Introdução: Transplante de pulmão (TxP) é o melhor tratamento para doenças pulmonares avançadas (DPA). Objetivo : Comparar variáveis clínico-funcionais de pacientes pré TxP em um hospital escola. Métodos : Estudo retrospectivo de prontuários de 33 pacientes. Formados quatro grupos: G1 - DPOC, G2 - bronquiec - tasia, G3 - fibrose pulmonar e G4 - outras DPA. Obtidos idade, sexo, capacidade vital forçada (CVF) e volume expiratório forçado no primeiro segundo (VEF 1 ); teste de caminhada de seis minutos, pressões respiratórias máximas e escores de qualidade de vida. Resultados: G1 apresentou CVF (2,00 ± 0,68 L) maior (p = 0,04) que G2 (1,23 ± 0,39 L), VEF 1 menor (p < 0,01) que G2 e G3 (0,67 ± 0,26 L vs 0,75 ± 0,25 L vs 1,51 ± 0,35 L) e PImáx (–58,94 ± 22,41cmH 2 O) menor (p = 0,04) que G4 (–90 ± 20,98 cmH 2 O). Conclusão : Pacientes com DPOC apresentaram pior função pulmonar e menor força muscular inspiratória comparado aos com outras DPA.
RESUMOIntrodução: O câncer de esôfago (CE) afeta mais de 450 mil pessoas no mundo por ano, enquanto o câncer de estômago é a segunda causa de morte por câncer no mundo. Objetivo: Avaliar e comparar o perfil sociodemográfico e clínico de pacientes com neoplasia de esôfago e estômago. Método: Foram avaliados 24 indivíduos internados em um hospital escola em período pré-operatório. Esses responderam a um questionário com dados sociodemográficos e passaram por uma avaliação clínica. Foi considerado o grau de independência, de acordo com a escala Performance Status. Foi aplicada estatística descritiva e inferencial com teste t de Student não pareado para comparar as variáveis sociodemográficas e físicas dos pacientes, sendo considerada diferença estatisticamente significativa, valores de p≤0,05. Resultados: Dos pacientes com neoplasia de esôfago, 100,00% (n=8) eram homens e, na neoplasia de estômago, 68,75% (n=11) eram homens. Quanto à escolaridade e renda, a maioria dos pacientes em ambas as doenças possuía ensino fundamental e renda de um salário mínimo. Na escala Performance Status, o escore mais elevado foi II para neoplasia de esôfago e 0 para neoplasia de estômago. Em relação às características físicas, verificou-se diferença significativa (p=0,01) na comparação do índice de massa corpórea entre homens (21,68±2,62 kg/m 2 ) e mulheres (27,91±5,97 kg/m 2 ). Já na força muscular, a maioria apresentou grau 5 para membros superiores e inferiores. Conclusão: O perfil desses pacientes se caracteriza pela prevalência de homens brancos e pardos, idosos, com baixa escolaridade, baixa renda e independentes para as atividades de vida diária. As for education and income, most patients with both types of cancer had attended primary education and would receive one minimum wage. Introdução: A imobilidade no leito de pacientes em unidades de terapia intensiva (UTI) favorece o maior tempo de permanência na unidade. Objetivos: Comparar a funcionalidade na admissão e na alta da UTI segundo a especialidade médica e correlacionar a funcionalidade na alta com o tempo de internação na UTI. Métodos: Estudo exploratório, longitudinal, realizado na UTI Geral de um hospital escola. Os pacientes foram divididos em grupos de acordo com a especialidade de internação: neurologia, pneumologia, gastroenterologia, politrauma e outros. O estado funcional prévio à internação foi obtido por meio da medida de independência funcional (MIF). Na alta foi aplicada a escala de mobilidade funcional em UTI (EMU). Resultados: Amostra de 174 pacientes, 53% do sexo masculino. O grupo politrauma apresentou idade significativamente menor que os outros grupos (p < 0,0001). O tempo médio de internação para os grupos neurologia e pneumologia apresentou correlação inversamente proporcional com p = 0 ,02 / r = -0,5 e p = 0,009 / r = -0,4 respectivamente. Não houve diferença significativa entre as médias da MIF na admissão (p = 0,11) e da EMU na alta (p = 0,24) entre as especialidades médicas em que os pacientes foram admitidos. Conclusão: Quanto maior o tempo de internação na UTI, menor a funcionalidade na alta em pacientes neurológicos e pneumopatas.
Introdução: A fraqueza muscular adquirida é uma complicação que acomete de 30% a 60% dos pacientes internados nas unidades de terapia intensiva (UTI). Estratificar a funcionalidade do paciente crítico na admissão é uma importante ferramenta para estabelecer prognósticos funcionais. Objetivo: Avaliar e comparar a mobilidade funcional de pacientes críticos internados em UTI na admissão e na alta da unidade. Materiais e Métodos: Estudo piloto realizado na UTI Geral e UTI Neurocirúrgica de um hospital escola, em São José do Rio Preto, SP. Foram selecionados prospectivamente pacientes adultos entre 18 e 60 anos, sedados, em ventilação mecânica invasiva. Foram identificados dados sociodemográficos, clínicos e hemodinâmicos à admissão na unidade e aplicada a escala de mobilidade funcional em UTI (EMU), na admissão e na alta da unidade. Foi aplicada estatística inferencial com teste de Wilcoxon. O nível de significância foi p≤0,05. Resultados: A amostra foi composta por 28 pacientes, maior prevalência (54%) do sexo masculino, com média de idade de 54,56±22,52 anos. A especialidade de internação mais prevalente foi a neurológica com 57%. Houve incremento significativo (p0,0001) na comparação da mobilidade funcional entre admissão (0,25±0,44) e alta (2,21±1,77) da UTI. Conclusão: Essa amostra de pacientes críticos, internados em unidade de terapia intensiva sedados e em ventilação mecânica invasiva, evoluiu com melhora estatística significativa da mobilidade funcional na comparação entre a admissão e a alta da unidade. Contudo, em termos clínicos, não houve evolução na funcionalidade desta amostra de pacientes da admissão para a alta da UTI.
Introduction: In cancer patients, the reduced food intake causes weight loss and promotes protein-calorie malnutrition. This results in loss of lean body mass, which affects both skeletal muscles and respiratory muscles. Objective: Evaluate and compare the respiratory muscle strength of patients with esophageal and stomach neoplasia during the preoperative period. Methods: This is a cross-sectional study carried out with 24 patients of both genders hospitalized in a teaching hospital. They underwent a physical therapy evaluation composed of anthropometric data and measurement of respiratory muscle strength through manovacuometry. Paired and unpaired t-tests were used to compare the values obtained with the predicted equations. Results: Regarding the disease prevalence, 66.66%(16) of the individuals had stomach neoplasm and 33.33%(8) esophageal neoplasm. Of the patients with esophageal neoplasm, 100% were men with a mean age of 63 ± 9.16 years. Of those with stomach neoplasm, 68.75% were men with a mean age of 69.36 ± 10.92 years. Female patients with stomach neoplasm had significantly higher BMI (p = 0.01) than male patients, and they were classified as overweight. Both neoplasms had significantly lower real values (p ≤ 0.05) than predicted values at the maximal expiratory pressure. Conclusion: Patients with esophageal and stomach neoplasms in the preoperative period present reduction in the expiratory muscle strength. There were no statistically significant differences, when we compared the maximum respiratory pressures between the two types of neoplasms investigated.
Parkinson's disease (PD) consists of a chronic and progressive disorder characterized by depletion of dopamine due to degeneration of nigral neurons leading to cognitive and motor losses. Patients with PD present an intolerance to physical effort, leading to impaired functional capacity (FC). The aim of this study was to analyze the efficacy of the six-minute walk test (6MWT) as a tool for assessing CF in the elderly with PD. A literature review was performed in the PubMed, SciELO and LILACS databases, from January 2005 to December 2015, through the cross-referencing of the physical therapy, exercise test and functional capacity descriptors with the descriptor Parkinson's disease. Five studies evaluating 203 elderly were identified, of which 166 were affected by PD. Among the elderly with PD, the mean age was 66.6 ± 2.8 years and the mean distance covered in the 6MWT ranged from 332 to 546 meters. The studies concluded that the elderly with PD presented reduction of CF. The analysis performed in this review suggests that the six-minute walk test seems not to be the most effective tool for assessing the functional capacity of elderly people with Parkinson's disease, as it may underestimate its functionality.
RESUMOOBJETIVOS: Avaliar os níveis álgicos de pacientes críticos, antes, durante e após uma intervenção de fisioterapia respiratória. MÉTODOS: Estudo piloto realizado na Unidade de Terapia Intensiva de um hospital escola. Foram selecionados pacientes adultos entre 18 e 60 anos, sedados e em ventilação mecânica invasiva. Foram levantados dados sociodemográficos, clínicos e hemodinâmicos e aplicada a escala Behavioural Pain Scale para avaliar a dor em pacientes incapazes de serem avaliados pelo auto relato. O protocolo de fisioterapia consistiu em manobras de vibrocompressão manual em ambos os hemitórax e aspiração intratraqueal. As variáveis hemodinâmicas e a escala de dor foram analisadas em três momentos: imediatamente antes, no quinto minuto de intervenção e imediatamente após a sessão de fisioterapia. Foi aplicada estatística com teste Shapiro-Wilk e análise de variâncias (ANOVA). O nível de significância foi p ≤ 0,05. RESULTADOS: A amostra foi composta por 22 pacientes, a maioria do sexo masculino, com média de idade de 55±23,8 anos. A doença mais frequente foi a pneumonia nosocomial. Não houve diferenças significativas nas variáveis hemodinâmicas e na avaliação da dor (p = 0,78) dos pacientes críticos em nenhum dos momentos avaliados. ABSTRACTAIMS: To evaluate pain levels of critically ill patients before, during, and after a chest physical therapy intervention. METHODS: Pilot study conducted at the intensive care unit (ICU) of a teaching hospital. Sedated and mechanically ventilated adult patients aged 18 to 60 years were selected. Sociodemographic, clinical, and hemodynamic data were analyzed, and the Behavioral Pain Scale (BPS) scale was used to assess pain in patients unable to be evaluated by self-report questionnaires. The physical therapy protocol consisted of manual vibrocompression maneuvers in both hemithoraces and of intratracheal aspiration. The hemodynamic variables and the BPS were analyzed in three periods: immediately before, in the fifth minute of intervention, and immediately after physical therapy. The Shapiro-Wilk test and ANOVA were used for the statistical analyses. The level of significance was p ≤ 0.05. RESULTS: The sample consisted of 22 patients, most of them male, aged 55±23.8 years. Nosocomial pneumonia the most prevalent pathology. There were no significant differences in hemodynamic variables and in pain assessment (p = 0.78) of the critically ill patients in any of the evaluated periods.
