Актуальность. Идеология лечения пациентов с врожденными деформациями позвоночника на фоне изолированных пороков развития тел позвонков заключается в проведении коррекции и стабилизации деформации в максимально раннем возрасте с сохранением возможности дальнейшего роста. Однако для лечения детей с врожденной деформацией при множественных и комбинированных аномалиях развития позвонков и ребер применение данной тактики затруднительно. С этой целью все шире используют внепозвоночные металлоконструкции, однако публикации, отражающие результаты их применения, немногочисленны. Цель -анализ результатов лечения пациентов с множественными аномалиями развития позвоночника и грудной клетки после этапных оперативных вмешательств с использованием внепозвоночных металлоконструкций. Материалы и методы. За период с 2012 по 2016 г. проведено хирургическое лечение 20 пациентов в возрасте от 2 до 12 лет, которым выполнены 44 оперативных вмешательства. Средний возраст пациентов на момент вмешательства составил 6,9 года. В 13 наблюдениях деформация имела грудную локализацию, у 7 детей -грудопоясничную. Бифокальный реберно-реберный захват применили в 18 наблюдениях, реберно-позвоночные металлоконструкции -в 26 случаях. Результаты хирургического вмешательства оценивали по величине коррекции сколиотической дуги деформации непосредственно после оперативного вмешательства, также оценивали количество этапов проведенного оперативного лечения, частоту и характер осложнений. Результаты. Средняя величина сколиотической деформации до хирургического вмешательства составила 68,7° по Cobb. Количество этапных вмешательств варьировало от 3 до 7. Средняя величина коррекции деформации после хирургического вмешательства составила 15,1°. В отдаленном периоде после проведенных этапных операций у всех 20 пациентов величина деформации не превышала исходную. Частота осложнений в послеоперационном периоде составила 15,9 %. Заключение. Использование внепозвоночных металлоконструкций в лечении пациентов с врожденными сколиотическими деформациями на фоне комбинированных аномалий развития позвонков и одностороннего синостоза ребер позволяет уменьшить имеющуюся деформацию, приостановить темпы ее прогрессирования и увеличить объем гемиторакса.Ключевые слова: врожденный сколиоз, хирургическое лечение, синостоз ребер, дети.Aim. Treatment of patients with multiple congenital anomalies of the spine is a complex problem, demanding both correction and stabilization to be performed as early as possible without disturbing the growth of the spinal column.To achieve the latter, different instrumentation, such as rib-based distractors, may be useful. Little is known this treatment of congenital spine deformities. We aimed to investigate the results of treatment in our patients with the use of out-of-spine-based metal implants. В настоящее время отмечается, что врожденные пороки, вызы-вающие тяжелую и грубую деформацию позвоноч-ника, более чем в 50 % наблюдений локализуются в зоне грудопоясничного перехода и поясничном от-деле позвоночника. Врожденные деформации поз-воночника на фоне нарушения формирования тел позвонка характеризуются короткой и «крутой» основной дугой, вовлечением в нее небольшого ко-личества позвонков, включая аномально развитый, отсутствием или слабо выраженной компенсатор-ной диспластической противодугой, ригидностью имеющейся деформации [1, 3].Зона грудопоясничного перехода имеет свои биомеханические особенности, которые характе-ризуются отсутствием дополнительной стабиль-ности за счет реберного каркаса, мобильностью этого отдела позвоночника и наличием многоп-лоскостных движений в зоне грудопоясничного перехода. В этом отделе позвоночника при врож-денном нарушении формирования тел позвонков формируется сколиотическая и/или кифосколи-отическая деформация, нарушается фронталь-ный и сагиттальный профили позвоночника, что приводит к нарушению стабильности в этой зоне и изменению осевой нагрузки позвоночника. The present brief review focuses on the features of the Layer-by-Layer (LbL) synthesis of coatings containing hydroxyapatite nanoparticles and assesses their use in solving several biomedical problems. This work provides the state-of-art of this field. This method is based on the sequential chemical adsorption of reagents on the substrate surface that makes it possible to apply nanolayers of the specified composition on the surface of a wide range of substrates of complex shape, to control the thickness of the synthesized layers accurately at the nanometer level. It also enables the modification of surface characteristics, including roughness, hydrophilicity, and surface charge, and allows artificially constructed multilayers consisting of hybrid organic and inorganic substances to be obtained. The experimental material presented in the review demonstrates the effectiveness of LbL synthesis for creating new 3D scaffolds as bone substitutes, coatings on the surface of metal implants, and drug delivery systems. A promising direction for the development of LbL synthesis is the creation of methods that involve ion-substituted hydroxyapatites as reagents. Success in this area can pave the way for significant advances in biomedicine and open new opportunities for creating a new generation of structures that mimic the structural, compositional, and mechanical properties of the bone mineral phase.
Idiopathic scoliosis is a common orthopedic disease of unknown etiology in childhood that limits the patient’s activity for a lifetime. Treatment of idiopathic scoliosis includes both conservative and surgical methods and requires psychological consideration, which is important for the rehabilitation of patient with chronic disease. Systematic research on the psychological aspects of the treatment and rehabilitation of patients with idiopathic scoliosis in the form of analysis allows us to evaluate the medical and psychological approaches to the problem and to identify the factors contributing to the successful adaptation of the patient to chronic disease situation. Methodology. We selected the articles related to the psychological aspects of scoliosis and its treatment between 2017 and 2018. The primary selection included 16 publications, of which 2 were survey studies. Earlier foreign and domestic publications were also included in the analysis in order to compare the changes in treatment and rehabilitation approaches. Literature analysis. Idiopathic scoliosis was considered as a risk factor for psychological discomfort in the forms of stress, negative emotions, anxiety, distortion of the image of “I,” reduced self-esteem, and communication problems. These increase the risk of mental disorders, such as depression, suicidal tendencies, and psychological disadaptation. Discussion. Analysis allows us to highlight the most important topics in the studies of adolescent idiopathic scoliosis in recent years: topic on mental health/ill health in adolescent idiopathic scoliosis (AIS), theme of the psychological component of pain, topic on clinical psychological and social psychological factors that determine the course of the disease of the patients with AIS, quality of life of a child or adolescent with AIS, issues of psychological resources for adaptation to a chronic disease, topic on parents’ perception of their child’s illness, and topic on the psychological accompaniment of patients suffering from AIS. Conclusions. A modern approach to the study of idiopathic scoliosis suggests a point of view from a biopsychosocial model of the disease; therefore, it is necessary to consider various factors affecting the patient’s quality of life, including the psychological component of the disease. Psychological support of the treatment should focus on the formation of the patient’s active position in relation to his or her own life and active coping strategies with chronic disease.
Background. Currently, there are many studies on the evaluation of the results of surgical intervention and the comparative analysis of various surgical techniques for treating children with congenital spinal deformities. However, there is no consensus regarding the choice of surgical access to the abnormal vertebra that considers the duration of surgery, the volume of intraoperative blood loss, the degree of correction achieved during the intervention, the length of metal fixation, and the preservation of the result in the long-term observation period. Aim. The goal is to identify the advantages and disadvantages of the dorsal and combined approaches to the surgical treatment of preschool children with congenital deformities of the thoracic and lumbar spine against the background of an isolated violation of the vertebral formation. Materials and methods. A retrospective analysis of 56 patients under the age of five with congenital spinal deformities and a history of an isolated hemivertebra in the thoracic and lumbar regions who underwent one-stage hemivertebral extrusion from a dorsal approach (1st group: n = 30) or from a combined approach (2nd group: n = 26) was conducted. Results. All patients showed improvement in the sagittal and frontal profiles of the spine. However, during separation in the first group of patients, a progression of the kyphotic component of the deformity in the lumbar spine from 19 to 8 was noted, while the correction value of the curvature of the scoliotic component remained stable. Intraoperative blood loss in the first group of patients was less (234 mL) compared with that in the second group (319 mL), while the duration of surgery was longer (310 min and 185 min, respectively). On average, in the first group, a longer metal structure was used to correct the spinal deformity compared with the second group. Conclusions. The correction of the patients congenital spinal deformities with a single hemivertebra from a combined access approach allows a complete correction of the congenital curvature, the fixation of a smaller number of vertebrae, and the maintenance of a stable result in the long-term observation period compared with the dorsal approach. Isolated dorsal access to the hemivertebral body is characterized by less intraoperative blood loss compared with the combined approach, although the length of surgical intervention is increased.
