Patients with T2D have significantly higher levels of A-FABP and lower levels of adiponectin. These adipokines correlate with indicators of vascular damage and could contribute to cardiovascular risk in patients with T2D. A-FABP may participate in direct endothelium damage.

Cardiovascular (CV) disease is the primary cause of death in diabetic patients and one of the explanations may be increased arterial stiffness. Arterial stiffness assessment using pulse wave analysis, is a predictive factor of CV events. The aim of this paper is to review the current knowledge of relations between diabetes mellitus and pulse wave analysis. A MEDLINE search was performed to retrieve both original and review articles addressing the relations and influences on arterial stiffness in diabetics. Pulse wave analysis is considered as a gold standard in CV risk evaluation for patients at risk, especially diabetics. Arterial stiffness assessment may be helpful for choosing more aggressive diagnostic and therapeutic strategies, particularly in younger patients to reduce the incidence of CV disease in these patients.

Aim of this trial was to assess (1) the accuracy and precision of electrochemistry-based glucometers CONTOURLINK, CALLA, and LINUS and (2) the diabetes control using Ambulatory Glycaemic Profiles (AGP) as markers of therapeutic effectiveness. Glucometers and COBAS INTEGRA 400 Plus analyzer were used by one laboratory professional to estimate P-glucose (PG) in 112 out-patients. There were 112 sets of 12 PG estimations analyzed. In each set, means of 3 capillary PG estimations on 3 respective glucometers and on INTEGRA analyzer were calculated. The statistical program SPSS, v. 15, was applied. The mean INTEGRA PG values ranged from 2.7 to 25.3 mmol/L. There were strong correlations between mean PG on INTEGRA versus CONTOURLINK, versus CALLA, and versus LINUS; PG deviations from INTEGRA were mostly within the range ±15%. Wilcoxon Signed Rank Test revealed differences between CONTOURLINK-INTEGRA and LINUS-INTEGRA; CALLA-INTEGRA showed no difference. SD INTEGRA = 0.061 mmol/L, SD CONTOURLINK/SD CALLA/SD LINUS were 0.256/0.290/0.286 mmol/L. All patients were trained to perform defined 10-point PG profiles to adapt food, exercise, and insulin doses. The PG differences between all tested glucometers and reference values were in borderline of ISO 15197 but worthy of consideration. AGP are helpful markers of diabetes control.

To compare circulating pigment epithelium derived factor (PEDF) levels in type 2 diabetes patients (T2D) with and without metabolic syndrome (MetS+/-) to healthy controls and assess PEDF association with plasminogen activator inhibitor-1 (PAI-1) and von Willebrand factor (vWF) as markers of endothelial dysfunction. Fifty T2D individuals and forty healthy controls were included. PEDF, PAI-1, vWF, anthropological parameters, lipids, and markers of insulin resistance were investigated in all subjects. Compared to controls only MetS+ diabetics had higher PEDF levels [14.2 (10.2-16.0) mg/l vs. 11.1 (8.6-14.4) mg/l; p<0.05]. PEDF significantly correlated: positively with body mass index (ρ=0.25), smoking (ρ=0.21), C-reactive protein (ρ=0.22), triglycerides (ρ=0.38), non-HDL-cholesterol (ρ=0.39), apolipoprotein B (ρ=0.38), fasting glucose (ρ=0.22), glycated hemoglobin (ρ=0.24), C-peptide (ρ=0.28), insulin (ρ=0.26); and negatively with HDL-cholesterol (ρ=-0.42) and apolipoprotein A1 (ρ=-0.27). Independent association of PEDF with vWF in T2DMetS- subjects was found. Significantly elevated PEDF in T2DMet+ patients and its association with adverse metabolic profile confirmed PEDF as a marker of insulin resistance. Negative independent association of PEDF with vWF in T2DMetS- patients may reveal its angio-protective role.

Background: Adiponectin, adipocyte fatty acid-binding protein (A-FABP), and fibroblast growth factor-19 (FGF-19) belong to proteins involved in glucose metabolism regulation. The aims of the study were to compare the plasma levels of these proteins in women with early diagnosed gestational diabetes mellitus (GDM) to those in healthy controls and to investigate their changes during pregnancy after early intervention. Methods: The study was undertaken as a case-control study. Early GDM diagnosis was based on repeated fasting plasma glucose ≥5.1 and <7.0 mmol/L during the first trimester of pregnancy and exclusion of overt diabetes. Age-matched controls comprised healthy pregnant and non-pregnant women. In addition to adipokines, clinical parameters and measures of glucose control were assessed. Results: Women with GDM (n = 23) had significantly lower adiponectin and higher A-FABP levels compared to healthy pregnant (n = 29) or non-pregnant (n = 25) controls, while no significant differences in FGF-19 between the groups were found. The therapeutic intervention shifted adiponectin and A-FABP levels in GDM women towards concentrations of healthy pregnant controls. Adipokines were associated with visceral adiposity and glucose control. Conclusion: Women with GDM showed altered adipokine production even in the first trimester of pregnancy. Early therapeutic intervention not only improved glucose control but also normalized impaired adipokine production.

Background and Aim. Diabetes mellitus is a very common metabolic disease with a rising incidence. It is both a leading cause of chronic renal disease and one of the most serious comorbidities in renal transplant recipients. New-onset diabetes after renal transplantation (NODAT) is associated with poor graft function, higher rates of cardiovascular complications and a poor prognosis. The aim of this paper is to review current knowledge of NODAT including risk factors, diagnosis and management. Methods. A MEDLINE search was performed to retrieve both original and review articles addressing the epidemiology, risk factors, screening and management of NODAT. We also focused on microRNAs as potential biomarkers of NODAT. Results and Conclusion. Understanding the risk factors (both modifiable-e.g. obesity, viruses, and unmodifiable-e.g. age, genetics) may help reduce the incidence and impact of NODAT using pre-and post-transplant management. This can lead to better long-term graft function and general transplant success.
