ÖZETAmaç: Bu çalışmada sekiz farklı ses sistemi kullanılarak frekans ve perturbation paremetrelerini ölçmek ve programları karşılaştırmak amaçlanmıştır. Gereç ve Yön-temler: Ses hastalıkları polikliniğimize Ocak 2011 ile Aralık 2011 tarihleri arasında başvuran 131 hasta ve 36 sağlıklı gönüllü çalışmamıza dahil edildi. Arşivde yer alan bu hastalara ait uzun [a] sesleri (2 ya da 3 s'lik); MDVP, Dr Speech Vocal Assesment, Praat, LingWAVES, Voxmetria, TF32, Chaos Data Analyzer ve SpeechTool programları kullanılarak analizleri yapıldı. Ayrıca LingWAVES yazılımının Vospector modülü ve Dr Speech Vocal Assesment programlarından yararlanılarak algısal analizleri yapıldı. Çalışmada kullandığımız sekiz analiz programı ile toplam 58 aynı ve farklı gruplarda yer alan parametrelerin kendi aralarındaki korelasyon değerleri ölçüldü. Bulgular: Çalışma ve kontrol grubunun ses kayıtlarının 8 ayrı programda yapılan analizleri ile ölçülen temel frekans (F0) da MDVP ile Praat ve Dr Speech programları arasında çok güçlü korelasyon (r>0,93), Frekans pertürbation parametrelerinde WoxMetria, LingWAVES, MDVP ve Praat programları arasında çok iyi korelasyon (r>0,79), Amplitüd perturbation paremetrelerinde MDVP ile Praat, TF32 ve VoxMetria progrmları arasında çok iyi korelasyon (r>83) izlendi. Çalışmamızda afonik, ileri derecede bozuk sesleri dahil edilmediği için kaotik analiz parametreler ile diğer parametreler arasında çok iyi uyum sağlanmadı. Sonuç: Ses analizlerinde kayıt için kullanılan yazılım ve donanımın farklı olması ölçülen sonuçların farklı çıkmasında önemli bir etkendir. Standart bir donanım ve yazılım kullanılarak kaydedilen ses üzerinde ölçüm yaparak bu sakınca ortadan kaldırılmış, programlar arasındaki farklılık ön plana çıkarılmıştır. Praat programının hasta ve kontrol bireylerdeki analiz sonuçlarının en sık kullanılan ses analiz programları ile olan yüksek korelasyon ilişkisi, programının ücretsiz oluşu ve betik dile sahip olması diğer programlara göre avantaj olarak görülmüştür. Amaç: Koanal atrezi nazal kavite ve nazofarinks arasındaki iletişimin gelişimindeki bozukluk olarak tanımlanmıştır. Her 5-8 bin canlı doğumda bir koanal atrezi tespit edilmektedir. Çift taraflı olması halinde hayatı tehdit eden bir durumdur. Tek taraflı koanal atrezi ise ileri yaşlara kadar farkedilmeyebilir. Koanal atrezilerin cerrahi tedavisi sonrası stent uygulaması tartışmalıdır. Yazımızda kliniğimizde yapılan 8 koanal atrezi hastasının stentsiz tedavisi sunulmuştur. Gereç ve Yöntemler: 2011 ile 2015 yılları arasında kliniğimizde gerçekleştirilen olgular retrospektif olarak tarandı. Tespit edilen 8 olgunun bilgilerine hasta dosyalarından ulaşıldı. Hastalara ait yaş, müdahale zamanı, atrezik plak içeriği, ek anomaliler ve atrezinin tarafı verileri not edildi. Bulgular: Koanal atrezili hastaların ikisi erkek altısı kızdı. Yaş ortalaması 5.1 yıl olarak tespit edildi. Atrezi 1 hastada bilateral, 7 hastada ise unilateral izlendi. Bilateral koanal atrezili hastanın solunum zorluğu varken tek taraflı atrezisi olanlarda genellikle tek taraflı burun akıntısı, burun tıkanıklığı şikayetleri mevcuttu. Hastaların dördünde sağ tarafta, üçünde sol tarafta atrezi tespit edildi. Atrezik plak içeriği 7 hastada membranoosseöz 1 hastada ise osseöz olarak tespit edildi. Bir hastada ensefalosel tespit edildi. Hastanede kalış süresi ortalama 4.1 gündü. Restenoz bir hastada görüldü. Sonuç: Koanal atrezi morbidite ve mortaliteye sebep olabilecek, zamanında müdahale edilmesi gereken bir patolojidir. Endoskopik sistemlerin gelişmesi ile birlikte endoskopik nazal yaklaşım tercih edilen bir yöntem haline gelmiştir. Stentsiz koanal atrezi cerrahisinin güvenli ve başarılı bir teknik olduğunu düşünmekteyiz.
Amaç: Trakeotomi, antik çağlara dayanan hayat kurtaran bir uygulamadır. Pediatrik trakeotominin endikasyonları son kırk yılda çok değişmiştir. 1970'li yıllarda, trakeotominin en sık endikasyonları akut inflamatuvar hava yolu obstrüksiyonudur. Son yıllarda H. İnfluenza ve C. Difteria karşı aşıların geliştirilmesi ve yoğun bakım olanaklarının artması sonucu enfeksiyöz nedenli trakeotomi sayıları azalmıştır. Trakeotomi işlemi yetişkinlere göre çocuklarda daha risklidir. Çalışmada, çocuk yoğun bakımda trakeotomi açılan hastaların endikasyon, komplikasyon ve tecrübelerimizin aktarılması amaçlanmıştır. Gereç ve Yöntemler: Çalışmada Kahramanmaraş Sütçü İmam Üniversitesi Tıp Fakültesinde 2009-2015 yılları arasında trakeotomi prosedürü uygulanan hastaların retrospektif analizi yapıldı. Hastaların trakeotomi endikasyon sebebi, ek hastalıkları, erken ve geç komplikasyonları ve hastanede kalış süreleri tarandı. Bulgular: Çalışmaya 27 (%64) erkek, 15 (%36) kadın olmak üzere 47 hasta dahil edildi. Trakeotomi endikasyonu % 97 oranında uzamış entübasyon olarak bulundu. Komplikasyon oranı erken dönemde % 23, geç dönemde % 14 olarak ortaya çıktı. Trakeotomi kaynaklı ölüm bizim serimizde gerçekleşmedi. Hastanede kalış süresi ortalaması 55 gündü. Sonuç: Trakeotomi endikasyonları son yıllarda enfeksiyöz sebeplerden uzamış entübasyon tarafına kaymıştır. Trakeotomi pediatrik yaşlarda riskli bir uygulama olmasına rağmen güvenli ve hayati bir işlemdir.
In this report, with regard to the diagnosis and treatment, we aimed to share our case in whom there was symptomatic multiple mucoceles in a minor salivary gland, which were localized in the nasopharynx in the left rosenmüller fossa and in the aryepiglottic fold in the hypopharynx, which is very rarely encountered. CaseA 45-year-old female patient presented to our clinic with the complaints of intermittent difficult breathing through the left side of the nose and the feeling of food getting stuck in the throat during swallowing, which had been presented for one year. The physical examination performed on the patient revealed normal findings, and on the endoscopic examination of the nasopharynx and the larynx, a cystic mass was observed in the nasopharynx filling the left rosenmüller fossa and in the left aryepiglottic fold neighboring the epiglottis (Figure 1, 2). The laboratory findings were normal and the patient's past medical history was unremarkable.Under general anaesthesia, using the endoscopic method, the cystic mass filling the left rosenmüller fossa in the nasopharynx was marsupialized, and in laryngoscopy, the cystic mass in the left aryepiglottic fold neighboring the epiglottis was totoally excised under microscopy. The pathology of the excision material was reported as a salivary gland mucocele (Figure 3). No recurrence was observed on the follow-ups performed for two years.
