Aim: To study the structural and functional parameters of echocardiography in healthy representatives of the urban Shor population, including men and women.Material and Methods. The study included 96 indigenous and 66 non-indigenous urbanized residents of Gornaya Shoria, matched by age and divided according to ethnic and gender characteristics into 4 groups: indigenous men (n=40), indigenous women (n=56), non-indigenous men (n=32), and non-indigenous women (n=34). All individuals underwent echocardiographic study. Body surface area-indexed left and right heart structural parameters, pulse wave Doppler data, data on propagation atrioventricular flow rates, and spectral Doppler tissue data were studied.Results. According to the results of anthropometry, men and women of Shor nationality had a lower body mass and body surface area compared with the corresponding values in men and women of non-indigenous nationality (p<0.0001 in all cases). However, the indices of the thickness of the interventricular septum, the posterior wall of the left ventricle, left ventricular mass and stroke volume, the diameter of the aorta, and the anterior-posterior dimension of the right ventricle were significantly higher in the group of indigenous women compared to non-indigenous ethnic groups. (p<0.01 in all cases). The values of the left ventricular end-diastolic volume, left atrial volume, pulmonary artery diameter were significantly higher in both men and women of indigenous nationality relative to the corresponding female and male comparison groups (p<0.01 in both cases). The tricuspid flow propagation velocity in male Shors was significantly higher than that in non-indigenous men (p<0.01). The Shor nationality was associated with a decrease in body surface area and increases in aortic diameter indices, pulmonary artery, left ventricular posterior wall, left ventricular myocardial mass index, pulmonary artery index, right atrial area index, and tricuspid flow propagation velocity; only a decrease in body surface area was associated with the female sex.Conclusion. In healthy representatives of the indigenous population of Gornaya Shoria, ethnic differences in the structural and functional parameters of echocardiography prevailed over gender-related differences.
Patients with chronic lung disease even without the development of PH exhibited a decreased ETFPV along with the increased size of the right heart. Another indicator of RV diastolic function is a tricuspid valve annular velocity ratio in early and late diastole; the e't/a't ratio was not considerably different in the groups, although its decline was observed in all the groups probably due to age-related changes. Thus, RV diastolic function should be comprehensively evaluated in patients with lung disease regardless of the presence of PH.

Изучение прогностическо-го значения показателей систолической и диастолической функции ПЖ у больных ХСН. Материалы и методы. Обследовано 129 больных ХСН I-IV ФК NYHA (средний возраст 63 [59][60][61][62][63][64][65][66][67][68][69][70][71][72] года, 74 мужчины и 55 женщин) на фоне ИБС и АГ. Период проспективного наблюдения составил 5 лет. Конечной точкой считали смерть от сердечно-сосудистой патологии, которая была констатирована у 39 больных -они составили группу неблагоприятного прогноза. В группу выживших вошли 90 боль-ных. Всем больным была выполнена ЭхоКГ, при которой были исследованы структурные показатели правых отделов сердца, скорость систолического движения кольца трикуспидального клапана TAPSE, данные потоков наполнения ПЖ, скорость распространения трикуспидального потока (СРТП) в комбинированном цветном М-режиме, данные спектральной ткане-вой допплерометрии кольца трикуспидального клапана (скорости систолической части спектра sт, диастолической части спектра е't и а't, их отношение е'т / а'т, а также отношение площади правого предсердия к скорости распространения три-куспидального потока ППП / СРТП), определено время изоволюмической релаксации ВИРПЖ, рассчитан индекс общей производительности ПЖ (индекс ТеиПЖ). Результаты. Группы были сопоставимы по возрасту, полу, антропометрическим данным, числу больных ХСН в границах каждого ФК, числу пациентов с фибрилляцией предсердий и легочной гипертензией. При построении моделей множественной линейной регрессии, куда были включены факторы, имеющие вес в однофакторной модели, независимым прогностическим значением обладали такие факторы, как увеличение ВИР ПЖ >90 мсек (ОШ=3,66, при ДИ от 1,07 до 12,55; р=0,035), увеличение индекса Теи ПЖ>0,8 (ОШ=7,07, ДИ 1,95-25,6; р=0,002), увеличение отно-шения ППП / СРТП >0,7 (ОШ=2,64, при ДИ от 1,13 до 6,2; р=0,023), а также снижение скорости раннего диастолического движения кольца трикуспидального клапана e't ≤11 см / сек (ОШ=2,47, при ДИ от 1,06 до 5,73; р=0,033). Заключение. ЭхоКГ оценка функции ПЖ имеет прогностическое значение у больных ХСН, о чем свидетельствуют выраженные взаимосвязи данных потоков наполнения ПЖ и спектральной тканевой допплерометрии кольца трикуспидального клапана с летально-стью. Проведение комплексного исследования структурных и функциональных показателей ПЖ целесообразно проводить для прогнозирования исхода заболевания. Это вполне объяс-нимо -именно поражение левых отделов сердца при ИБС ведет, в конечном счете, к их ремоделированию, дилатации и развитию сердечной недостаточности (СН). Тем не менее, в исследованиях последних лет показано, что одной из ведущих причин дисфункции ПЖ являются ИБС, ишемия и инфаркт миокарда DIASTOLIC DYSFUNCTION OF THE RIGHT VENTRICLE IN CORONARY HEART DISEASE pATIENTSArkhipov O. G. Results. The prevalence of diastolic dysfunction of RV in CHD patients was 29%. Most significant was the decline of velocity of tricuspid flow in group 2, being 22% lower than group 1 (p<0,0001). From the diastolic part of spectrum of tricuspid anulus there was significant decrease of the velocity of its early diastolic motion е' t in RVDD patients by 16% (Z=4,5; p<0,0001). Relation е' t /а' t significantly decreased in group 2 (p=0,001). Relation Е t /е' t increased significantly only in group 2, by 14% (p=0,001).The independent factors associated with RVDD were age (p=0,02), diabetes (p=0,021), clinical picture of angina III functional class (p=0,00011), decrease of LV ejection fraction (p<0,0001) and diastolic dysfunction of LV (LVDD) (p<0,0001 and p=0,0069 with different parameters). Conclusion. RVDD was found in 29% CHD patients, investigated outpatient. With RVDD there was association of age, diabetes, angina III class, decreased LV ejection fraction and LVDD. In RVDD assessment it is purposeful to apply complex approach with the parameters of intraventricular currents.
