Myome des Uterus sind die häufigsten soliden Tumoren der Frau. Sie sind hormonabhängig und durch ein gutartiges Wachstum charakterisiert. Bei ca. 20 -30 % aller Frauen über 30 Jahre finden sich Myome des Uterus. Die Zahl steigt bei 40-jährigen Frauen auf über 40 % an [150]. Bei 1245 durchgeführten Hysterektomien wiesen 69 % der Patientinnen Myome auf [74]. Durch ihre Hormonabhängigkeit stellen Myome ein Problem der Frau im reproduktiven Alter dar. Dennoch muss davon ausgegangen werden, dass die meisten Myome klein und asymptomatisch sind und keiner medizinischen Intervention bedürfen. Im Rahmen von Autopsien fanden sich Myome bei 77 % aller Frauen [25]. Größere Myome bzw. solche mit entsprechender Lokalisation können symptomatisch werden und sind häufig der Grund für operative Eingriffe [166]. Auffällige Myome sind häufig assoziiert mit folgenden Symptomen: Schmerzen, Blutungsunregelmäßigkeiten, rezidivierenden Aborten und Sterilität [52,120]. Die Probleme, die durch Myome hervorgerufen werden, sind der häufigste Grund für die Entfernung der Gebärmutter. In den USA wie auch in Deutschland sind Myome der Grund für ein Drittel aller durchgeführten Hysterektomien. Dies entspricht jährlich ca. 200 000 Hysterektomien in den USA [60, 200]. Im Jahr 2000 war die Diagnose Leiomyome des Uterus mit insgesamt 94 066 auf Rang 13 aller Krankenhausdiagnosen bei Frauen in Deutschland [175]. Myome werden bei Schwarzen 2-mal häufiger diagnostiziert als bei Weißen, während die Häufigkeit bei Asiatinnen und Latinos mit der von Weißen vergleichbar ist. Die höchste Inzidenz wird für Weiße zwischen dem 40. und 44. Lebensjahr angegeben. Trotz der hohen durch Myome verursachten Morbidität ist über die komplexe molekulare Ätiologie der uterinen Myome bisher wenig bekannt [166]. Risikofaktoren Einer der größten Risikofaktoren ist das steigende prämenopausale Alter der Frau, d. h. die längere Dauer der reproduktiven Lebensphase [52,166]. Obwohl die Angaben in der Literatur nicht einheitlich sind, scheint eine frühe Menarche mit einem höheren Risiko für die Entstehung von Myomen verbunden zu sein. Parität und Myomentstehung sind invers korreliert. Nulliparität oder verminderte Fertilität mit anovulatorischen Zyklen und vermehrtem nicht antagonisiertem Östrogeneinfluss auf das Myometrium ist mit zunehmender Häufig-GebFra-Refresher Die Hormonabhängigkeit macht die gutartigen Myome zu einem Problem der Frau im reproduktiven Alter. Symptome der Myome: Schmerzen, Blutungsunregelmäßigkeiten, rezidivierende Aborte, Sterilität. Die meisten Myome sind asymptomatisch. Das steigende prämenopausale Alter ist der größte Risikofaktor.
Einleitung Das Ullrich-Turner-Syndrom (UTS) betrifft 1 : 2500 weibliche Neugeborene und stellt somit die häufigste Form der Gonadendysgenesie dar. Verursacht wird das UTS durch eine vollständige oder partielle X-chromosomale Monosomie bei weiblichem Phänotyp [1]. Zusätzlich finden sich beim UTS bestimmte variable phänotypische Charakteristika, wie z. B.: ■ Kleinwuchs ■ Lymphödem im Bereich des Hand-und Fußrückens ■ Flügelfell ■ tiefer Haaransatz ■ Schildthorax ■ weiter Mamillenabstand ■ Hörstörungen ■ kardiovaskuläre Fehlbildungen wie Aortenisthmusstenose ■ renale Fehlbildungen wie eine Hufeisenniere In ca. 90% der Fälle kommt es aufgrund der "StreakGonaden" zu einer primären Ovarialinsuffizienz mit hypergonadotropem Hypogonadismus; etwa 10% der Mädchen mit UTS haben eine normale Pubertätsent-wicklung mit normalen Menstruationszyklen. Die Diagnose UTS sollte bereits vom Kinderarzt aufgrund der typischen Symptome oder spätestens bei der Abklärung eines Kleinwuchses gestellt werden. Dennoch können auch in der gynäkologischen Praxis Patientinnen in der Adoleszenz wegen Pubertas tarda, primärer oder sekundärer Amenorrhö oder im Erwachsenenalter bei Infertilität mit einem UTS diagnostiziert werden. Die Diagnose UTS erfolgt meist bereits durch den Pädiater, seltener durch den Gynäkologen. Aus gynäkologischer Sicht stehen beim UTS v. a. die durch die Ovarialinsuffizienz notwendig werdenden Therapien von der Pubertätsinduktion über die weiterführende Hormontherapie bis hin zur Beratung bei Infertilität oder Betreuung in der Schwangerschaft im Fokus. Tipp für die Praxis Aufgrund der großen Bandbreite v. a. endokrinologischer und internistischer Probleme, die mit dem UTS einhergehen können, darf die Sicherstellung der interdisziplinären Betreuung der Patientin auf keinen Fall in Vergessenheit geraten. Wird die Diagnose eines UTS in der Kindheit gestellt, dann werden die Mädchen mit UTS meist in spezialisierten pädiatrischen Zentren umfassend betreut und aufgrund des Kleinwuchses mit Wachstumshormon behandelt.
