BackgroundStudies about work ability have grown in importance owing to the worldwide aging of active populations. Research has shown that measuring work ability has a predictive value in cases of long-term sickness absence and early retirement. Our goal was to analyze the work ability and associated factors of civil servants from a higher education institution in Brazil. The participants in this cross-sectional study were 600 technical-administrative workers at a public university. Work ability was measured using the work ability index.ResultsThe participants were as follows: 51.8 % male; mean age of 45 years (SD = ±11); married or in a stable union (61.5 %); holding a graduate degree (56.7 %); having only one job (83.3 %), working 40 h a week or less (78.6 %); not working evenings (79.8 %); and having direct contact with the public (58.3 %). The prevalence of reduced work ability was 13.9 %. The following factors were found to be associated with reduced work ability: age 50 years old or above (PR = 2.58; 95 % CI 1.25–5.09); female (PR = 2.77; 95 % CI 1.25–3.60); education up to secondary school (PR = 2.37; 95 % CI 1.13–3.59); overall poor self-assessed health (PR = 2.96; 95 % CI 1.32–3.93); signs and symptoms of depression (PR = 4.86; 95 % CI 2.23–6.55); sedentariness (PR = 3.00; 95 % CI 1.38–4.68) and poor social support at work (PR = 4.01; 95 % CI 1.66–4.37).ConclusionsMost of the participants showed good work ability, but some subjects had reduced work ability. This study makes a contribution to expanding the discussion about the factors associated with work ability toward proposing actions for maintaining that ability or helping recovery in the case of diminished ability. Such actions can help reduce work absenteeism and early retirement, both of which have a social and economic impact in Brazil. Studying the determinants of work ability and recommendations to address those determinants will help efforts to improve the quality of life of individuals, both at work and personally, and promote healthy aging.
OBJECTIVE:To estimate the prevalence of falls in elderly individuals and to analyze associated factors. METHODS:Cross-sectional study with 420 elderly subjects (aged 60 years or older) living in the city of Juiz de Fora (Southeastern Brazil) in 2010. A household survey was conducted and the occurrence of falls in the 12 previous months was described. For the analysis of factors associated with the outcome, a theoretical determination model with three hierarchical blocks was built. The variables were adjusted among each other within each block; those with level of signifi cance ≤ 0.20 were included in the Poisson regression model and adjusted to the immediately higher level, with 5% signifi cance level. RESULTS:The prevalence of falls among the elderly was 32.1% (95%CI: 27.7; 36.9). Among those who experienced falls, 53% had a single fall and 19% had fractures as a consequence. Most of the falls (59%) occurred at the elderly person's home. The occurrence of falls was associated with old age, female sex, need of help for locomotion and self-reported diagnosis of osteoporosis. CONCLUSIONS:Falls are frequent among the elderly. Knowledge of the factors associated with the occurrence of this event can aid the development of prevention strategies and adequate health services.
RESUMO Objetivo Avaliar a prevalência de depressão e os fatores associados em mulheres de 20 a 59 anos de áreas cobertas pela Estratégia de Saúde da Família de município da Zona da Mata Mineira. Métodos Trata-se de um estudo transversal, com mulheres de 20 a 59 anos cadastradas em duas Unidades de Saúde da Família, que utilizou um questionário contendo variáveis sociodemográficas, apoio social, autoavaliação de estado de saúde, estilo de vida, morbidade e saúde da mulher. O desfecho depressão foi avaliado segundo o Patients Health Questionnaire-9 (PHQ-9). Resultados Das 1.958 mulheres incluídas nesta análise, 28,5% encontram-se na faixa etária entre 30 e 39 anos; 15,4% não concluíram o ensino elementar; 54,5% não trabalham ou nunca trabalharam; 44,2% declararam não ser da raça branca. Fatores associados à ocorrência de depressão na população estudada: possuir baixa escolaridade, trabalhar atualmente e ter doença mental prévia. Como fatores de proteção observaram-se: ser casada ou viver com companheiro, realizar atividades físicas regularmente e relatar autoavaliação positiva de saúde. Conclusão Os resultados deste estudo revelam prevalência de depressão de 19,7% nas mulheres de 20 a 59 anos de áreas cobertas pela Estratégia de Saúde da Família, apontando para a necessidade de um cuidado especial na atenção primária à saúde às mulheres com baixa escolaridade, que trabalham, apresentam doença mental e não praticam exercícios físicos, de modo que se possa reduzir o sofrimento e promover a saúde. Ressalta-se a lacuna na utilização de instrumentos de rastreamento dos casos de depressão na atenção primária.
Objective:To analyze the clinical, pathological, and sociodemographic aspects between triple-negative breast cancer (TNBC) and non-TNBC in a Brazilian cohort and identify potential prognostic factors.Methods:This hospital-based retrospective cohort study included 447 women with breast cancer treated at referral centers in Southeastern Brazil. Overall and disease-free survival were compared; prognostic factors were evaluated.Results:Triple-negative breast cancer corresponded to 19.5% of breast cancer diagnosis and was more prevalent among nonwhite and less educated women. The patients with TNBC tended to present with stage III cancer, high p53 expression, lymphocytic infiltration, and multifocality and treated with radical surgery and chemotherapy. The 5-year overall and disease-free survival were 62.1% and 57.5% for TNBC and 80.8% and 75.3% for non-TNBC, respectively (P < .001). The TNBC recurrence was associated with multicentricity, whereas lymph node involvement increased the risk of both recurrence and death. Non-TNBC worse clinical course was associated with nonwhite ethnicity, lower education level, lymph node involvement, and advanced stage.Conclusions:Triple-negative breast cancer exhibited a more aggressive behavior, earlier and more frequent recurrence, and worse survival compared with non-TNBC. While biological and social variables were associated with poorer prognosis in non-TNBC, only lymph node involvement and multicentricity were correlated with worse clinical outcomes in TNBC.
Artigo original Resumo OBJETIVOS: avaliar a evolução da adequação do processo de atendimento às gestantes usuárias do Sistema Único de Saúde (SUS) e consolidar metodologia para monitoramento da assistência pré-natal. MÉTODOS: estudo de séries temporais múltiplas, com auditoria em cartões de gestantes que realizaram pré-natal em município do Sudeste brasileiro (Juiz de Fora, Minas Gerais) nos semestres iniciais de 2002 e 2004 (370 e 1.200 cartões, respectivamente) e utilizaram o SUS no atendimento ao parto a termo (p ≤ 0,05). Obedeceu-se a uma sequência em três níveis complementares: utilização do pré-natal (início e número de atendimentos) no nível 1; utilização do pré-natal e procedimentos clínicoobstétricos obrigatórios em uma consulta pré-natal [aferições de pressão arterial (PA), peso, altura uterina (AU), idade gestacional (IG), batimentos cardiofetais (BCFs) e apresentação fetal] no nível 2; e utilização, procedimentos clínico-obstétricos obrigatórios e exames laboratoriais básicos, segundo o Programa de Humanização no Pré-natal e Nascimento/PHPN [tipagem ABO/Rh, hemoglobina/hematócrito (Hb/Htc), VDRL, glicemia e exame comum de urina] no nível 3. RESULTADOS: confirmou-se a alta cobertura pré-natal (99%), aumento da média de consultas/gestante (6,4 versus 7,2%) e decréscimo da idade gestacional na primeira consulta (17,4 versus 15,7 semanas). Resumo: O objetivo do presente estudo foi estimar a prevalência e identificar fatores associados à não realização do exame citopatológico do colo do útero entre mulheres que frequentaram o pré-natal. Trata-se de estudo transversal, de base populacional, com 308 mulheres de um município do interior de Minas Gerais, Brasil. Utilizou-se amostragem complexa com estratificação e conglomeração. Para análise estatística, empregou-se o teste do qui-quadrado e o modelo de regressão logística com as variáveis que apresentaram valor de p ≤ 0,05 na análise bivariada. A prevalência de não submissão ao exame foi de 21,3%. Entre as mulheres com 25 anos ou mais, a prevalência foi de 15,1%. A não adesão foi mais frequente entre as mulheres jovens, solteiras e com baixa escolaridade. A escolaridade se manteve associada ao desfecho (OR = 0,41), indicando que mulheres com mais anos de estudo têm maiores chances de realizarem o exame. O contato com o serviço de saúde para realização do pré-natal não foi determinante para garantir o acesso ao exame, indicando perda de oportunidades onde o rastreamento é oportunístico.
