BACKGROUND AND OBJECTIVES: Transcranial Direct Current Stimulation is a neuromodulation method to treat neuropsychiatric and other diseases. This study aimed at evaluating the effect of this technique on pain and quality of life of fibromyalgia patients, in addition to studying brain images by Brain Perfusion Scintigraphy before and after procedures. METHODS: Prospective, double-blind, randomized, controlled study with parallel arms evaluated clinically and by CT scan with single photon emission (Brain Spect). Sample was made up of 20 female fibromyalgia patients divided in two groups of 10. Parameters evaluated were quality of life by the pain Visual Analog Scale, Quality of Life Questionnaire SF-36, Fibromyalgia Impact Questionnaire and pain level measurement on tender points with Fischer's Algiometer. The effective group has received 1.0mA anodal stimulation for 20 minutes in M1 to the left, cathode positioned on the right supraorbital region once a week in a total of 10 consecutive weeks. The Sham group received no electric impulses. RESULTS: There has been decrease in the Fibromyalgia Impact Questionnaire (55.36 to 35.42; p=0.0059) and the Visual Analog Scale (6.05 to 3.60; p=0.0316) scores. There has been statistically significant difference (p<0.05) in trigger points measurement in the effective group. RESUMO -A impressão basilar ocupa lugar de destaque entre as malformações occipitocervicais. A forma primária decorre de erro embriogênico, enquanto a forma secundária é causada por afecções ósseas diversas, como doença de Paget, displasia fibrosa, osteoporose e osteomalacia. A descrição original da impressão basilar foi feita por Ackermann 1 , em 1790, em estudos sobre o cretinismo. Virchow 32 , em 1857, denominou-a de platibasia e rejeitou a relação entre impressão basilar e cretinismo, como fora admitida por Ackermann 1 . Virchow 33 , em 1876, denominou a malformação de impressão basilar, tendo descrito detalhadamente as modificações ósseas e sendo o primeiro autor a admitir a origem congênita em muitos de seus casos. Homén 23 , em 1901, foi o primeiro autor a correlacionar os dados clínicos com os achados anátomo-patológicos. A necropsia de seu paciente evidenciou achatamento do bulbo e cerebelo, bem como presença de cavidade cística na medula espinhal. Explicou a patogenia dos sintomas clínicos através da compressão das estruturas comprometidas na invaginação da apófise odontóide. Quanto à clínica, a impressão basilar sintomática pode apresentar síndromes clínicas diversas, isoladas ou combinadas, tais como síndrome dos nervos intracranianos, síndromes sensitivas superficial e profunda, cerebelar, de hipertensão intracraniana, bulbar, medular, de insuficiência vertebrobasi 1 ar 2 , 6 , 2 4 ' 2 5 .
-A 29-year-old woman with acute lancinating headache, throbbed nuchal pain and subacute paraparesis underwent brain MRI in supine position that depicted: the absence of the cisterna magna, filled by non herniated cerebellar tonsils and compression of the brain stem and cisternae of the posterior fossa, which are aspects of the impacted cisterna magna without syringomyelia and without hydrocephalus. During eight days, pain was constant and resistant to drug treatment. Osteodural-neural decompression of the posterior fossa, performed with the patient in sitting position, revealed: compression of the brainstem, fourth ventricle and foramen of Magendie by herniated cerebellar tonsils, which were aspirated. Immediately after surgery, the headache and nuchal pain remmited. MRI depicted the large created cisterna magna and also that the cerebellar tonsils did not compress the fourth ventricle, the foramen of Magendie and the brainstem, besides the enlargement of posterior fossa cisternae. Four months after surgery, headache, nuchal pain and paraparesis had disappeared but hyperactive patellar and Achilles reflexes remained.KEY WORDS: headache, nuchal pain, craniovertebral malformation, tight cisterna magna, impacted cisterna magna, Chiari malformation, posterior fossa decompression, paraparesis, herniated tonsils.Cisterna magna impactada sem siringomielia associada a cefaléia lancinante, dor na nuca terebrante e paraparesia tratadas com descompressão da fossa posterior RESUMO -Uma paciente de 29 anos de idade com quadro agudo de cefaléia lancinante, dor terebrante na nuca e paraparesia subaguda foi submetida a RM do encéfalo, em posição supina, que revelou: ausên-cia da cisterna magna, preenchida por tonsilas cerebelares não herniadas e compressão do tronco encefá-lico e das cisternas da fossa posterior, compatíveis com o diagnóstico de cisterna magna impactada sem siringomielia e sem hidrocefalia. Por oito dias a dor foi constante e resistente aos analgésicos. Com a paciente em posição sentada, foi realizada descompressão osteodural-neural da fossa posterior associada a aspiração das tonsilas cerebelares. Os achados perioperatórios foram caracterizados por herniação das tonsilas cerebelares que comprimiam o tronco cerebral, o quarto ventrículo e o forame de Magendie. No pós-operatório imediato houve remissão da cefaléia e da dor na nuca. A RM evidenciou a cisterna magna recém-criada, alargamento do quarto ventrículo e das cisternas do tronco encefálico. Quatro meses depois, a paciente continuava sem cefaléia, sem dor na nuca e sem paraparesia. Entretanto, permaneceu a hiperatividade dos reflexos patelares e aquileus.
From the neurological point of view, there was no difference on the evolution between the group treated exclusively with D-P and the one treated with Zn. Analysis of MRI scans with longer intervals after the beginning of treatment depicted a trend for neuroimaging worsening, without neurological correspondence, among patients treated with Zn. Neuroimaging pattern of evolution was more favorable for the group that received exclusively D-P.
RESUMO -Os autores relatam os achados clínicos e angiográficos de dois casos de espasmo hemifacial familiar. Esta é a quinta descrição sobre esse tema na literatura e apresenta mãe e filha com idades de 76 e 51 anos respectivamente, nas quais o lado esquerdo foi o acometido. Exames de angiorressonância realizados nas pacientes revelaram dolicobasilar com origem lateralizada à esquerda para ambos os casos. Também demonstraram artéria cerebelar póstero-inferior muito desenvolvida e irregularidades murais nas artérias vertebrais e basilar sugestivas de arteriosclerose na mãe e vasos intracranianos levemente alongados na filha. Dados de revisão da literatura e sobre a etiologia do espasmo hemifacial são enfocados.PALAVRAS-CHAVE: espasmo hemifacial familiar, angiorressonância, nervo facial, compressão nervosa. Familial hemifacial spasm: report of two casesABSTRACT-The authors report the clinical and angiographical findings of two cases of familial hemifacial spasm. This is the fifth description in the literature and presents mother and daughter at the ages of 76 and 51 respectively, in whom the left side was affected. They underwent exams of angioresonance that showed dolichobasilar with left side origin in both patients. The exams also demonstrated postero-inferior cerebellar artery very developed and irregularities in the walls of the vertebral and basilar arteries suggestive of arteriosclerosis in the mother and slightly elongated intracranial vessels in the daughter. Em sua forma típica de apresentação surgem contrações involuntárias intermitentes da metade inferior do músculo orbicular dos olhos, em um dos lados da face, que gradualmente pioram em frequência e intensidade e, posteriormente, passam a acometer a porção superior do referido músculo. Após meses ou anos, os espasmos podem estender-se para outros músculos homolaterais inervados pelo facial. Os músculos platisma e frontal são acometidos somente em 15% dos casos 1,2 . Quando o músculo estapédio é acometido, os pacientes referem tinidos no ouvido homolateral ao EH, sincrônicos com as contrações musculares 3,2,4 . Nos casos mais graves, discreta paresia hemifacial ipsilateral pode ser observada [4][5][6][7][8] . The prime objective in the surgical treatment of basilar impression (BI), Chiari malformation (CM), and/or syringomyelia (SM) is based on restoration of the normal cerebrospinal fluid (CSF) dynamics at the craniovertebral junction and creation of a large artificial cisterna magna, avoiding the caudal migration of the hindbrain. It is observed that a large craniectomy might facilitate an upward migration of the posterior fossa structures. There are many surgical techniques to decompress the posterior fossa; however, a gold standard approach remains unclear. The authors present the results of 192 cases of BI, CM, and SM treated between 1975 and 2008 and whose surgical treatment was characterized by a large craniectomy without tonsillectomy with the patient in the sitting position, large opening of the fourth ventricle, and duraplasty.Key words: basilar impression, Chiari malformation, hindbrain herniation, craniovertebral decompression, syringomyelia. A esquistossomose mansônica encontra-se largamente distribuída no Brasil, constituindo-se em problema de saúde pública, levando-se em consideração os 12 milhões de brasileiros acometidos da moléstia !> 2 . No Nordeste do país apresenta-se de forma endêmica, de onde pode ser levada a outras áreas através das correntes migratórias populacionais, fato este muito frequente nessa região, particularmente, durante os períodos de longa estiagem. Desde que a neuroesquistossomose foi constatada entre nós, a partir da publicação do relato de Gama e Marques de Sá 8, em 1945, o número de casos descritos na literatura nacional sofreu aumento significativo. Tanto a forma encefálica como a medular já foram registradas, sendo o comprometimento da medula bem mais frequente 2 » 4 . Fundamentando-se em dados obtidos de necrópsias, admite-se que 20 a 30% dos portadores de esquistossomose apresentam lesões do sistema nervoso 1,2,7,15.Embora a radiculomielopatia esquistossomótica esteja clinicamente bem caracterizada, a observação de formas atípicas, lembrando outras patologias, tem sido referida 3,5,9. Tais manifestações sintomáticas, embora de comparecimento ocasional ou raro, podem induzir a erros diagnósticos. Por isso, achamos por bem relatar este caso, cuja sintomatologia está em acordo com a síndrome de Brown-Séquard.
RESUMO -Apresentamos 5 casos de parkinsonismo nos quais as imagens de ressonância magnética (RM) mostram alterações em estruturas do circuito dos núcleos da base (NB). Foram estudados 5 pacientes: 3 do sexo masculino e 2 do feminino nos quais as manifestações clínicas tiveram início nas faixas estárias de 5 a 52 anos. Os exames de RM foram realizados com equipamento de 1.5T. As imagens nas sequências em T2 evidenciaram hipersinal bilateral e simétrico nas seguintes topografias: exclusivamente na substância negra (3 casos), exclusivamente no globo pálido (1 caso) e envolvendo simultaneamente a substância negra, o globo pálido e as conexões nigroestriatais (1 caso). Em três casos havia dados sugestivos de parkinsonismo secundário: um pelo herbicida glifosato; outro após vacinação anti-sarampo; outro após período de coma por encefalite. Nos dois casos restantes, o diagnóstico clínico era de doença de Parkinson (DP). Entretanto, nesses dois casos, os dados da RM permitiram excluir o diagnóstico inicial de DP. A RN foi fundamental para identificar casos de parkinsonismo secundário, embora em alguns destes pacientes não tenha sido possível determinar os agentes etiológicos.PALAVRAS-CHAVE: ressonância magnética, substância negra, parkinsonismo secundário, doença de Parkinson, núcleos da base.Neuroimaging abnormalities in parkinsonism: study of five cases Neuroimaging abnormalities in parkinsonism: study of five cases Neuroimaging abnormalities in parkinsonism: study of five cases Neuroimaging abnormalities in parkinsonism: study of five cases Neuroimaging abnormalities in parkinsonism: study of five cases ABSTRACT -We report the brain magnetic resonance (MR) imaging abnormalities observed at the basal ganglia system of 5 patients (2 female and 3 male), who fulfilled the criteria of parkinsonism. The onset of parkinsonian syndrome ranged from 5 to 52 years old. All patients underwent MR exams with a 1.5T MR equipment. High field T2-weighted sequences disclosed hypersignal bilateral and symmetrically located exclusively at substantia nigra (3 cases), exclusively at globus pallidus (1case) and simultaneously at substantia nigra, globus pallidus and nigro-strital interconnections (1case). For three patients, the diagnose of secondary parkinsonism was supported by clinical data: the first had the onset of the symptoms after the exposure to an herbicide (glyphosate); the second after vaccination against measles; the third after coma due to encephalitis. For the other two patients, the onset of PS was progressive, resembling a typical idiopathic Parkinson's disease (PD) but the findings at the MR dimissed this initial diagnose. In this study, the contribution of neuroimaging was crucial to recognize secondary parkinsonism though the ethiological agents could not be determined in these patients.
