Study design Cohort study.Objectives The objectives of this study were examination, observation and surgical interventions of pediatric patients with diastematomyelia, a rare congenital deformity of the spinal cord. Setting Spine surgery and neurosurgery clinic under ministry of health, the Russian Federation. Methods Twenty children (1-18 years of age) with diastematomyelia participated in this study. Diagnosis was based on subjects' medical history, results of clinical examination, spine radiography in anteroposterior and lateral views, and neurophysiological examinations. Septum resection surgery was done in seventeen subjects. Results All the subjects had skin manifestations along the midline of the back. Neurological symptoms were noted in 17 subjects: paraparesis in 11, urinary incontinence in 2, monoparesis in 4, and spasticity in 2 subjects. Three subjects did not show any neurological deficit. Orthopedic examination revealed spinal deformities of various degrees of severity. In 4 subjects, partial restorations of neurological functions were observed; and in 1 subject, bladder incontinence developed after the surgery. Conclusions Patients who have limb length discrepancy, congenital scoliosis, skin manifestations should be examined for the presence of diastematomyelia. Patients having neither neurological deficit nor orthopedic deterioration should be monitored, and in case of deterioration, surgical treatment should be reserved. We advocate for performing resection of the septum in patients with diastematomyelia prior to surgical correction of scoliosis or orthopedic correction of lower limb deformities.
Проведена предварительная оценка сравнительного анализа результатов этапной хирургической коррекции врожденной деформации грудного отдела позвоночника у детей с множественными пороками развития позвонков с применением спинальных и реберных эндокорректоров. Проведено обследование и лечение 13 пациентов с деформациями позвоночника на фоне множественных пороков развития. Пациенты были разделены на 2 группы: у пациентов первой группы (n=6) выполняли удаление аномальных позвонков, коррекцию локальной деформации и стабилизацию позвоночно-двигательного сегмента; пациентам второй группы (n=7) устанавливали реберные эндокорректоры и проводили этапную дистракцию каждые 6-8 месяцев. По данным рентгенологического обследования (рентгенография и МСКТ) оценивали влияние выполненных вмешательств на величину коррекции сколиотического и кифотического компонентов деформации, а также объем легких у пациентов обеих групп после выполнения первичного вмешательства. После хирургического лечения величина коррекции сколиотического компонента деформации у пациентов первой группы составила 63.7%, кифотического-18,7%; у пациентов второй группы величина коррекции сколиотической деформации составила 8.5%. Общий объем легких у пациентов первой и второй групп до проведения лечения составил 972.8 + 339.1 и 990.6 + 399 см 3 соответственно. Общий объем легких у пациентов первой и второй групп после проведения лечения составил 994.8 + 346.1 и 1484.2 + 407.1 см 3 соответственно. В ходе проведенного исследования установлено, что коррекция врожденной деформации грудного отдела позвоночника у детей с множественными аномалиями развития позвонков при помощи спинальных систем более эффективна по сравнению с реберными эндокорректорами. Проведение радикальной коррекции локальной деформации и стабилизации позвоночно-двигательных сегментов при помощи спинальных металлоконструкций не оказывает отрицательное воздействие на развитие органов грудной клетки. A clinical case of surgical and multimodal conservative treatment of an 11-year-old female patient with Hajdu – Cheney syndrome with multiple vertebral fractures in the thoracic and lumbar spine is presented. The main and significant manifestations of this syndrome are osteoporosis and acroosteolysis. After the injury, the child underwent correction of posttraumatic spinal deformity and stabilization of spinal motion segments by a placement of multi-anchor instrumentation through a posterior approach. The long-term postoperative period was characterized by the absence of bone block formation, development of distal junctional kyphosis and scoliotic deformity, which required additional surgical intervention in the amount of elongation of instrumentation and correction of the developed curvature. The paper describes in detail the clinical manifestations of the syndrome, the results of surgical treatment, and clinical and radiological characteristics.
Актуальность. Проблема медицинской помощи детям первых месяцев жизни при родовой травме пле-чевого сплетения является актуальной и дискутабельной. Согласно литературным данным, объем реко-мендуемых лечебно-диагностических мероприятий значительно варьирует, а их эффективность не всегда является доказанной. Цель. На обсуждение выносится разработанный нами протокол, предназначенный для врачей, оказываю-щих медицинскую помощь новорожденным и детям первых месяцев жизни с родовой посттравматической плексопатией. Материалы и методы. Проанализированы данные отечественной и зарубежной литературы, посвященной проблеме родового повреждения плечевого сплетения. Результаты. Выделены ключевые моменты диагностики и лечения пациентов данной группы. Разработан протокол их ведения. Заключение. Внедрение предложенного алгоритма в клиническую практику необходимо для формирова-ния четкого понимания врачами разных специальностей вопросов этиологии, патогенеза, особенностей естественного течения заболевания, дифференциальной диагностики и приоритетного лечения с целью повышения качества оказываемой помощи.Ключевые слова: родовое повреждение плечевого сплетения, парез Эрба, лечение, алгоритм.Родовая травма плечевого сплетения варьиру-ет по степени тяжести (от легкого растяжения до отрыва корешков от спинного мозга) и выражен-ности клинических проявлений (от малозамет-ных до плегии). Встречается с частотой 0,38-5,1 на 1000 новорожденных, преимущественно у до-ношенных. В большинстве случаев травмирует-ся верхний ствол плечевого сплетения (корешки С 5 -С 6 ), у трети пациентов -в комбинации со средним стволом (корешок С 7 ). Изолированная травма нижнего ствола (корешки С 8 -Т 1 ) отмеча-ется очень редко, приблизительно в 1 % случаев. Тотальный тип повреждения плечевого сплете-ния (С 5 -Т 1 ) наблюдают в среднем в 15 % случаев. В 70-80 % случаев наступает полное восстановле-ние в течение первых месяцев жизни, в 20-25 % сохраняются функционально значимые параличи. Вопрос о необходимости проведения нейрохирур-гического лечения рассматривают при отсутствии выздоровления к 3-4-му месяцу жизни. Отсут-ствие единого алгоритма раннего сопровож дения таких детей приводит к формированию грубых контрактур суставов и значительному ограниче-нию функции верхней конечности [1][2][3][4][5][6][7][8].Основными задачами оказания медицинской по-мощи детям с родовой травмой плечевого сплетения в течение первых месяцев жизни являются: про-филактика развития контрактур суставов верхней конечности; тщательное динамическое наблюдение с целью своевременного решения вопроса о необхо-димости проведения нейрохирургического лечения. Этиология1. Дистоция плечиков плода (крупный плод/ узкий таз).2. Патологическое положение плода в родах (затылочное, тазовое, запрокидывание ручки).3. Тракционно-ротационные акушерские ма-нипуляции (ручные, щипцы, вакуум-экстракция) [9][10][11][12][13][14].
Purpose— to study alterations in the bony structures in cases of congenital deformity of the thoracic spine combined with malsegmentation of lateral surfaces of vertebrae due to pathological processes during growth period of childern and to find correlation with above alterations.Material and Methods. The study included 30 patients with congenital deformity of thoracic spine combined with malsegmentation of lateral surfaces of vertebrae. age of patients ranged from 3 to 10 years (mean of 7 years). gender distribution — 19 female and 11 male patients. all patients underwent imaging examination including spine roentgenography in two standard views in prone position as well as computer tomography.Results. Magnitude of deformity scoliotic curve was 67° (45–88°) and of pathological thoracic lordosis — 23° (18–27°). Mean number of block vertebral motion segments included into the main deformity curve was 6 (3–8). Mean distance between the outline of lateral vertebral surface and lateral outline of projection of curve root of apical vertebra on convex side of deformity (aB) was 6 mm (4–10 mm). correlation analysis demonstrated that aB distance mostly depended on the number of vertebral motion segments included into the block along non-segmental area: pearson coefficient 0.67. Based on cT scans the authors observed no rotation component of bodies’ deformity along the non-segmental area.Conclusion. Children with congenital thoracic spine deformity along with malsegmentation of lateral surfaces of vertebrae feature pathological lordosis in affected zone. alterations in the bony structures result from asymmetric growth of one half of the vertebral body at the level of non-segmentation. asymmetry severity has a strong correlation dependency from the number of vertebral bodies included in the block along the non-segmental area.
Background. Currently, there are many studies on the evaluation of the results of surgical intervention and the comparative analysis of various surgical techniques for treating children with congenital spinal deformities. However, there is no consensus regarding the choice of surgical access to the abnormal vertebra that considers the duration of surgery, the volume of intraoperative blood loss, the degree of correction achieved during the intervention, the length of metal fixation, and the preservation of the result in the long-term observation period. Aim. The goal is to identify the advantages and disadvantages of the dorsal and combined approaches to the surgical treatment of preschool children with congenital deformities of the thoracic and lumbar spine against the background of an isolated violation of the vertebral formation. Materials and methods. A retrospective analysis of 56 patients under the age of five with congenital spinal deformities and a history of an isolated hemivertebra in the thoracic and lumbar regions who underwent one-stage hemivertebral extrusion from a dorsal approach (1st group: n = 30) or from a combined approach (2nd group: n = 26) was conducted. Results. All patients showed improvement in the sagittal and frontal profiles of the spine. However, during separation in the first group of patients, a progression of the kyphotic component of the deformity in the lumbar spine from 19 to 8 was noted, while the correction value of the curvature of the scoliotic component remained stable. Intraoperative blood loss in the first group of patients was less (234 mL) compared with that in the second group (319 mL), while the duration of surgery was longer (310 min and 185 min, respectively). On average, in the first group, a longer metal structure was used to correct the spinal deformity compared with the second group. Conclusions. The correction of the patients congenital spinal deformities with a single hemivertebra from a combined access approach allows a complete correction of the congenital curvature, the fixation of a smaller number of vertebrae, and the maintenance of a stable result in the long-term observation period compared with the dorsal approach. Isolated dorsal access to the hemivertebral body is characterized by less intraoperative blood loss compared with the combined approach, although the length of surgical intervention is increased.
