Considerando singolarmente le indicazioni multiple di alcuni dei farmaci interessati, per i quali esistono studi registrativi differenti, a partire dai 32 Atti sopra citati si è giunti a un totale di 39 studi registrativi a (d'ora in avanti indicati anche come studi clinici di riferimento). Poiché in 7 di tali studi il dato mediano della sopravvivenza complessiva (overall survival, OS) non era riportato né si poteva ricostruire (operazione che, nella maggior parte dei casi in cui ciò è stato possibile, è consistita nell'interpolazione grafica delle curve di Kaplan fino al 50% dei pazienti sopravvissuti), l'analisi è stata condotta sui 32 restanti. La ricerca era volta a individuare il beneficio dei nuovi farmaci oncologici sulla base della OS (il gold standard per la valutazione clinica dei trattamenti, anche in campo oncologico b) conseguita. Da sottolineare che solo in 12 studi questo parametro costituiva l'endpoint primario e presentava quindi la massima robustezza da un punto di vista statistico. Come misura di tale parametro si è adottato in linea generale il valore mediano (anziché quello medio, sia pure in generale preferibile per le valutazioni economiche) perché più frequentemente reperibile/ricostruibile negli studi clinici selezionati. Anche la qualità della vita ha un'importanza rilevante, per un paziente oncologico, nella misura dei benefici. Nonostante ciò, essa è raramente rilevata negli studi clinici e di conseguenza non è stata qui presa in considerazione Nel corso dell'analisi, le 32 pubblicazioni sono state suddivise in due gruppi, a seconda che la differenza tra l'OS mediana conseguita con la nuova terapia e l'OS analoga della

Seguono poi il cancro allo stomaco (9,7%), quello al fegato (9,2%), quello al colon retto (8%) e quello al seno (6,1%) (1).Tutte queste stime non sono del tutto confrontabili con quelle pubblicate in periodi precedenti, a motivo della crescente disponibilità di dati e dei miglioramenti nei metodi di stima. Con questa premessa, è comunque indicativo il confronto tra il dato globale d'incidenza sopra riportato (12,7 milioni di casi, su un insieme di 27 principali tipologie di cancro considerate) e l'analogo stimato per il 1980: 6,4 milioni di casi (su 16 tipologie) (2). L'incidenza della malattia appare dunque in forte crescita. Tenendo conto di due processi: 1) l'aumento della popolazione mondiale (combinato allo spostamento new oncological drugs: analysis of survival gain Cancer incidence is growing worldwide, boosting substantial investment in research and development for ever better therapies. Starting from regulatory approvals for reimbursement issued by AIFA (Agenzia Italiana del Farmaco) and officially published between January 2010 and July 2013, 39 pivotal trials were identified on antineoplastic agents (21 related to new chemical entities and 18 to therapeutic extensions). In seven studies, OS data was not reported nor could be reconstructed; so they were not included in the analysis. Among the remaining 32 studies, the survival gain was statistically significant (p <0.05) in 12 studies. It ranged from 22 months (rituximab + fludarabine + cyclophosphamide vs fludarabine + cyclophosphamide in patients with chronic lymphocytic leukaemia) to 2.2 months (nab-paclitaxel vs conventional paclitaxel in women with breast cancer); the median value of the gain was 3.2 months and the mean value was 5.9 months (95% CI: 2.5-9.3). In the 20 studies where significance was not found in the median OS difference (though in most cases it was found if time-dependent endpoints were used as outcome measures), the survival gain ranged from 106 months (nilotinib vs imatinib in patients with chronic myeloid leukemia) to -2 (i.e. a survival loss) months (vandenatib vs placebo in patients with locally advanced or metastatic medullary thyroid cancer); the median value was 2.3 months and the mean value was 14.5 months (95% CI:1.2-27.8). Introduction: Primary immune thrombocytopenia is a rare autoimmune disease characterised by a decreased platelet count resulting in an increased risk of bleeding events and even life-threatening haemorrhages. Thrombopoietin receptor agonists (TPO-RAs) are the standard of care second-line therapy for adult patients with chronic immune thrombocytopenia. The first TPO-RAs approved and reimbursed in Italy, eltrombopag and romiplostim, while effective, pose some issues in terms of safety (e.g., hepatotoxicity) or general management (e.g., dietary restrictions). Avatrombopag, an effective and well-tolerated TPO-RA, was recently granted reimbursement. Methods: A 3-year (2023–2025) budget impact analysis (BIA) was conducted to estimate its impact on the Italian National Health Service (NHS). Two scenarios were compared, of which one represents the current situation, without avatrombopag, and the other provides for an increasing market share of avatrombopag (up to 26.6%). Results: BIA shows that the increase in the use of avatrombopag correlates with savings for NHS: in the first year, saving would be €1,300,564, increasing to €2,774,210 in the third year, for a total of €6,083,231 over the 3-year period. The sensitivity analysis confirmed these savings in the scenario with avatrombopag. Conclusions: Based on this BIA, the introduction and reimbursement of avatrombopag is an efficient and advantageous choice for the Italian NHS.
