RESUMO:O acidente vascular cerebral isquêmico (AVCI) é um déficit neurológico focal persistente que se dá pela obstrução proximal de uma artéria por um trombo, êmbolo ou compressão por tumor. Existem vários fatores de risco que estão associados a esse evento. Entretanto, ainda há poucos estudos nacionais que abordem essa temática. Dessa forma, esse estudo aborda os principais fatores de risco modificáveis e não modificáveis dos AVCI. Para tanto, esse trabalho analisou vários estudos transversais, estudos prospectivos e coortes realizadas em vários países, sobretudo nos últimos seis anos. As principais revistas consultadas para esse estudo foram The New England Journal of Medicine, Springer, Lancet, Stroke e Circulation -American Heart Association (AHA), Journal of the American College of Cardiology. Todas essas revistas estão indexadas à Pubmed Central®, base de dados utilizada nesse trabalho. Durante a pesquisa, foram utilizados os seguintes termos: "ischemic stroke AND modifiable risk factors", "ischemic stroke AND nonmodifiable risk factors", "ischemic stroke AND hypertension", "ischemic stroke AND atrial fibrillation", "ischemic stroke AND diabetes mellitus", "ischemic stroke AND dyslipidemia", "ischemic stroke AND obesity" and "ischemic stroke AND smoking". Observou-se que os principais fatores não modificáveis são: idade, sexo, raça, localização geográfica e hereditariedade. Já os principais fatores modificáveis são: hipertensão, fibrilação atrial, diabetes melito, dislipidemia, obesidade e o tabagismo. RESUMO:O ABCDE é um mnemônico que padroniza o atendimento inicial ao politraumatizado e define prioridades na abordagem ao trauma. Entretanto, com a aplicação prática dos protocolos, essa sistematização passa por modificações. Esse estudo sistematiza as principais condutas pré-hospitalares e hospitalares que lograram êxito no atendimento ao traumatizado. Para tanto, foram analisadas publicações recentes de revistas nacionais e internacionais. Este trabalho observou que a obstrução da via aérea é a causa de mortalidade mais rápida no politraumatizado, sendo seguida por distúrbios na ventilação e respiração e hemorragia. Por isso, a avaliação das vias aéreas com controle cervical deve ser a primeira conduta na abordagem do trauma. Com essa sistematização definida pelo ABCDE, é possível direcionar o atendimento ao politraumatizado e, com efeito, reduzir mortalidade. ABSTRACT:The ABCDE is a mnemonic that standardizes initial care for the traumatized and defines an priorities in the approach to trauma. However, with a practical application of the protocols, this systematization undergoes modifications. This study systematize the main prehospital and hospital conduits that were successful in the assessment to the traumatized. For this, recent publications of national and international journals were analyzed. This study observed that an obstruction of the airway is faster cause of faster mortality in the polytraumatized, followed by disturbances in ventilation and breathing and hemorrhage. Therefore, an evaluation of the airways with cervical control should be the first step in the approach to trauma. With this systematization defined by the ABCDE, it is possible to direct care to polytraumatized and, therefore, reduce mortality.
Modelo do estudo: Estudo de prevalência. Objetivo do estudo: Descrever o perfil clínico e epidemiológico dos portadores de Sífilis Congênita entre Janeiro de 2010 a Junho de 2016 em uma capital do Norte brasileiro. Metodologia: Foi realizado um levantamento das fichas de notificação para infecção por sífilis congênita da Vigilância Epidemiológica em Rio Branco-Acre no período de Janeiro 2010 a Junho de 2016. Os dados foram analisados por meio do Statistical Package for Social Sciences (SPSS). Resultados: Foram notificados 189 casos de sífilis congênita precoce. Apenas 36,51% (n=69) receberam diagnóstico de sífilis gestacional antes do parto. Os casos evoluíram com 74,6% de nascidos vivos, 15,3% de natimortos, 6,3% de abortos e 2,1% de óbitos por sífilis congênita. A relação entre o Venereal Disease Research Laboratory (VDRL) materno e o desfecho dos casos sugere que filhos provenientes de mães com títulos ≥1:16 representam 82,2% das evoluções deletérias associadas (p=0,045). Conclusão: A sífilis na gestação é uma patologia ainda subdiagnosticada durante a gravidez e, nesse estudo, foi mais frequente em filhos de mães no auge da menacme, de baixa escolaridade e moradoras da zona urbana. Apesar das medidas preventivas já implantadas, a sífilis congênita continua sendo um problema de saúde pública nesta capital do Norte brasileiro e deve continuar como alvo de estudos que gerem novas estratégias de prevenção.
Os Acidentes vasculares cerebrais isquêmicos (AVCI) ocorrem pela obstrução de uma artéria cerebral por um trombo, êmbolo ou compressão mecânica por tecidos circundantes. Os AVCI respondem por 80-87% dos Acidentes Vasculares Cerebrais (AVC). Essa etiologia é a segunda maior causa de morte no mundo e de óbitos naturais em homens e mulheres no Brasil. Entretanto, existem poucos estudos sobre o perfil dos AVCI em cidades interioranas do país como Uberlândia. Nesse sentido, o objetivo desse trabalho é descrever o perfil de AVCI em um Hospital de Uberlândia entre 2011-2015 por sexo e faixa etária. Trata-se de um estudo transversal. Foi considerado apenas esse tipo de AVC, excluindo os eventos por causas hemorrágicas. Esse estudo observou que o AVCI se comporta de maneira diferente de acordo com o sexo e faixa etária. Quanto ao sexo, esse evento vascular foi levemente mais prevalente em mulheres. Quanto à faixa etária, até os 70 anos, o AVCI predominou em homens. Após essa idade, as mulheres passaram a liderar. Esse padrão foi semelhante à prevalência por faixa etária dos seguintes fatores de risco modificáveis: hipertensão, fibrilação atrial, diabetes mellitus tipo 2 e tabagismo. O conhecimento dessa epidemiologia nos oferece uma maior proximidade ao real contexto do AVCI em nosso meio, refletindo a necessidade de uma atenção individualizada aos grupos mais acometidos mostrados nesse estudo.

It is defined as axillary temperature ≥ 37.8 ° C on several occasions, lasting ≥ three weeks, in the absence of diagnosis after three days of hospital investigation or three outpatient visits. The main etiologies are: infectious, neoplastic, and rheumatic. The diagnosis is based on the detailed clinical history and physical examination of these patients, in order to direct the specific complementary tests to be performed in each case. Empirical therapy is not recommended (with few exceptions) in patients with prolonged fever, as it may disguise and delay the diagnosis and conduct to treat the specific etiology. The prognosis encompasses mortality of 12% -35%, varying according to the underlying etiology. In this sense, the objective of this study is to review the main topics about fever of undetermined origin, bringing historical and scientific aspects, national and international.

RESUMO: A Liga Acadêmica de Medicina de Urgência eEmergência (LAMURGEM) é uma entidade sem fins lucrativos que estuda as emergências clínicas e cirúrgicas e desenvolve atividades nessa área. No entanto, não existem estudos que abordam o perfil dos ingressantes nessa liga e os fatores que levam os estudantes a realizarem essa escolha. Esse estudo retrata o padrão dos inscritos e ingressantes na LAMURGEM, comparando os resultados obtidos com os achados da literatura. Trata-se de um estudo transversal, realizado a partir da análise dos trinta e oito estudantes inscritos no processo seletivo da LAMURGEM em 2016. Foi analisado o perfil dos estudantes inscritos e ingressantes de acordo com o período cursado. 100% dos estudantes inscritos estavam entre o segundo e o sexto período do curso de medicina, e o número de inscritos no processo seletivo diminuiu ao longo dos períodos. Esse trabalho mostra que estudantes dos primeiros períodos são os ingressantes majoritários na LAMURGEM, confirmando os resultados abordados nas literaturas.Descritores: Estudantes de medicina/classificação; Medicina de emergência/educação; Educação médica; Educação de graduação em medicina/tendências. ABSTRACT:The Urgency and Emergency Medicine Academic League (LAMURGEM) is a nonprofit entity that studies surgical and clinical emergencies and develops activities in this area. However, there aren't any studies that evaluate the profile of entering students in this league and the factors that lead them to this choice. This investigation depicts the pattern of inscribed and entering students in LAMURGEM, comparing the results with other literature findings. This is a retrospective observational study involving the analysis of thirty-eight students inscribed in the selection test for entering in LAMURGEM in 2016. The profile of inscribed and entering students was analyzed according to the graduation period. One hundred percent (100%) of the inscribed students were between the second and sixth semester of medical course, and the number of inscribed students in the selection process progressively reduced over the semesters. This paper shows that students of the initial periods are the majority of entering students in LAMURGEM, confirming the results discussed in the literature.
RESUMO:A tosse é um reflexo de defesa da via aérea em condições fisiológicas. Entretanto, ela compõe o quadro clínico de diversas doenças pulmonares, cardíacas e gastrintestinais. A persistência desse sintoma afeta a qualidade de vida do paciente e acomete 3 a 40% da população geral. Embora possua uma prevalência alta, a fisiopatologia desse sintoma ainda não é bem esclarecida. Nesse sentido, anualmente, são publicados novos achados sobre a fisiopatologia desse sintoma. Assim, o objetivo desse trabalho é realizar uma revisão de literatura sobre a fisiopatologia da tosse. Essa revisão utilizou quarenta publicações presentes nas seguintes bases de dados: Scholar Google, SciELO, Pubmed Central® (PMC), National Center for Biotechnology Information (NCBI) e Medline. A fisiopatologia da tosse está intimamente ligada à estimulação química das fibras-C e à ativação por estímulos mecânicos dos receptores de adaptação rápida (RARs) e dos receptores de estiramento de adaptação lenta (SARs). O conhecimento desses mecanismos fisiopatológicos é importante para o manejo clínico de pacientes com esse sintoma e contribui para o entendimento do mecanismo de ação de antitussígenos que agem sobre esses receptores.Descritores: Tosse/fisiopatologia; Tosse/diagnóstico; Reflexo; Antitussígenos. ABSTRACT:Cough is a reflex of airway defense under physiological conditions. However, it composes the clinical condition of several pulmonary, cardiac and gastrointestinal deseases. The persistence of the symptom affects the welfare of the patient and affects 3 to 40% of the general population. Although it has an important prevalence, the pathophysiology of the symptom is still not clear. In this sense, new symptom findings are published annually. Thus, the objective of the study is to perform a review of the pathophysiology of cough. This review used forty recent publications in the following databases: Scholar Google, SciELO, Pubmed Central® (PMC), National Center for Biotechnology Information (NCBI) and Medline. The pathophysiology of cough is closely linked to the chemical stimulation of C-fibers and to activation by mechanical stimuli of the rapid adaptation receptors (RARs) and the slow adaptating stretch receptors (SARs). The knowledge of these pathophysiological mechanisms is important for the clinical management of patients with this symptom and contributes to the understanding of the mechanism of action of antitussives that act on these receptors.
