таргетной терапии йоднегативных форм дифференцированного рака щитовидной железы.
Диффузный токсический зоб (болезнь Грейвса) является наиболее частой причиной гипертиреоза у детей и подростков. Методами выбора в лечении являются терапия тиреостатиками, оперативное вмешательство, радиойодтерапия (РЙТ). Изучение эффективности и безопасности РЙТ тиреотоксикоза у детей и подростков является актуальной задачей. В настоящей статье мы суммировали результаты серии клинических наблюдений и проанализировали эффективность и безопасность РЙТ диффузного токсического зоба (ДТЗ) у детей и подростков. Мы наблюдали всего 25 пациентов в воз-расте от 11 до 17 лет (в среднем 14,8 года) с ДТЗ. Десяти пациентам в 2016 г. РЙТ (активности 550-920 мБк) была про-ведена в ФГБУ ЭНЦ. Период в этой подгруппе наблюдения -6-11 мес. Вторая подгруппа (15 пациентов) была пролече-на в радиологическом отделении города Нижнего Тагила (Свердловская область) в период с августа 2005 г. по сентябрь 2012 г. Период наблюдения составил от 3,5 до 11,5 лет (в среднем 8,54 ± 2,87 года). РЙТ была проведена без каких-либо осложнений, непосредственных или отдаленных. У двух пациентов, имевших признаки эндокринной офтальмопатии в неактивной фазе, не возникло ухудшения глазных симптомов после РЙТ. У 17 (68%) из 25 пациентов через 6 мес после РЙТ развился гипотиреоз. В одном случае -эутиреоз. В остальных 7 наблюдениях зарегистрирован рецидив тиреоток-сикоза. Подгруппы пациентов не различались по возрасту, соотношению по полу, объему щитовидной железы и титру антител к рецептору тиреотропного гормона, но отличались по величине лечебной активности 131 I (подгруппа ЭНЦ -550-920 МБк; подгруппа из Нижнего Тагила -168-400 МБк). При этом эффективность лечения значимо не отличалась (p = 0,99): 68 и 73% соответственно. Таким образом, РЙТ является эффективным, хорошо переносимым и безопасным методом лечения ДТЗ у детей и подростков. Необходимо продолжить исследование в более многочисленной выборке, при больших сроках наблюдения, а также совершенствовать эффективность РЙТ. However, treatment protocol of children and adolescents doesn't exist. In the present case series study we have evaluated the effectiveness and safety of RIT in children and adolescents. We have observed totally 25 patients in age 11-17 years old (mean 14.8 years) with Graves' disease. Ten patients were treated with RIT in Endocrinology Research Centre (Moscow) in 2016 year with activities 550-920 MBq. Follow-up period varied 6-11 months in this subgroup. The second subgroup (15 patients) was treated in radiology department in Nijniy Tagil rural hospital (Ural region) in the period 2005-2012 years. Профилактика и контроль прогнозируемой токсичности.Ключевые слова: дифференцированный рак щитовидной железы, радиойодрезистентность, рак почки, ингибиторы ангиогенеза, ленватиниб.On 22 June 2017 in St. Petersburg the expert panel was held on the topic "Management of toxicity of angiogenesis inhibitors", which discussed current issues of systemic therapy of advanced differentiated thyroid cancer resistant to radioactive iodine therapy, advanced kidney cancer and questions of efficacy and safety of new target drugs in the treatment of these diseases. The reports and discussions of experts raised the following questions: 1. Own experience of using lenvatinib in patients with differentiated thyroid cancer refractory to therapy with radioactive iodine and kidney cancer. 2. Profile of efficacy and safety of modern targeted therapy with multikinase inhibitors. 3. Prophylaxis and management of predictable toxicity.
Болезнь Грейвса (БГ) – основная причина тиреотоксикоза у детей. В современной клинической практике применяется три метода лечения БГ: тиреостатики, радиоактивный йод и операция. Все эти методы лечения применяются давно, и в мире накоплен огромный опыт применения каждого из методов лечения. Лечение начинают практически всегда с тиреостатиков, непереносимость или неэффективность которых становится показанием к выбору альтернативных методов лечения. Для осознанного выбора метода лечения необходимо понимать его потенциальные параметры эффективности и безопасности, а также влияющие на них факторы. Цель обзора – критический анализ эффективности и безопасности радиойодтерапии в лечении тиреотоксикоза у детей и подростков в сравнении с аналогичными показателями других методов лечения, а также показания и противопоказания к данному виду лечения в современной клинической практике эндокринологов, радиологов и эндокринных хирургов. Выбор тактики лечения БГ у ребенка на каждом этапе осуществляется согласованно с ним и его родителями/опекунами путем информирования лечащим врачом об объективных преимуществах и недостатках методов лечения на основе имеющейся доказательной базы. Также приведены основные рекомендации эффективной и безопасной терапии радиоактивным йодом БГ у детей и подростков.
Medullary thyroid cancer (MTC) is about 5% of all thyroid carcinomas and is hereditary in 2030% of cases. Multiple endocrine neoplasia syndrome type 2 (MEN2), in addition to medullary thyroid cancer, may include pheochromocytoma, hyperparathyroidism, and some other manifestations. The multidisciplinary data bank (MDB) developed by us is a specialized resource for storing, accumulating and subsequent analysis of data on patients with MTC and MEN2 syndromes. The MDB allows you to collect data from any sources (primary care, medical institutions at various levels, federal centers) and provides internal automated control of the quality and quantity of input data. It is necessary to keep a record of such data to improve the efficiency of diagnostics, treatment and rehabilitation of patients. MDB is an integrated digital platform containing reference materials for doctors, which allows combining evidence-based clinical experience in patients, and provides the opportunity for expert support of medical decisions. In the long run, it will help us organize digitally stored data for machine learning and lays the groundwork for the development of artificial neural networks.
Modern multidisciplinary clinic is not only a medical, but also an engineering and often biotechnical facility. The technological complexity of the object depends on the planned (bio)medical profiles and functionality, the need for scalability and upgradeability, as well as many other factors. When viewed from the outside, the creation, from an idea to commissioning, of a modern specialized or multidisciplinary medical center does not look prohibitively complicated, and its stages (pre-project surveys, medical and technical specifications, draft design, design stages, construction, equipping and entering the planned production power) are seen as understandable and achievable. However, our own experience of direct participation and analysis of the implementation of various specialized medical centers in our country indicates the presence of a lot of false prejudices, mistakes, outdated principles and other problems in practice. In the article, we analyze, explain and systematize typical misconceptions and vices when creating an oncology center, but the same problems arise when creating any multidisciplinary medical center. We believe that our experience will be useful for familiarization not only to designers, technologists and architects, but also to doctors, healthcare organizers, as well as to all specialists involved in the creation of modern medical centers.
One of the most common threats to reducing the duration and quality of life among the population is malignant tumors, which have high biological aggressiveness. It is possible to increase the detection of aggressive tumors at an early, potentially curable, stage of the tumor only through specialized medical examination of high-risk groups among the population, taking into account gender, age, hereditary predisposition, the influence of carcinogenic and other factors. In this regard, there is a growing need for interdisciplinary communication, improvement of diagnostic and therapeutic technologies, an alliance of doctors, biologists, chemists, physicists, engineers and mathematicians for symbiosis in the development of biotechnologies, machine learning algorithms, replenishment of the evidence base and improvement of auxiliary resources of artificial intelligence. The article is devoted to the analysis of the challenges facing modern biomedicine and ways to solve them.
